We present the case of a 980-g, female, premature infant with a late-onset maternofetal sepsis, of which the first sign was a diaper-shaped cellulitis. This dermatologic sign in a premature infant is very unusual and needs prompt investigation. Late-onset group B Streptococcus infection can be manifested by a cellulitis-adenitis syndrome.

Group A streptococci (GAS) are gram positive cocci that can be divided into more than 100 M-serotypes or emm types based on their M proteins. Their virulence is related directly to the M protein on the cell surface that inhibits phagocytosis. Currently there are 93 validated M serotypes of Streptococcus pyogenes, Lancefield group A streptococcus (GAS), and >130 emm genotypes. A marked increase in the number of non-typable GAS isolates (2 % in 2000, 4 % in 2001 and 9 % in 2002) from invasive disease cases referred to the authors' reference laboratory was noted during 2000-2002. The superantigens cause a massive polyclonal activation of T-cells, producing an immense liberation of proinflamatory cytokines, which induces the clinical data of toxic shock syndrome. In international studies the administration of polyclonal intravenous gammaglobulin has been observed to diminish the mortality 50 to 20%. But at the present it has not been reported in Mexico the clinical effectiveness of this therapeutic modality in toxic shock syndrome. We report three cases of toxic shock syndrome treated with gammaglobulin intravenous, and we describe their favorable clinical evolution.

DESIGN: Data for outpatients with a diagnosis of diabetes were abstracted from electronic records. The number of hospitalizations from 1998 through 2001 was determined after linking our dataset with a statewide discharge dataset. This report presents the case of a 20-year-old female who presented with features of right panophthalmitis with secondary orbital cellulitis masquerading as an orbital tumour. This presented a diagnostic difficulty to several ophthalmologists. An orbital ultrasound scan revealed an underlying orbital mass, which on histology was discovered to be a well differentiated invasive large cell keratinizing squamous cell carcinoma. This report further emphasizes the value of ultrasound scan in detecting orbital tumours.

The subsequent evolution of the disease required extensive investigations which provide the definitive diagnosis of non-Hodgkin, diffuse B-cell lymphoma, with extranodal orbital involvement. Necrotising fasciitis is a severe, life-threatening soft tissue infection. It produces an extensive cellulitis with severe involvement of subcutaneous tissue, fascia, muscle or both, resulting in necrosis of the tissue. All age groups, including neonates, can be affected.
