A programmable nanoplatform for inflammatory-state reprogramming in psoriasis treatment.

Psoriasis is a chronic, immune-mediated inflammatory skin disease. Although current treatments alleviate symptoms, they primarily focus on inflammation suppression through single-target approaches, which fail to comprehensively address the complex pathological imbalances. To overcome this limitation, we developed a programmable delivery system based on a nanocomposite material composed of Tannic acid (TA), Cerium (Ce), and Luteolin (Lut), integrated into an in situ gelling hydrogel platform (TA-Ce-Lut-Gel), enabling multi-dimensional treatment strategy for psoriasis by precisely modulating oxi

Superior sagittal sinus thrombosis with intracranial hemorrhage in a patient on infliximab: a case report.

BACKGROUND: Superior sagittal sinus thrombosis (SSST) is a rare but life-threatening form of cerebral venous thrombosis. Although several prothrombotic and autoimmune conditions are recognized risk factors, SSST occurring in patients with rheumatoid arthritis (RA) receiving tumor necrosis factor-alpha (TNF-α) inhibitor therapy remains exceptionally rare and poorly described in the literature. CASE PRESENTATION: We present a 59-year-old Caucasian female with RA treated with infliximab who developed extensive superior sagittal sinus thrombosis with concurrent intracranial and subarachnoid hemor

Quality standards for the diagnosis and management of ATTR and AL amyloidosis: a nominal group technique study.

BACKGROUND: Transthyretin (ATTR) and immunoglobulin light-chain (AL) amyloidosis are progressive conditions that can have debilitating effects on people. Delayed diagnosis and variable access to high-quality care continue to affect quality of life and prognosis. Clear and ambitious quality standards could drive consistent care and improve outcomes. METHODS: A modified nominal group technique was used to develop consensus-based quality standards for the diagnosis and management of ATTR and AL amyloidosis. Eighteen experts participated, including patient representatives and clinicians from high

Case Report: Light chain amyloidosis in a patient with low-grade B-cell lymphoma.

BACKGROUND: Light chain (AL) amyloidosis is typically associated with plasma cell dyscrasias but can rarely arise from low-grade B-cell non-Hodgkin lymphoma. Renal involvement may present with nephrotic-range proteinuria, acute kidney injury, edema, anasarca, and hypoalbuminemia. Recognizing lymphoma-associated AL amyloidosis is essential because diagnosis requires tissue confirmation, and treatment must target the underlying clonal B-cell disorder. CASE: We report a rare case of severe renal AL amyloidosis in a patient with known low-grade B-cell non-Hodgkin lymphoma previously treated with

Pathogenesis and molecular mechanisms of steroid-induced femoral head necrosis in children: a narrative review.

In recent years, significant progress has been made in the study of the pathological mechanisms and pathology of steroid-induced osteonecrosis of the femoral head (SONFH) in adults. However, research on the molecular mechanisms and clinical translation of SONFH in children remains relatively limited. This narrative review synthesizes recent studies and reviews the latest advances in etiology, pathogenesis, genetic susceptibility, and molecular mechanisms, with a particular focus on pediatric-specific features. Unlike adults, children's skeletons are still in the developmental stage, and their

Targeted protein degradation via chimeric antigen receptor (CAR)-mediated antigen endocytosis in T cells.

Targeted protein degradation (TPD) holds significant therapeutic potential over conventional biologics. However, existing platforms are constrained by their reliance on host protein degradation machinery and limited durability, rendering them inadequate for chronic diseases requiring sustained treatment. Here, we constructed a chimeric antigen receptor (CAR) incorporating the TNFR1 ectodomain as the antigen-binding domain (TNFR1T cells) and used CRISPR-mediated knockout of BCOR and ZC3H12A to generate persistent TNFR1TIF cells. TNF binding, endocytosis, and degradation were assessed in vitro b

Biologics in alopecia areata: promise, paradox, and the precision medicine frontier.

INTRODUCTION: Alopecia areata (AA) is a chronic, immune-mediated non-scarring hair loss affecting approximately 2% of the population. The approval of oral JAK inhibitors has established targeted systemic therapy for moderate-to-severe AA. Incomplete response rates, relapse upon treatment discontinuation, and safety contraindications, however, leave a substantial proportion of patients requiring alternative options. AREAS COVERED: This review evaluates the immunopathogenic rationale and available clinical evidence for biologic therapies in AA, including inhibitors of tumor necrosis factor (TNF

A Recurrent 3'UTR Variant in CAPN3 Highlights the Diagnostic Impact of Non-Coding Variants in Limb-Girdle Muscular Dystrophies.

OBJECTIVE: A substantial proportion of individuals with suspected genetic conditions, including neuromuscular disorders, lack a definitive molecular diagnosis. Non-coding variants, particularly those in the 3' untranslated region (3´UTR), which regulate mRNA stability, translation, and localization, may underlie part of this gap. We investigated the clinical, genetic, and functional relevance of a novel 3'UTR variant in CAPN3 in individuals with persistent hyperCKemia, most of whom also presented with proximal muscle weakness. METHODS: Clinical assessment, including muscle biopsy and muscle m

Nerandomilast treatment improves a case of progressive pulmonary fibrosis combined with severe plaque psoriasis: A case report from a clinical trial.

Psoriasis (PsO) and interstitial lung disease (ILD), especially progressive pulmonary fibrosis (PPF), are often associated with abnormal immune-activation and chronic inflammation. The management of patients with PsO existing with PPF remains challenging. There is an urgent need for treatment approaches that effectively manage the progression of both diseases and minimize treatment-related risks. We report the case of a 70-year-old male patient with PsO and ILD that progressed to PPF. The patient was initially managed for PsO with methotrexate, acitretin capsules, compound glycyrrhizin tablets

Host Metabolites Directly Suppress Fusarium Mycotoxin Biosynthesis and Enable Integrated Control of Rice Spikelet Rot Disease.

Rice spikelet rot disease (RSRD) threatens food safety by contaminating grains with diverse mycotoxins. This study revealed a climate-driven shift in pathogen dominance from Alternaria spp. to Fusarium spp., determining associated toxin chemotypes. Untargeted LC-MS metabolomics of wheat infected by Fusarium graminearum (wild-type vs ΔTri5) identified host-derived metabolites, 5-hydroxyindole-3-acetic acid (5-HIAA) and rutin, as key toxin-suppressing candidates. Both compounds strongly inhibited mycotoxin biosynthesis (>95%) with minimal antifungal activity. Mechanistic analyses showed that rut

Outcomes of Patients With AL Amyloidosis and End-Stage Renal Disease After Initiation of Therapy.

In AL amyloidosis, advanced renal disease can impact treatment options and cause significant morbidity. Patients with ESRD requiring dialysis are a group with currently unmet needs. In this collaborative study, we identified 362 patients from three European centers who initiated dialysis during the course of the disease. Median survival after the initiation of dialysis was 40 months. Heart involvement, higher BMPC infiltration at diagnosis, and lambda light chain type were associated with inferior survival. At the time of dialysis initiation, cardiac biomarkers were prognostic for postdialysis

Lipid nanoparticle-mediated transcription factor EB mRNA delivery for the treatment of metabolic dysfunction-associated steatotic liver disease.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a prevalent disorder characterized by excessive hepatic lipid accumulation, frequently progressing to steatohepatitis, fibrosis, and cirrhosis. Transcription factor EB (TFEB) coordinates lysosomal biogenesis, autophagy, and lipid catabolism, and therefore represents a potential therapeutic target; however, safe and direct strategies for selectively enhancing hepatic TFEB activity remain limited. Here, liver-targeted lipid nanoparticles (LNPs) were developed to deliver mRNAs encoding wild-type TFEB or the constitutively active
