Ultrasonographic evaluation of fibrous structure and mechanical characteristics of the infrapatellar fat pad in female patients with knee osteoarthritis: a cross-sectional study.

BACKGROUND: Fibrosis of the infrapatellar fat pad (IFP) is common in knee osteoarthritis (KOA); however, a comprehensive evaluation is lacking. This study aimed to use ultrasonography to investigate the fibrous structure and mechanical characteristics of the IFP in patients with KOA. METHODS: This cross-sectional study included patients with KOA, age-matched controls, and healthy young adults. Fifty knees with KOA were analyzed for Kellgren-Lawrence grade differences, whereas 50 knees with KOA, 14 age-matched controls, and 15 knees from healthy young adults were analyzed for between-group com

Semitendinosus Tendon Reconstruction for Recurrent Acromioclavicular Joint Instability Following Repeated Fixation Failure and Postoperative Trauma: A Case Report.

Recurrent instability following surgical treatment of Rockwood type III acromioclavicular (AC) joint injuries is uncommon but represents a challenging clinical problem, particularly after repeated fixation failure. A 67-year-old man presented with a Rockwood type III AC joint separation following a same-level fall. Initial suture-button fixation failed 45 days postoperatively. Revision hook plate fixation subsequently failed after a further traumatic event, and a third procedure involving rigid AC stabilization with dual plating also resulted in early mechanical failure. Definitive treatment w

Patient-specific management in high-risk revision total hip arthroplasty: mid-term outcomes of a next-generation constrained acetabular liner.

BACKGROUND: Managing recurrent instability in revision total hip arthroplasty (rTHA) requires a personalized approach, as traditional constrained liners often yield suboptimal results due to locking-mechanism failure and limited impingement-free range of motion. This study aimed to evaluate the mid-term clinical outcomes of a novel constrained acetabular liner (CAL) when utilized as a patient-specific solution for a high-risk cohort undergoing complex rTHA. By tailoring the constraint to individual biomechanical deficits, this next-generation device seeks to mitigate historic failure modes. M

Co-occurrence of retinoblastoma and oculocutaneous albinism in a Mexican patient: a case report.

INTRODUCTION: Retinoblastoma (RB) and oculocutaneous albinism (OCA) are genetically independent disorders whose coexistence is exceptionally rare, with an estimated theoretical frequency of 1 in 800 million births. METHODS: We report a 5-month-old Hispanic male with OCA who developed unilateral multifocal RB. Next-generation sequencing identified a de novo heterozygous pathogenic RB1 nonsense variant (c.1411C > T, p. Gln471*) and homozygous pathogenic OCA2 missense variant (c.2228C > T, p. Pro743Leu). RESULTS: The profound retinal hypopigmentation characteristic of OCA precluded standard pig

Postoperative alignment and patients reported outcome measures improvement after high tibial osteotomy: a significant interaction with osteoarthritis severity.

PURPOSE: Optimal postoperative alignment in high tibial osteotomy (HTO) remains controversial due to inconsistent target definitions and limited evidence supporting osteoarthritis (OA) severity-based strategies. This study evaluated clinical outcomes across postoperative weight-bearing line (WBL) categories stratified by OA severity, hypothesizing that overcorrection would be detrimental in early OA but not in advanced OA. METHODS: This retrospective single-center study included 181 knees that underwent open-wedge or closed-wedge HTO between 2017 and 2023 with a minimum 2-year follow-up. Knee

Complicated labral injuries in female patients with developmental dysplasia of the hip are associated with localized stress concentration in the superior acetabulum.

INTRODUCTION: Developmental dysplasia of the hip (DDH) alters joint mechanics and increases the risk of labral injury, compromising hip stability and leading to abnormal stress distribution and cartilage degeneration. However, the relationship between labral injury severity and joint loading stress in DDH remains unclear. This study investigated the effect of labral injury severity on stress distribution in DDH patients using computed tomography osteoabsorptometry (CT-OAM) and finite element analysis. MATERIALS AND METHODS: We analyzed 54 hips from 45 female patients undergoing joint-preservi

Addressing the challenges of including older people with osteoarthritis and multi-morbidity in digitally enabled trials: a mixed methods Study Within a Project (TIPTOE SWAP).

INTRODUCTION: Older adults, particularly those with multiple long-term conditions (MLTCs) and other intersectional characteristics, are under-represented in clinical trials. This includes digitally enabled trials as older people may be less likely to engage with digital methods. We embedded a 'Study Within a Project' (SWAP) in TIPTOE, a UK randomised controlled trial evaluating a self-management support intervention for older adults with knee and/or hip osteoarthritis and MLTCs. Osteoarthritis is a musculoskeletal condition that is more common with age. The aim of the SWAP was to understand th

Mitochondria-Targeted Supramolecular Nanoparticles Alleviate Cellular Senescence by Restoring Mitochondrial Homeostasis in Osteoarthritis.

Osteoarthritis (OA) is a common age-related degenerative joint disorder with no effective disease-modifying drugs. Cellular senescence and mitochondrial dysfunction that disrupt chondrocyte homeostasis are key drivers of OA initiation and progression. Here, we develop mitochondria-targeted supramolecular nanoparticles (EG NPs) self-assembled from Epigallocatechin-3-gallate (EGCG) and L-Glutamic acid (L-Glu), enabling efficient scavenging of mitochondrial reactive oxygen species (mtROS) through targeted delivery and ultimately restoring mitochondrial homeostasis. This leads to reduced DNA damag

Recurrent gene conversions and subcluster-specific variants are associated with Mycobacterium tuberculosis adaptation to intensive host pressure: a comparative genomic study.

BACKGROUND: Long-read sequencing has revealed complex variations, such as gene conversions in immunologically critical PE/PPE gene families, yet their role in Mycobacterium tuberculosis host adaptation remains unclear. METHODS: We performed a comparative genomic analysis across a gradient of host-imposed selective pressure: chronic within-host infection, latent household transmission, and a 21-year clonal outbreak in Bern, Switzerland. RESULTS: We observed a constrained accumulation of mutational events across distinct epidemiological settings, with rates ranging from 0.36 to 0.81 events per

ZnPc(Lys)5-mediated photodynamic inactivation and reduction of Candida albicans biofilm-associated cells in vitro and in vivo.

INTRODUCTION: Candida-associated stomatitis is a common oral mucosal condition, particularly among denture wearers, and is closely associated with persistent Candida albicans biofilms on denture surfaces. ZnPc(Lys)5 is an amphiphilic phthalocyanine-peptide nanomaterial that has previously been shown to form nanoscale supramolecular assemblies in aqueous environments. This study evaluated the efficacy of ZnPc(Lys)5-mediated antimicrobial photodynamic therapy (aPDT) in reducing denture-associated Candida biofilms and alleviating associated mucosal inflammation. METHODS: Candida albicans suspens
