Postherpetic neuralgia (PHN) is a persistent neuropathic pain condition resulting from varicella-zoster virus reactivation, yet remains inadequately treated. Here, we elucidate a pathogenic function of Serpin Family E Member 1 (SERPINE1) in neuropathic pain using a resiniferatoxin (RTX)-induced neuropathic pain to simulate PHN. RTX administration triggered sustained mechanical allodynia and thermal hyposensitivity, accompanied by damage in the dorsal root ganglion (DRG) and sciatic nerve. Transcriptomic analysis identified SERPINE1 as a prominently upregulated gene in DRG following RTX challen

BACKGROUND: Varicella-zoster virus infection poses a significant risk to pediatric hematopoietic stem cell transplant recipients owing to compromised T-cell-mediated immunity. Although varicella-zoster virus-specific T-cell-mediated immunity is crucial for preventing reactivation, its reconstitution following allogeneic hematopoietic stem cell transplant in pediatric patients remains poorly characterized.
METHODS: This prospective study assessed varicella-zoster virus-specific T-cell-mediated immunity in 32 pediatric allogeneic hematopoietic stem cell transplant recipients pre-transplant and

During preparedness activities in Senegal to the 2024 Mpox Public Health Emergency of International Concern, a study was conducted to assess the prevalence of Varicella-Zoster virus among patients suspected of having Mpox. Samples, including skin swabs, serum, and nasopharyngeal swabs, were collected from 103 patients who presented with Mpox-like symptoms. Molecular testing via qPCR revealed that 30.1% of patients tested positive for herpesviruses, whereas no Mpox cases were detected. Common symptoms include fever, skin rash, headache, and myalgia, which closely resemble Mpox symptoms, increas

The recent surge in unaccompanied minor asylum seekers (UMAS) arriving in Switzerland presents significant public health challenges, particularly regarding immunization. Many of these adolescents lack vaccination documentation, requiring catch-up vaccination strategies. This study aimed to evaluate immunity against tetanus, hepatitis B, measles, mumps, rubella and varicella among UMAS upon arrival in Switzerland, to optimize vaccination strategies, and provide protection. This retrospective observational study included 100 UMAS patients followed at Geneva University Hospitals in 2023. Patients

Elsberg syndrome (ES) is an acute-to-subacute onset, lumbosacral radiculitis, with or without myelitis, confined to lower spinal cord, often caused by herpes simplex virus (HSV)-2. We report a case of a 70-year-old lady with ES due to varicella zoster virus (VZV) reactivation, presenting as herpes zoster in right side L5-S2 dermatomes, with radiculitis in the respective roots and myelitis in the conus medullaris region. The diagnosis is often made by considering the clinical picture, as well as the investigations, such as magnetic resonance imaging (MRI) of the spine, cerebrospinal fluid (CSF)

A 28-year-old male with paroxysmal nocturnal haemoglobinuria (PNH) presented with headache, nasal bridge discomfort and haemoglobinuria. He developed a macular-papular rash which rapidly progressed into purpura, necrosis and peri-orbital oedema. Investigations demonstrated severe haemolytic anaemia and acute parvovirus B19 infection. A skin biopsy confirmed extensive necrotising vasculitis with intravascular thrombi, affecting the dermal and subcutaneous vessels. Eculizumab, corticosteroid and intravenous immunoglobulin were given, and within 24 h, symptoms markedly improved. The patient later

INTRODUCTION: Herpes Zoster Oticus is etiologically associated with the reactivation of the Varicella-Zoster virus in the geniculate ganglion. Its primary manifestation includes pain, vesicles in the external auditory canal, and peripheral facial paralysis. The prognosis of the syndrome remains under scrutiny and debate, fluctuating based on symptom severity and clinical manifestation. The scarcity of articles accurately correlating clinical presentation and prognosis, coupled with individual factors and the syndrome's rarity, constrains more comprehensive analyses.

BACKGROUND: Herpes zoster (HZ), or shingles, results from the reactivation of latent varicella-zoster virus and poses a significant health burden and immunocompromised adults are at higher risk of HZ and its complications. In 2018, the recombinant zoster vaccine (RZV, Shingrix®) was strongly recommended by the National Advisory Committee on Immunization (NACI) for immunocompetent adults aged 50 years and older. Since then, evidence has accumulated on the use of RZV in immunocompromised adults and in 2021, Health Canada expanded the authorization of RZV to adults 18 years of age and older who a

Varicella is an acute, highly contagious infectious disease that primarily impacts young children with approximately 90% of cases occurring under the age of 15. Due to the high incidence of varicella infection, the potential for serious complications, and the significant economic and public health burden, vaccination of toddlers against varicella is included in routine vaccination programs in many countries. This study assessed the immunogenicity and safety of the investigational varicella vaccine (VNS) compared to a licensed, current standard-of-care vaccine. In this phase II, observer-blind,

Herpesviruses are highly species-specific, having co-evolved with their respective hosts over millennia. This evolutionary relationship has led to the development of highly specialized immune evasion strategies, enabling these viruses to establish lifelong infections. Periods of immunosuppression can lead to severe disease, such as graft rejection, cancer, neurodegenerative disease and autoimmune diseases, which have been linked to herpesvirus infections. Consequently, developing strategies to prevent herpesvirus-associated diseases is a critical public health priority. However, varicella zost

BACKGROUND: Inborn errors of immunity (IEIs) affecting thymic cell development or function can result in thymic aplasia or hypoplasia, autoimmunity, and the generation of neutralizing autoantibodies (Auto-Abs) targeting type I interferons (IFNs) (AAN-I-IFNs). FOXN1 is a master transcriptional regulator of thymic epithelial cells and the impact of the FOXN1 variants spans a spectrum from pathogenic to benign. In pediatric patients with heterozygous FOXN1 variants the full spectrum of clinical and immunological determinants remains to be defined.
METHODS: Using a luminex multiplex immunoassay,

Herpes zoster, also known as shingles, is caused by the reactivation of the Varicella-Zoster Virus (VZV), which remains dormant in the cranial sensory nerve ganglia and spinal dorsal root ganglia after the initial VZV infection is cured in a patient. When it reactivates, it travels down the nerve to the skin, causing a painful rash and blisters. The pathophysiology involves viral replication in nerve cells, inflammation, and subsequent skin lesions. Herpes zoster can have a significant impact on an individual's physical, psychological, and social well-being. While the condition is usually not

The use of high-power lasers for facial rejuvenation has been growing every year. In particular, the Thulium laser in non-ablative mode has gained prominence due to its safety, postoperative comfort, and wide clinical indication. This report details the first documented case of a varicella-zoster virus expression following the use of a non-ablative fractional 1927 nm Thulium laser for facial skin aging and pigmentated lesions. The treatment proposed involved a combination of phototherapies as an adjunctive local strategy, alongside systemic acyclovir therapy. In nine days, herpetic lesions and

A study was conducted to assess seroprevalence against varicella zoster virus(VZV) and rubella virus among health care workers providing care to antenatal patients and newborn babies. A cross-sectional study enrolled healthcare workers(HCWs) working in the departments of Obstetrics and Gynecology and Pediatrics, collecting sociodemographic details along with a request for 3 mL blood samples. Serum titers of IgG antibodies against VZV and rubella were measured by ELISA(Enzyme-Linked Immunosorbent Assay). Two-hundred HCW participants were enrolled in the study. The average age of the study parti

BACKGROUND: Varicella infection during late pregnancy is uncommon but can lead to serious complications for the neonate, including neonatal varicella and, rarely, early-onset herpes zoster. Standard management includes postnatal separation, administration of intravenous immunoglobulin, and antiviral treatment when indicated. However, prophylaxis may not always prevent infection or reactivation, especially in cases where maternal immunity is absent.
CASE PRESENTATION: In March 2024, a 35-year-old Iranian pregnant woman who developed varicella at 37 weeks of gestation presented. She was treated

Varicella-zoster virus (VZV) establishes lifelong latency following primary infection and can reactivate later in life to cause herpes zoster (HZ) and its debilitating complication postherpetic neuralgia (PHN). The implementation of varicella vaccinations has profoundly altered the epidemiological landscape of VZV. This narrative review examines the current literature to evaluate the impact of these vaccination programs on the incidence of HZ and PHN. Early mathematical models based on the "exogenous boosting" hypothesis predicted a substantial surge in adult HZ due to reduced exposure to circ

SUMMARYChildcare outside the home is a common experience for many children in high-income countries. It is associated with an increase in the incidence of infectious diseases-not just for the child but also for their parents and other household members. In this review, we explore this phenomenon from multiple angles, combining age at first infection, maternal antibody dynamics, seroepidemiology, cohort studies, and outbreak reports to understand the relationship between the immune systems of children starting childcare and the pathogen milieu they encounter. We consider the interaction between

The uvula is a soft tissue that freely hangs down from the soft palate posterior margin. It prevents the soft palate from entering the nasopharynx during sneezing and coughing. The uvula contains mucus glands, which help to keep the throat moist and facilitate swallowing. The uvula plays a vital role in speech and helps deflect foreign bodies accidentally entering the oral cavity by triggering the gag reflex on touch. Deviation of the uvula is often an overlooked sign by oral physicians. Deviation of the uvula is linked to viral illnesses such as herpes zoster (HZ), coronavirus disease 2019, o
