Continuum of care in hospitalized patients with opioid or stimulant use disorder and infectious complications of drug use-substance use/infectious disease integrated clinic compared to treatment as usual to prevent infection-related readmission (CHOICE-STAR): a study protocol for an effectiveness-implementation randomized controlled trial.

BACKGROUND: People who inject drugs (PWID) experience high rates of serious injection-related bacterial and fungal infections (SIRI), including cellulitis, osteomyelitis, and endocarditis. These infections often require prolonged antibiotic treatment and result in frequent rehospitalizations, with over 50% of patients readmitted within 1 year. Few evidence-based interventions exist to optimize continuity of care for addiction, management, and prevention of SIRI following hospitalization. METHODS: CHOICE-STAR is a hybrid type 1 randomized effectiveness-implementation trial, guided by the Explo

Postoperative oral cephalexin and metronidazole effectiveness in preventing complications in obese pregnant women undergoing cesarean delivery: A systematic review and meta-analysis.

This meta-analysis aimed to evaluate the effectiveness of adding postoperative oral cephalexin and metronidazole to standard preoperative prophylaxis in preventing surgical site infections (SSI) among obese women undergoing cesarean delivery. We systematically searched PubMed, EMBASE, and Cochrane Central Register of Controlled Trials for randomized controlled trials. Studies involving obese women undergoing cesarean delivery with the intervention of postoperative oral cephalexin plus metronidazole were included. Data were analyzed using a random-effects model to pool risk ratios (RRs) with 95

Necrotizing fasciitis of the face triggered by a neglected hordeolum: a case report and literature review.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection that rarely involves the periorbital region. We report an unusual case of periorbital NF caused by methicillin-sensitive Staphylococcus aureus (MSSA) in a previously healthy 48-year-old woman, originating from a neglected, recurrent hordeolum-an otherwise benign eyelid condition. The infection resulted in an extensive fullthickness defect of the upper eyelid and adjacent temporal area, presenting major reconstructive challenges. Reconstruction was performed using a radial forearm free flap combined with

Cutaneous Adverse Effects in Patients Treated with BTK Inhibitors.

Bruton's tyrosine kinase (BTK) inhibitors have revolutionized the treatment landscape for patients with indolent lymphoid malignancies such as chronic lymphocytic leukemia (CLL) and mantle cell lymphoma (MCL). The most common adverse events include cardiac arrhythmia, bleeding, infection, diarrhea, arthralgias, hypertension, and skin changes. Second-generation BTK inhibitors, e.g., acalabrutinib and zanubrutinib and the non-covalent BTK inhibitor pirtobrutinib, are less toxic than the first-generation BTK inhibitor ibrutinib. The most common toxic skin symptoms related to BTKi treatment includ

Necrotizing Fasciitis Camouflaging As Cellulitis: A Case Report Highlighting Diagnostic Pitfalls.

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection that is often misdiagnosed in its early stages due to clinical similarities with cellulitis. Early recognition and prompt surgical intervention are essential to reducing morbidity and mortality. This report presents one case of NF initially manifesting as cellulitis in a patient with multiple risk factors, including intravenous drug use and recent incarceration. Despite initial antibiotic therapy and transient clinical improvement, the patient experienced worsening pain and systemic symptoms. Imaging id

Improvement of Steroid-Dependent Encephalopathy Clinically Diagnosed as Paraneoplastic Neurological Syndrome in Onconeural Antibody-Negative Sigmoid Colon Cancer after Laparoscopic Tumor Resection.

INTRODUCTION: Paraneoplastic neurological syndrome (PNS) is a rare complication of malignancy caused by immune-mediated mechanisms. Although characteristic onconeural antibodies are useful for the diagnosis of PNS, 30%-40% of cases are seronegative, which makes recognition difficult. We present a case of sigmoid colon cancer complicated by onconeural autoantibody-negative PNS which caused impaired consciousness. The patient's symptoms recurred with steroid tapering but improved after resection of the primary tumor and radiofrequency ablation (RFA) of the metastasis. This case highlights the im

A toothache gone wrong: an unexpected case of perioperative lactation ketoacidosis.

BACKGROUND: Lactation ketoacidosis is a rare metabolic complication of breastfeeding, characterised by high anion gap metabolic acidosis due to elevated levels of ketone bodies in the blood or urine. Common symptoms include nausea, vomiting, abdominal pain and general malaise. The typical triggers for lactation ketoacidosis are intensive exercise, a ketogenic diet, reduced food intake or fasting, often associated with an attempt to lose weight. CASE PRESENTATION: We report the case of a 31-year-old woman who had been breastfeeding continuously for two years, including throughout her most recen

Rebuilding After Weight Loss: A Nationwide Retrospective Cohort Study on the Outcomes of Autologous Breast Reconstruction in Post-Bariatric Patients.

PURPOSE: Obesity is a known risk factor for complications in autologous breast reconstruction, yet few studies evaluate outcomes after massive weight loss. As the incidence of bariatric procedures increase, more post-weight loss patients will seek reconstruction. This study compares surgical outcomes in patients with and without bariatric surgery, addressing a critical area in reconstructive planning. METHODS: A retrospective cohort study was conducted using the TrinetX network, querying a database of 133 million US medical records over 20 years. Patients undergoing autologous breast reconstr

Imaging of acute musculoskeletal infections in children and their differential diagnoses.

Musculoskeletal complaints are a frequent cause of emergency department visits in pediatric patients. While many of these visits are due to acute trauma, infection is also common and can cause substantial morbidity. This article provides a review of acute pediatric musculoskeletal infections with an additional emphasis on important differential diagnoses to consider. The scope includes cellulitis, abscess, necrotizing soft tissue infections, pyomyositis, infectious tenosynovitis, septic bursitis, septic arthritis, and osteomyelitis. Important differential considerations for these musculoskelet

Cardiovascular Signs and Symptoms: Edema.

Lower extremity edema results from an imbalance between capillary hydrostatic pressure, oncotic pressure, and lymphatic drainage, leading to fluid accumulation in the interstitial space. Most cases are bilateral, typically due to systemic causes of volume overload such as heart failure, cirrhosis, or kidney dysfunction. Unilateral edema is more often due to localized pathology, including deep venous thrombosis, cellulitis, or trauma. Edema is a common presenting symptom and may be associated with pain, reduced mobility, and functional impairment. Initial evaluation should include a history and
