Severe upper limb soft tissue infection with compartment syndrome following a scratch injury: a case report.

BACKGROUND: Phlegmon is a severe skin and soft tissue infection (SSTI) that can escalate rapidly if not promptly recognized and treated. Its early presentation may appear deceptively mild, potentially delaying recognition of its severity. Without timely intervention, phlegmon can extend into deeper tissues and result in life-threatening systemic complications. Although SSTIs are common, progression from a seemingly minor scratch injury to a fulminant upper extremity infection complicated by compartment syndrome and multiorgan failure is uncommon. This case is reported to highlight the potentia

Clinical Outcomes During Concurrent Dupilumab and Advanced Immune-Modifying Therapy: A Real-World Cohort Study.

BACKGROUND: The use of dual advanced therapy is increasing among patients with overlapping immune-mediated diseases. This is particularly relevant in patients with inflammatory bowel disease (IBD) and eosinophilic esophagitis (EoE), where dupilumab may be added to an existing IBD-directed biologic regimen. However, real-world safety data regarding concomitant dupilumab and advanced immune-modifying therapies commonly used in IBD remain limited. METHODS: We performed a retrospective cohort study using the All of Us Research Program Controlled Tier dataset. Adults receiving advanced immune-modi

Disseminated melioidosis presenting as pre-septal cellulitis and multifocal abscesses in an adolescent with type 1 diabetes mellitus: a case report.

Melioidosis, caused by Burkholderia pseudomallei, produces a wide range of clinical manifestations and may be difficult to diagnose in the pediatric population. We describe the case of a 12-year-old girl with poorly controlled type 1 diabetes mellitus that posed diagnostic challenges. Over 4 months, she experienced relapsing fever, multiple splenic abscesses with a 'target sign' on ultrasonography, right-sided preseptal cellulitis, and a left thigh ulcer. Repeated cultures of superficial swabs and blood yielded no growth of bacteria. Radiological mimicry and culture-negative results raised con

Rare Case of Necrotizing Fasciitis With Polyarteritis Nodosa in a Multi-Comorbid Patient.

Necrotizing fasciitis can mimic cellulitis in immunocompromised patients, delaying diagnosis. This case highlights the importance of early clinical suspicion despite low LRINEC score. Prompt surgical intervention, appropriate antibiotics, and recognition of underlying conditions like polyarteritis nodosa are critical to reduce morbidity and mortality in rapidly progressive soft tissue infections.

The surge in infectious complications of dental and periodontal infections in the Czech Republic between 2010 and 2022.

Dental and periodontal infections can lead to serious systemic complications through bacterial dissemination. While global awareness of these risks is increasing, data on the incidence and outcomes of such complications remain limited. This study aims to quantify the incidence of serious extraoral infectious complications associated with dental infectious foci, using dental procedures as markers of active oral infection, in the Czech Republic between 2010 and 2022. A retrospective analysis was conducted using data from the National Register of Reimbursed Health Services. Patients who underwent

Differences in prescription for impetigo and cellulitis across years in three European countries.

BACKGROUND: Cellulitis and impetigo account for many outpatients' visits and antibiotic prescriptions. They are mainly caused respectively by Streptococcus spp. and Staphylococcus aureus. French guidelines recommend amoxicillin as a first-line antibiotic for cellulitis and pristinamycin for severe impetigo, whereas the UK guidelines recommend flucloxacillin for both infections and no Belgian guidelines are published. OBJECTIVES: To investigate the prescription patterns of cellulitis and impetigo in general practice and to assess the potential impact of the implementation of national guideline

Surgical Management of Xylazine-Associated Wounds: A Retrospective Review and Algorithmic Approach.

BACKGROUND: Xylazine is a veterinary sedative that is added to illicit fentanyl to prolong its euphoric effects. When injected subcutaneously, xylazine causes large, necrotic wounds. In 2023, the White House designated fentanyl adulterated with xylazine as an emerging US public health threat. To date, the optimal surgical management of xylazine-associated wounds is not well understood. METHODS: A total of 66 inpatient plastic and orthopedic surgery consultations for xylazine-associated wounds at an academic center in the northeast United States between 2018 and 2023 were reviewed. Following I

An unusual case of acute pseudogout in the hand masquerading as cellulitis.

RATIONALE: The wrist joint represents one of the most commonly affected anatomical sites in calcium pyrophosphate deposition disease, yet it remains underreported in the literature, frequently leading to diagnostic errors. PATIENT CONCERNS: We present a case of acute pseudogout of the wrist whose symptoms mimicked cellulitis. Our patient presented with a swollen hand, which was initially treated as acute cellulitis with antibiotics, but without any improvement. DIAGNOSES: A final diagnosis of acute chondrocalcinosis was made based on multiple intra-articular calcifications. OUTCOMES: The pa
