Fixed Nonsynonymous vOka Vaccine Mutations in IE62 drive Varicella-Zoster Virus Attenuation in Human Skin.

UNLABELLED: The live, attenuated vaccine for Varicella-Zoster Virus (VZV) protects against chickenpox in children and adults. However, the vaccine strain Oka (vOka) establishes latency and may reactivate. The molecular basis of vOka attenuation remains unknown. We investigated whether three high-frequency single-nucleotide polymorphisms (SNPs) that differentiate vOka from parental strain Oka (pOka) are responsible: S628G and R958G in the immediate-early regulatory protein IE62, and *130R in the membrane protein ORF0. We evaluated how these fixed SNPs mediate the attenuated growth phenotype of

Weighted ensemble learning outperforms statistical, machine-learning, and deep-learning models for varicella forecasting: a 12-model benchmarking study using 11-year surveillance data from Nanjing, China.

BACKGROUND: Varicella imposes a sustained public health burden in China, where decades of voluntary single-dose immunization have been associated with recurrent school outbreaks and an age shift toward adolescents. Accurate forecasting of varicella incidence is crucial for public health preparedness and resource allocation. Systematic head-to-head benchmarks of statistical, machine-learning, deep-learning, and ensemble forecasts for varicella in Chinese metropolitan settings are scarce. METHODS: Monthly varicella case counts for Nanjing, China (January 2015-December 2025) were extracted from

Blood transcriptomic immune signatures in herpes zoster and postherpetic neuralgia: parallel cohort analyses of IFN-related pathway patterns.

Herpes zoster (HZ) results from reactivation of latent varicella-zoster virus (VZV), and postherpetic neuralgia (PHN) is a major chronic pain complication. We performed parallel analyses of two independent public blood transcriptomic cohorts: GSE242252 (n=80; HZ onset, 1-year recovery, and controls) and PRJNA1142765 (n=23; 15 HZ and 8 PHN). Because the cohorts differed in platform, library preparation, design, and sampling structure, expression matrices were not merged; cross-cohort interpretation was restricted to within-cohort results and pathway-level directional concordance. HZ onset showe

Divergent Strategies in Innate Immune Evasion: A Comparative Review of Three Alphaherpesvirus Subfamily Members-PRV, HSV-1, and VZV.

Pseudorabies virus (PRV), an important member of the Alphaherpesvirinae subfamily, shares high evolutionary homology with other members of the same subfamily, including human herpes simplex virus (HSV) and varicella-zoster virus (VZV). However, significant differences exist among them in terms of host range, pathogenicity, and immune evasion strategies. This review systematically compares the roles of PRV, HSV type 1 (HSV-1), and VZV in modulating host innate immune responses, encompassing the cyclic GMP-AMP synthase (cGAS)-stimulator of interferon genes (STING) signaling, RIG-I-like receptor

Seroprevalence of varicella antibodies in unvaccinated children with nephrotic syndrome in disease remission.

Background and objectives Varicella-zoster (VZV) virus infection can be severe in children with nephrotic syndrome. Lack of varicella vaccination through the national immunisation programme in India leaves many vulnerable to infection. We conducted this study to document the seroprevalence of Varicella antibodies in patients with nephrotic syndrome. Methods This cross-sectional study (Oct 2024-May 2025) was conducted at a tertiary-care teaching hospital in India, in children (1-18 y) with nephrotic syndrome (both steroid-sensitive and steroid-resistant) in disease remission. Serum VZV IgG anti

Varicella zoster virus meningitis with a dermatomal rash in immunocompetent hosts: a systematic review and novel case report.

Varicella zoster virus (VZV) reactivation with central nervous system (CNS) involvement is an uncommon but important cause of lymphocytic meningitis in both immunocompromised and immunocompetent individuals. While cranial presentations, especially V1 involvement, are well documented, spinal-dermatomal VZV meningitis is rarely reported. We conducted a systematic review of PubMed, Scopus, and Embase using PRISMA guidelines to identify all cases of laboratory-confirmed VZV meningitis or encephalitis with a dermatomal rash in immunocompetent adults affecting spinal levels C1 and below. Data extrac

Climate change and vertically transmissible infections in maternal and neonatal populations: a systematic review.

BACKGROUND: Climate change influences infectious disease dynamics and disrupts maternal and neonatal health services, both of which are pathways through which the risk of vertically transmissible infections may increase. METHODS: We systematically reviewed evidence on the association between climate-related environmental exposures and infection-related outcomes relevant to vertical transmission in pregnant or intrapartum women, neonates, and infants. Screening, extraction, and risk-of-bias assessment were performed independently in duplicate. Owing to substantial heterogeneity, findings were

Cocirculation of Human Monkeypox Virus Clade 1b with Varicella Zoster Virus, Uganda.

In July 2024, we reported information on the initial human mpox cases in Uganda. Genomic characterization of clinically suspected cases (July-December 2024) revealed cocirculation of several monkeypox virus clade 1b clusters and varicella zoster virus. Our findings highlight the need to refine differential diagnoses by molecular techniques and strengthen surveillance.

Painless Thoracic Herpes Zoster Eruption in an 88-Year-Old Woman With Advanced Chronic Kidney Disease: A Case Report.

Herpes zoster (HZ) typically presents with a unilateral vesicular eruption accompanied by neuropathic pain. Although pain is a characteristic feature, painless presentations may occur and can create a diagnostic challenge. We report the case of an 88-year-old woman with advanced chronic kidney disease (CKD) who developed a unilateral vesicular eruption involving the right thoracic dermatome without pain at any stage of the disease. She was not receiving immunosuppressive therapy and had no known diabetes mellitus, previous HZ, or neurological disorder. The diagnosis was made clinically based o

Herpes Zoster as a Trigger of Cardiovascular Events: A Systematic Review of Epidemiological Evidence and Pathophysiological Mechanisms.

BACKGROUND: Herpes zoster (HZ) has been associated with acute cardiovascular and cerebrovascular events, particularly stroke. We systematically reviewed epidemiological evidence and biological mechanisms linking HZ to vascular outcomes. METHODS: PubMed/MEDLINE, Scopus, Web of Science, and Embase were searched for observational studies evaluating cardiovascular or cerebrovascular events after HZ or herpes zoster ophthalmicus (HZO). Cohort, case-control, self-controlled case-series, and case-crossover studies were eligible. Mechanistic reports were considered separately to assess biological pla

Varicella zoster virus reinfection in a patient with refractory nephrotic syndrome: a case report.

Permanent immunity to varicella zoster virus (VZV) develops following infection in most patients, with reinfections extremely rare. Here, we report a 6-year-old boy with refractory nephrotic syndrome who contracted VZV infection during B-cell depletion after rituximab treatment. Although developing febrile neutropenia, he was promptly treated with acyclovir, which was successful. The VZV immunoglobulin G antibody titer was weakly positive (3.3) by enzyme immunoassay performed 42 days after the initial diagnosis of VZV infection, confirming the presence of VZV, although antibody production was

Exploring the role of infectious pathogens in Alzheimer's disease neuroinflammation.

Alzheimer's disease (AD) is the most common neurodegenerative disorder in older adults, characterized by progressive cognitive decline that ultimately leads to severe dementia. The primary pathophysiological hallmarks of AD include the accumulation of amyloid-β plaques, neurofibrillary tangles composed of hyperphosphorylated tau protein, synaptic dysfunction, and pronounced neuroinflammation. Although the exact relationship between neuroinflammation and AD pathogenesis remains incompletely understood, neuroinflammation is consistently recognized as an early and sustained feature of the disease

Global burden of varicella and herpes zoster in women of childbearing age: insights from the global burden of disease study 1990-2021.

INTRODUCTION: Although the epidemiological characteristics of varicella and herpes zoster have been systematically studied, there is currently a lack of research on the disease burden and changing trends in women of childbearing age (WCBA) (15-49 years) with varicella and herpes zoster. METHODS: Based on data from the Global Burden of Disease (GBD) study, we investigated the prevalent cases, incident cases, and disability-adjusted life years (DALYs), prevalence rates, incidence rates, and DALY rates of varicella and herpes zoster among women of childbearing age across 204 countries and territ

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5

Viral triggers of Guillain-Barré syndrome: implications for laboratory diagnostics.

Guillain-Barré syndrome (GBS) is a leading cause of acquired acute neuromuscular paralysis and is an immune-mediated disorder of the peripheral nervous system (PNS). Accumulating evidence indi-cates that GBS is often preceded by infectious events that trigger autoimmune responses through mechanisms such as molecular mimicry and autoantibody production. Campylobacter jejuni remains the most common bacterial agent, but viral pathogens also contribute to the development of GBS. This review summarizes the current evidence regarding viruses as triggers of GBS, including herpes-viruses, hepatotropic

Multiple Sclerosis and Viruses: « Liaisons Dangereuses ».

Multiple sclerosis (MS) is a complex, multifactorial neurodegenerative disease with an aetiology that is still partially elusive. Several risk factors are associated with MS development including genetic predisposition and various environmental stressors; however, recently, viral infections have gained significant attention as a crucial environmental trigger. Despite the well-established link between Epstein-Barr virus (EBV) and MS susceptibility, a single-pathogen model fails to clearly characterise the molecular cascades driving the disease, suggesting that the transition from infection to a

Uncertain magnitude of benefit of early second-generation androgen receptor antagonist treatment in advanced prostate cancer.

Guidelines currently recommend the early use of second-generation androgen receptor signaling inhibitors (ARSIs) in both castrate-resistant and -sensitive metastatic prostate cancer (with or without chemotherapy) and also in biochemical non-metastatic castrate-sensitive relapse. We highlight design issues in the randomized controlled trials (RCTs) that bias the results of early ARSI use, suggesting that the magnitude of benefit might be overestimated and, therefore, uncertain. Issues include sub-optimal post-protocol care, inadequate control arms, and the inclusion of highly selected patients,
