Herpes Zoster Induced by the Shingrix Vaccine: A Case Report and Literature Review.

Herpes zoster (HZ) is a dermatological condition that is caused by the reactivation of the varicella zoster virus (VZV). The recombinant zoster vaccine (RZV; Shingrix, GlaxoSmithKline Biologicals, Rixensart, Belgium) is approved in the United States for the prevention of HZ reactivation in adults aged >50 years. We present a case of a 50-year-old immunocompetent female patient who developed HZ within the first 72 hours post Shingrix vaccination. The patient was evaluated by their primary care provider for a painful, vesicular rash, along with headache and fatigue. A diagnosis of Shingrix vacci

A modular nanoparticle display strategy for varicella-zoster virus gE based on a licensed protein scaffold.

Varicella-zoster virus (VZV), the cause of shingles, remains a significant health issue worldwide, particularly among aging populations. The recombinant zoster vaccine (Shingrix, RZV) is the only approved vaccine in many countries and has demonstrated >90% efficacy with durable protection lasting over a decade, highlighting the value of subunit vaccines targeting VZV glycoprotein E (VZV gE). Although RZV provides durable and highly effective protection, alternative vaccine platforms remain important for advancing antigen design and improving immune presentation. Advances in nanoparticle techno

Brainstem encephalitis and anti-GQ1b antibody syndrome triggered by varicella-zoster virus presenting as intractable hiccups: a first reported case.

BACKGROUND: Varicella-zoster virus (VZV) reactivation without rash (zoster sine herpete, ZSH) is a known cause of neurological disease. However, its association with an overlap syndrome of brainstem encephalitis and anti-GQ1b antibody syndrome is exceedingly rare, and its presentation with intractable hiccups as the initial symptom has not been previously reported. CASE PRESENTATION: We describe a 35-year-old male whose illness began with isolated intractable hiccups, which progressed to include brainstem signs such as cranial neuropathies and autonomic failure. Diagnosis was confirmed by a tr

Impact of immunization strategies, public health and social measures on the incidence of varicella among children aged 0-14 years in Quzhou City.

This study evaluates the impact of different immunization strategies and public health and social measures (PHSMs) on varicella incidence among children in Quzhou. Data on varicella cases among children aged 0-14 y in Quzhou from 2006 to 2022 were collected. An interrupted time-series (ITS) model was used to analyze trends in monthly varicella incidence during the implementation of the single-dose and two-dose vaccination strategies and the PHSMs. ITS analysis indicated that, following the introduction of the two-dose strategy, the varicella monthly incidence rate decreased immediately by 8.19

Exploring the impact of reimbursement ratios on willingness to vaccinate: A mixed-effects modeling approach using panel data.

Vaccination remains one of the most cost-effective methods for disease prevention. However, utilization of self-paid vaccines, including EV71, varicella, influenza, and DTaP-IPV-Hib in this study, remains insufficient among children under six in China. To investigate the determinants of willingness to vaccinate (WTV) for self-paid vaccines and assess cost-WTV heterogeneity, we conducted structured-questionnaire surveys with 2212 randomly selected households in Hangzhou, each with at least one child under six. Multiple regression analysis was used to identify the key determinants of WTV, and a

Clinical epidemiology of herpes zoster and postherpetic neuralgia in Barcelona (Spain). A single-center study.

INTRODUCTION: Varicella-zoster virus is an alphaherpesvirus that causes two diseases in humans: chickenpox, the primary infection, and herpes zoster (HZ). Although HZ incidence has been well-documented, relatively little is known regarding the clinical-epidemiological characteristics of HZ prodromal pain, acute pain, and subacute pain. The aim of the study was to analyse the clinical-epidemiological characteristics of HZ and postherpetic neuralgia (PHN) in cases diagnosed between 2007 and 2020. METHODS: We conducted a clinical-epidemiological study of cases of HZ and PHN diagnosed between 200

Disseminated Cutaneous Herpes Zoster in an Immunocompetent Patient: A Diagnostic Challenge.

Disseminated cutaneous herpes zoster (DCHZ) typically occurs in immunocompromised individuals. However, rare cases arise in immunocompetent adults. These cases may initially resemble benign dermatoses or be difficult to distinguish from other systemic rashes when overlapping risk factors obscure diagnosis. Early recognition is essential to prevent visceral or neurologic complications.  A 32-year-old immunocompetent male with childhood varicella presented with a 10-day history of a pruritic vesicular rash, right ear fullness, and viral prodromal symptoms. He had recent diving exposure five days

Rapidly progressive varicella zoster virus vasculopathy in a chemotherapy- and steroid-immunosuppressed patient with refractory diffuse large B-cell lymphoma: diagnostic and therapeutic challenges.

Varicella zoster virus (VZV) vasculopathy is a rare but severe neurological complication that frequently results in a reversible cerebral vasoconstriction syndrome (RCVS). Diagnosis is often delayed because early manifestations are nonspecific and radiologic findings often overlap with those of other cerebrovascular disorders. We describe the fatal case of a heavily pretreated 60-year-old man with refractory diffuse large B-cell lymphoma undergoing evaluation for chimeric antigen receptor (CAR) T-cell therapy. The patient developed multifocal intracranial arterial stenosis and an acute infarct

Genomic comparison reveals single-nucleotide polymorphic sites and attenuation-associated site combinations specific to Chinese live attenuated varicella-zoster virus vaccines.

The live-attenuated varicella-zoster virus (VZV) vaccine, mainly based on the Oka strain, is essential for the prevention and control of chickenpox (varicella). Although genomic profiles of internationally prevalent Oka vaccine strain (vOka)-derived preparations have been reported, the comprehensive single-nucleotide polymorphism (SNP) profiles of multiple commercially available Chinese vOka-derived vaccines are still lacking. We performed next-generation sequencing (NGS) on five Chinese-manufactured VZV vaccines, comparing genomic variations against the wild-type parental Oka (pOka) strain an

A kidney transplant recipient with shingles and necrotizing bacterial superinfection: a case report.

We report a 45-year-old kidney transplant recipient who developed primary varicella zoster virus infection complicated by monomicrobial necrotizing skin and soft tissue infection caused by Pseudomonas aeruginosa. Progressive facial and oropharyngeal edema led to airway compromise requiring endotracheal intubation. Blood and wound cultures grew Pseudomonas aeruginosa, and targeted therapy with meropenem resulted in clinical improvement. The patient recovered with preserved allograft function but was left with residual facial nerve palsy. Pseudomonas aeruginosa-associated monomicrobial necrotizi

Evaluation of IVIG for Prevention of Infections Occurring On-study and Treatment of Infection of Solid Organ Recipients With Secondary Antibody Deficiency: A Phase 2 Randomized Clinical Trial.

BACKGROUND: There is a paucity of data to guide the management of posttransplant secondary antibody deficiency (SAD). We aimed to evaluate the role of IVIG in solid organ recipients with SAD. METHODS: A phase 2, randomized, multicenter, open-label study of solid organ recipients with severe infection and SAD defined as serum IgG <600 mg/dL. The study evaluated the efficacy of adding a fixed protocol of IVIG early after detection of SAD to standard of care (SOC) for the prevention of infection occurring on-study and the treatment of infections, versus SOC. We also assessed reconstitution of hu

Varicella Zoster Virus Infection: Clinical Features, Molecular Pathogenesis, Treatment, and Prevention.

Varicella zoster virus (VZV) is a ubiquitous human herpesvirus that establishes lifelong latency and causes herpes zoster (HZ) upon reactivation, posing a growing clinical challenge in aging populations. The incidence of HZ increases sharply with age and immunocompromised states, and its clinical burden extends well beyond cutaneous disease to include postherpetic neuralgia, neurological complications, and systemic involvement. At the mechanistic level, HZ reflects a dynamic interplay between viral reactivation, host immune surveillance, and virus-induced neuronal injury. Despite substantial p

A case of uveitis and retinal vasculitis induced by varicella-zoster virus: vitrectomy treatment and literature review.

BACKGROUND: Infectious uveitis can be induced by a variety of factors, including viral, bacterial, and parasitic infections, among others. Among these, viral infections are the most common cause of infectious uveitis. Traditional diagnostic methods have limited sensitivity and are often cumbersome, which restricts their ability to accurately identify the viral pathogens responsible. As a result, there is still no gold-standard diagnostic tool for detecting uveitis. With advancements in molecular diagnostic technologies, metagenomic next-generation sequencing (mNGS) has become widely used in cl
