Facial-Stapedial Synkinesis Following Ramsay Hunt Syndrome: A Case Report.

We report the first documented case of facial-stapedial synkinesis (FSS) secondary to Ramsay Hunt syndrome (RHS) by describing the clinical manifestations and definitive surgical treatment outcome of this rare condition. A 35-year-old female presented with initial symptoms of severe right-sided facial weakness, otalgia, hyperacusis, ear fullness, and erythematous vesicles in the external auditory canal and tympanic membrane, suggestive of RHS. She later developed an intermittent "quivering" or "buzzing" sensation in her right ear initiated by facial movements such as closing her eyes and smili

Total immunoglobulin G concentration as a correlate of anemia in dialysis patients: machine learning analysis of clinical and serological factors.

BACKGROUND: Anemia affects up to 85% of hemodialysis patients and is associated with increased morbidity and mortality. Immune dysfunction in end-stage renal disease (ESRD) may contribute to anemia through inflammatory pathways. This exploratory study used machine learning to investigate associations between immune markers, particularly total immunoglobulin G (IgG) concentration and varicella zoster virus (VZV) serostatus, and anemia in dialysis patients. METHODS: This cross-sectional study enrolled 351 participants (179 dialysis patients, 172 healthy controls) between November 2023 and Febru

Live vaccines in solid organ transplant recipients.

PURPOSE OF REVIEW: Vaccine preventable infections (VPIs) contribute substantially to morbidity and mortality in solid organ transplant (SOT) recipients. Recent outbreaks and declining herd immunity have brought attention to optimizing vaccination pre and posttransplant, as many patients remain incompletely immunized at the time of transplant. This review summarizes modern evidence and evolving recommendations regarding live vaccination in SOT candidates and recipients. RECENT FINDINGS: Vaccination coverage at the time of transplant remains suboptimal, although targeted interventions have show

Antiviral prophylaxis against herpesviruses (herpes simplex virus, varicella-zoster virus and cytomegalovirus) among patients with cancer.

PURPOSE OF REVIEW: Patients diagnosed with cancer are at risk of viral reactivation, namely herpes simplex virus (HSV), varicella-zoster virus (VZV) and/or cytomegalovirus (CMV). However, the risk varies significantly with underlying malignancy and treatment regimen. We review the latest evidence for antiviral prophylaxis strategies, focusing on novel immune therapies and future directions. RECENT FINDINGS: Antiviral prophylaxis is indicated for all high-risk patients. The approval of letermovir for CMV prophylaxis has significantly reduced the burden of clinically significant infections in a

Oral Manifestations of Varicella and Their Contribution to Clinical Assessment in Hospitalized and Outpatient Patients.

BACKGROUND: Oral lesions, particularly enanthema, may accompany chickenpox and represent an important but often underrecognized component of the clinical presentation. Their timely identification is especially relevant in dental practice, as oral manifestations may be more frequent in patients with a more severe clinical course. This study aimed to describe characteristic oral cavity changes in hospitalized and outpatient patients with chickenpox, to identify patterns in the occurrence of oral findings in relation to disease severity, and to support clinical assessment in practice. METHODS: A

Global burden and time trends of vaccine-preventable diseases in children and adolescents.

BACKGROUND: Vaccine-preventable diseases (VPDs) are a major global health concern for children and adolescents. The COVID-19 pandemic further complicated VPDs prevention by halting vaccinations in many areas. This study aimed to analyze the global burden and trends of eight high - burden VPDs in 0 - 19 - year - olds from 1990 to 2021. METHODS: Using data from the Global Burden of Disease 2021 database across 204 countries, incidence and DALYs were stratified by age, sex, and SDI. A linear regression model calculated AAPCs, and a joinpoint regression model identified a significant trend - chan

Safety and immunogenicity of live attenuated vaccines during corticosteroid therapy in children with nephrotic syndrome: protocol for an open-label, non-inferiority, randomised controlled trial (VACCINES trial: VAccines in Children on CorticosteroIds for NEphrotic Syndrome).

INTRODUCTION: Live attenuated vaccines (LAVs) are recommended during moderate corticosteroid therapy (<2 mg/kg per day) for nephrotic syndrome (NS) largely based on observational/case-control study data. We aim to conduct a randomised controlled trial (RCT) to assess the safety and immunogenicity of LAVs in children with NS. METHODS AND ANALYSIS: To test the safety and immunogenicity of LAVs in NS in children on moderate dose corticosteroid therapy (1.5 mg/kg alternate day dose with maximum 40 mg alternate day dose; early arm) vs those off corticosteroid therapy for 4 weeks (standard arm), we

Varicella-zoster virus infection triggering GD3 antibody-positive variant Guillain-Barré syndrome: a case report and literature review.

BACKGROUND: Bilateral facial palsy variant Guillain-Barré syndrome (GBS) is a distinct clinical subtype. As an acute immune-mediated polyradiculoneuropathy often triggered by infections, varicella-zoster virus (VZV) is a rare yet significant precipitating factor for this variant. Anti-ganglioside antibodies, particularly anti-GD3, are involved in the pathogenesis of GBS, but their significance in VZV-associated bilateral facial palsy subtype remains incompletely elucidated. CASE PRESENTATION: We report a 46-year-old female who developed acute bilateral facial palsy and limb sensory deficits f

Varicella zoster virus infection and reactivation in a psychiatric ward during the COVID-19 pandemic: a case series analysis.

Severe mental illness (SMI) may increase susceptibility to varicella zoster virus (VZV), but VZV epidemiology in psychiatric wards is poorly described. We reviewed 54 women with SMI in a closed 50-bed psychiatric ward (July 2021-December 2024) and identified 3 varicella and 2 herpes zoster cases during April-October 2023. The varicella attack rate among exposed susceptibles was 6.38%, and herpes zoster incidence was 12.41 per 1,000 person-years. All case patients had received coronavirus disease 2019 vaccination and had prior severe acute respiratory syndrome coronavirus 2 infection. These fin

Clinical Characteristics and Diagnostic Markers of Genital Varicella-zoster in Women: A 12-year Retrospective Cohort.

BACKGROUND: Genital involvement in varicella-zoster virus (VZV) reactivation is uncommon and frequently misdiagnosed. Data on the clinical features of genital zoster are limited, particularly among female patients. METHODS: We conducted a retrospective case series of female patients with genital involvement by VZV at a tertiary referral center between 2013 and 2025. Inclusion required polymerase chain reaction (PCR)-confirmed VZV from genital swabs. Demographic characteristics, immune status, pregnancy status, clinical presentation, diagnostic pathways, and treatment patterns were extracted f

Varicella-zoster virus infection: a review about varicella and herpes zoster.

The Varicella-zoster virus (VZV) is a prevalent human pathogen that links dermatology, virology, and immunology. Following primary varicella infection, the virus establishes lifelong latency in the sensory ganglia, with reactivation manifesting as herpes zoster. The clinical spectrum ranges from typical dermatomal vesicular eruptions to atypical or disseminated presentations in older adults or immunocompromised patients, often posing diagnostic challenges. Early recognition and prompt initiation of antiviral therapy are essential to limit lesion progression, reduce viral shedding, and prevent

Epidemiology of herpes zoster at a tertiary care centre in Eastern India.

Herpes zoster infection is characterized by dermatomal distribution of painful vesicles occurring due to reactivation of the latent varicella-zoster virus and poses the risk of serious complications. Therefore, it is of interest to investigate the epidemiology, clinical presentation and outcomes of patients diagnosed with herpes zoster in a tertiary care centre in Eastern India. From June 2023 to December 2024, a series of herpes zoster patients consulting the Dermatology Outpatient Department at Burdwan Medical College and Hospital participated in this cross-sectional study. Detailed history,

Atypical multidermatomal herpes zoster in young immunocompetent adults.

This case series describes an unusual presentation of multidermatomal herpes zoster (HZ) in two immunocompetent young adult males, aged 35 and 20 years. Both patients presented with lesions involving multiple dermatomes, differing from the classic unilateral dermatomal pattern typically seen in HZ. Psychological stress was identified as a possible contributing factor in both cases. Dermoscopy is still infrequently utilized as a diagnostic tool in HZ. In these cases, it provided additional visual details that supported early recognition. Timely antiviral therapy and supportive measures, includi

Herpes Zoster Following Chemo-Immunotherapy With Pembrolizumab in Metastatic Non-small Cell Lung Cancer: A Case Report.

INTRODUCTION: Chemo-immunotherapy has reshaped the treatment of metastatic non-small cell lung cancer (NSCLC), yet data on varicella-zoster virus (VZV) reactivation under these regimens are limited. Major immunotherapy trials do not specifically describe herpes zoster events in patients receiving combined chemotherapy and immune checkpoint inhibitors. METHODS: We describe a case of metastatic lung adenocarcinoma (ADC) treated with first‑line Pemetrexed-Carboplatin-Pembrolizumab plus Denosumab in which herpes zoster occurred during treatment, detailing clinical course, management, and outcomes

Serological immunity against vaccine-preventable diseases in children with inflammatory bowel disease at diagnosis.

OBJECTIVES: Patients with inflammatory bowel disease (IBD) are at increased risk of vaccine-preventable diseases. However, vaccination coverage in this population is often suboptimal. This retrospective study assessed the vaccination status and vaccine serology of children diagnosed with IBD in a high-income country with broad vaccine access. METHODS: Medical records of children under 16 years diagnosed with IBD between 2009 and 2023 were retrospectively reviewed. Demographic and clinical data, vaccine history, and vaccine serology, at the time of diagnosis, were analyzed. RESULTS: A total o

The role of computed tomography scan in assessment of patient with herpes zoster opthalmicus complicated with periorbital necrotizing fasciitis.

BACKGROUND: Periorbital necrotizing fasciitis (NF) is a severe form of soft-tissue infection characterized by necrosis of the subcutaneous tissue, muscle, and other deep soft tissue around the fascia in the periorbital region. Although rare, such a condition can occur following predisposing factors, such as injury from previous trauma, procedures, or infection. In this report, we highlight the role of preoperative computed tomography (CT) scan assessment in a patient with periorbital NF following herpes zoster ophthalmicus. Immunocompromised conditions, particularly human immunodeficiency viru
