Comparative analysis of the impact of chickenpox and herpes zoster vaccination in Belgium under two different exogenous boosting mechanisms.

BACKGROUND: Chickenpox (CP) and herpes zoster (HZ), both caused by the varicella-zoster virus (VZV), present a significant public health burden in unvaccinated populations. Universal CP vaccination has been long debated due to concerns about a potential increase in HZ incidence as a consequence of the exogenous boosting hypothesis. METHODS: We performed a cost-utility analysis on two deterministic compartmental dynamic transmission models, each of which employed a different underlying mechanism of exogenous boosting: temporal or progressive immunity. We considered four vaccination strategies:

Changes in childhood vaccine hesitancy during the COVID-19 pandemic: a retrospective study based on data from Turkey.

BACKGROUND: Vaccine refusal is a growing public health concern, threatening herd immunity and the control of vaccine-preventable diseases. The COVID-19 pandemic has influenced healthcare delivery and vaccine refusal behavior as a proxy for vaccine hesitancy. This study aimed to assess changes in childhood vaccine refusal behavior during the pandemic in Turkey. METHODS: We conducted a retrospective observational study including 2,786 children born between 2017 and 2022 who reached 24 months of age and had complete vaccination records from primary healthcare. Children were categorized as pre-pan

Fatal intracerebral hemorrhage in a pregnant woman following varicella zoster virus infection: a rare neurological complication.

Varicella zoster virus (VZV) infection is usually mild in childhood but can be severe in adults, particularly in pregnancy, leading to significant complications. Neurological complications are relatively uncommon; in particular, vasculopathy resulting in stroke and hemorrhage is very rare. This case report describes a rare and lethal complication of primary varicella zoster virus (VZV) infection in a pregnant woman at 34 weeks of gestation. The patient, who presented with vomiting and altered sensorium, was diagnosed on postmortem with intracerebral hemorrhage (ICH) due to VZV-induced vasculit

Herpes Zoster of the Maxillary Division of the Trigeminal Nerve Presenting as Endodontic Post-treatment Pain: Case Report and Review of the Literature.

Herpes zoster (HZ), caused by the reactivation of varicella zoster virus, presents a major diagnostic challenge in dentistry, particularly when involving the trigeminal nerve. The prodromal symptoms of HZ can closely resemble endodontic pain, leading to misdiagnosis and delayed appropriate treatment. This case report described a 54-year-old male who presented to the emergency dental clinic with severe unilateral, radiating maxillary pain originating from posterior to anterior teeth. Initially diagnosed with symptomatic apical periodontitis on a previously endodontically treated tooth #3, the p

Safety and immunogenicity of varicella-zoster vaccination in pediatric heart transplant recipients.

BACKGROUND: Pediatric heart transplant recipients are at increased risk of morbidity and mortality from vaccine-preventable infections due to long-term immunosuppression. Although live vaccines have traditionally been contraindicated post-transplant, emerging evidence supports their administration in selected solid organ recipients. This study evaluated the safety and immunogenicity of varicella-zoster virus (VZV) vaccination in pediatric heart transplant recipients. METHODS: We retrospectively reviewed all pediatric heart transplant recipients who received a post-transplant VZV vaccination b

Outbreaks due to measles, mumps, rubella and varicella in schools: a systematic review of the literature.

Several decades into routine pediatric vaccinations worldwide, many vaccine-preventable diseases (VPDs) remain endemic and cause outbreaks with serious morbidity and fatalities, even in countries with long-term implemented pediatric vaccination programs. Schools play a critical role during childhood and adolescence not only for educational purposes, but also for the socialization and overall well-being of children and adolescents. Participation in most school activities usually requires close proximity and interaction among students in indoor settings, which facilitates the transmission of sev

Integrated Western and Traditional Chinese Medicine Approaches for Herpes Zoster and Post-Herpetic Neuralgia: A Narrative Review.

BACKGROUND: Herpes zoster (HZ) and postherpetic neuralgia (PHN) impose a substantial global burden, particularly in older and immunocompromised adults. Western therapies (antivirals, vaccination, and analgesics) reduce viral replication and acute pain but frequently fail to prevent chronic neuropathic pain or fully restore function. Traditional Chinese Medicine (TCM) is widely used as an adjunctive treatment in routine practice, yet its integration with Western care remains variably implemented and synthesized. OBJECTIVE: To provide a clinically oriented narrative synthesis of epidemiology, m

Severe Varicella Pneumonia in an Immunocompetent Adult Requiring Intensive Care Management: A Case Report.

Varicella is usually a mild childhood disease; however, primary infection in adults may lead to severe complications, particularly varicella pneumonia. We report the case of a 32-year-old previously healthy male admitted to the intensive care unit with acute hypoxemic respiratory failure occurring three days after the onset of a diffuse vesicular rash. Chest CT demonstrated bilateral ground-glass opacities with focal consolidation consistent with viral pneumonia. The diagnosis was established based on typical cutaneous findings, epidemiological exposure, and compatible imaging results. Early t

Varicella-Zoster Virus Infection in an Immunocompromised Patient With Seizure-Like Activity and Septic Shock: A Case Report.

Varicella-zoster virus (VZV) is well-known for causing primary chickenpox infection; however, it remains dormant in the central nervous system and can be reactivated during periods of decreased cell-mediated immunity. In addition to shingles, which is the most common form of reactivation, other rare secondary complications have been observed, including meningitis and encephalitis. Immunocompromised patients, such as those with acquired immunodeficiency syndrome (AIDS), are at an increased risk of such complications. We present the case of a 58-year-old male with AIDS (CD4 111 cells/mm3) who de

Facial Herpes Zoster With Hutchinson's Sign Complicated by Secondary Bacterial Superinfection: A Case Report.

Herpes zoster ophthalmicus results from the reactivation of latent varicella zoster virus involving the ophthalmic branch of the trigeminal nerve and may lead to severe ocular complications. Hutchinson's sign, defined by vesicular or crusted lesions on the nasal tip, is a key predictor of ocular involvement.  We describe a 57-year-old woman with no relevant past medical history who presented with a 15-day history of pruritus and burning in the right eye. She was initially misdiagnosed with blepharitis and later periorbital cellulitis, receiving topical tobramycin and systemic antibiotics witho

Addressing the crises of declining vaccination rates and measles resurgence by promoting health literacy.

Pediatric vaccination rates are declining in the United States, with the Centers for Disease Control and Prevention (CDC) reporting decreases in immunization coverage against measles-mumps-rubella (MMR), diphtheria-tetanus-acellular pertussis (DTaP), polio, and varicella. The recent measles outbreaks have resulted in tragic outcomes, highlighting the urgent need for clinician concern and proactive measures to prevent illness, hospitalizations, missed school and caregiver workdays, and deaths. According to the World Health Organization, MMR vaccination rates below 95% compromise herd immunity,

Herpes Zoster of the Nipple: A Rare Diagnostic Challenge.

The breast constitutes an essential component of a woman's identity, body image, and self-esteem. The objective of this article is to present a rare case of breast skin pathology and to examine its differential diagnosis. A fifty-year-old woman consulted our department for a burning sensation in her right breast in combination with a rash involving the nipple-areola complex. The patient received treatment for evolving bacterial mastitis; however, a zosteriform vesicular rash subsequently developed over the right scapular region. The diagnosis of varicella zoster virus infection was confirmed,

Herpes Zoster in a 9-Month-Old Infant Following Maternal Varicella Infection: A Rare Case Report.

Herpes zoster is caused by reactivation of latent varicella-zoster virus (VZV), typically occurring in older adults or immunocompromised individuals, and is rarely seen in infants. We report a rare case of herpes zoster in a 9-month-old infant, likely due to intrauterine exposure to VZV. The patient presented with dermatomally distributed vesicular lesions on the left anterior chest and back. Treatment included intravenous acyclovir, oral antiviral therapy, semiconductor laser therapy, boric acid compresses, and traditional Chinese herbal fumigation.

Evaluating varicella-zoster virus vaccine immunogenicity through Fc-mediated antibody functions: the roles of ADCP and ADCC.

Assessing the functional activity of vaccine-induced antibodies is critical for evaluating immunogenicity. We developed and validated antibody-dependent cellular phagocytosis (ADCP) and antibody-dependent cellular cytotoxicity (ADCC) assays to quantify Fc-mediated antibody responses elicited by varicella and zoster vaccines. Both assays demonstrated robust performance and broad linearity. Antibody titers were measured using fluorescent antibody to membrane antigen (FAMA) and ELISA. ADCP and ADCC activities, along with FAMA and ELISA geometric mean titers (GMTs), were significantly increased in

Modelling Impact of Different Varicella Immunisation Strategies Upon Introduction in the Swedish National Programme.

AIM: Countries with varicella immunisation programmes have seen major reductions in disease burden. While two-dose schedules are now universally adopted, dosing ages vary, and some countries initially include catch-up vaccination of older children to speed impact. This modelling study assessed three two-dose schedules in the Swedish setting (1) 12 and 18 months, (2) 18 months and 5 years, and (3) 18 months and 7 years, with and without catch-up of susceptible children based on their medical history. METHODS: A deterministic, compartmental, age-structured, dynamic transmission model was used t

Hutchinson's sign complicated by bilateral multiple cranial neuropathies and cerebral infarctions: a case report.

Varicella-zoster virus (VZV) infection is associated with serious complications, including cranial vasculopathy and herpes zoster ophthalmicus (HZO). Hutchinson's sign, characterized by VZV lesions on the tip, dorsum, and root of the nose, serves as a strong predictor of ipsilateral ocular complications in HZO. Here we describe a 61-year-old male who developed multiple cranial neuropathies and acute cerebral infarction secondary to VZV infection. His clinical manifestations began with Hutchinson's sign, progressed to HZO, and subsequently led to acute cerebral infarction and bilateral ophthalm
