Initial diagnosis of herpes zoster ophthalmicus complicated by central retinal artery occlusion and subsequent varicella-zoster virus encephalitis: a case report.

PURPOSE: To report the unusual case of a herpes zoster ophthalmicus (HZO) complicated by central retinal artery occlusion and viral encephalitis. CASE REPORT: A 65-year-old male with HZO who underwent intravenous acyclovir treatment complained of sudden onset blurry vision of his left eye. Central retinal artery occlusion (CRAO) was revealed. We administered methylprednisolone pulse therapy, yet resulted in no visual acuity improvement. CONCLUSION: There were only a few cases reported to have CRAO after diagnosed HZO. This cause of viral-related CRAO led to a complicated treatment decision.

Atovaquone/proguanil use and zoster vaccination are associated with reduced Alzheimer's disease risk in two cohorts: implications for a latent Toxoplasma gondii mechanism.

Alzheimer's disease (AD) is a progressive neurodegenerative disorder with a multifactorial and incompletely understood etiology. Identifying exposures associated with reduced AD risk may help generate mechanistic hypotheses and inform future prevention strategies. To investigate such associations, we analyzed electronic health records from a national Israeli health provider, retrospectively comparing 9124 individuals with AD to 18,248 matched controls. We systematically screened prior medication purchases recorded up to 10 years before diagnosis. Significant associations, adjusted for residual

Approach to vertigo in children: A single-center experience.

Pediatric vertigo is not a rare symptom, and it has a significant impact on the child's quality of life. The aim was to describe the etiologies, diagnostic steps, and treatment outcomes in pediatric vertigo. Forty patients who presented with vertigo between May 2018 and September 2024 were evaluated. The etiology was determined by physical examinations, family histories, blood tests, and cranial imaging methods of the patients, and treatments were planned accordingly; 22 (55%) of the patients were female, and 18 (45%) were male. The ages of the patients ranged from 3 to 18 (median 14). The dia

Secondary Deinococcus proteolyticus Infection Following Varicella-Zoster Virus Infection in a Child: A Case Report Describing a Rare Medical Condition.

Deinococcus proteolyticus belongs to Deinococcaceae, a family of bacteria characterized by an exceptional ability to withstand the lethal effects of DNA-damaging agents, including ionizing radiation, ultraviolet (UV) light, and desiccation. It was isolated from the Sahara surface sands, an extreme and nutrient-poor environment, regularly exposed to intense UV radiation, cycles of extreme temperatures, and desiccation. D. proteolyticus infection has seldom been implicated in human infections. The child presented with multiple necrotic skin lesions following a varicella infection, later complica

Sudden Sensorineural Hearing Loss as the Presenting Symptom of Varicella Zoster Virus Rhombencephalitis: A Case Report.

Sudden sensorineural hearing loss (SNHL) is an ENT emergency that requires prompt diagnosis and treatment. Most cases are thought to be idiopathic, and early oral steroid initiation is thought to increase the likelihood of recovery. MRI of the internal auditory meatus (IAM) is the imaging modality of choice, as retrocochlear pathology needs to be excluded. We report the case of a 54-year-old man who presented with left-sided sudden SNHL in the absence of any other symptoms. MRI IAM demonstrated inflammation in the posterior fossa (rhombencephalitis), prompting an urgent neurology referral. Lum

Herpes Zoster of the Larynx: A Diagnostic and Therapeutic Challenge.

Herpes zoster of the larynx is an exceptionally rare condition that may mimic more common causes of laryngitis. Reactivation of varicella-zoster virus in the vagus nerve can lead to odynophagia, dysphonia, and dysphagia, often without the typical cutaneous rash. Early recognition is essential, as delayed diagnosis may result in serious complications, including postherpetic neuralgia, persistent dysphagia, chronic voice impairment, and even laryngeal paralysis. We describe a 66-year-old woman on long-term low-dose corticosteroid therapy who presented with acute odynophagia, dysphagia, dysphonia

Fc-mediated immune effector functions elicited by recombinant and live-attenuated herpes zoster vaccines.

BACKGROUND: The role of Fc-mediated immune responses in protecting against herpes zoster remains poorly understood. This study aimed to evaluate the Fc-mediated immune responses of anti-VZV glycoprotein E antibodies induced by two licensed varicella-zoster virus vaccines. RESEARCH DESIGN AND METHODS: We established two cohorts, each consisting of 48 healthy participants aged 50 or above, who received either the ZVL or RZV. Serum samples were measured to evaluate the Fc-mediated immune responses for antibody-dependent cellular cytotoxicity (ADCC), antibody-dependent cellular phagocytosis (ADCP

Comparative evaluation of Savanna HSV 1+2/VZV multiplex assay and Simplexa HSV 1&2 and VZV direct kits for rapid molecular detection of HSV-1, HSV-2, and VZV.

Herpes simplex virus (HSV-1 and HSV-2) and varicella-zoster virus (VZV) cause cutaneous and mucocutaneous lesions. Primary infection and secondary reactivations affect the mouth, genitalia, and dermis at other bodily regions. Accurate clinical diagnosis without laboratory confirmation can be difficult since lesions caused by these viruses are nondescript, and areas of bodily infection can overlap. Rapid molecular test results have been shown to reduce the time to definitive diagnosis, providing additional opportunities for patient counseling, antiviral therapy, and appropriate infection preven

Antigen-specific TH17 cells offset the age-related decline in durable T cell immunity.

Older adults are susceptible to infections in part due to waning of immune memory. To uncover mechanisms of a long-lasting immune memory, we contrasted varicella zoster virus antigen-specific memory T cell responses in adults vaccinated at young (<20 years) or older age (>50 years) with a live-attenuated vaccine conferring durable protection only when given at young age or with an adjuvanted component vaccine eliciting long-lasting immunity in older adults. Unlike VZV-specific CD4+ T cells, CD8+ T cells exhibited profound age-sensitive changes including memory subset shifts, reduced T cell rec

Targeted metagenomics reveals hidden chickenpox epidemic amid Mpox surveillance in Uganda.

In regions where both monkeypox virus (MPXV) and varicella zoster virus (VZV) are co-circulating, overlapping clinical manifestations can complicate clinical diagnosis. During the MPXV outbreak declared in Uganda in July, 2024, symptomatic suspected cases tested PCR negative for Mpox. To determine the cause of symptoms, we employed metagenomic sequencing with a targeted Viral Surveillance panel in 284 MPXV negative samples. VZV was identified as the predominant pathogen in 86% of MPXV-negative cases, suggesting a concurrent chickenpox surge. Using the VaricellaGen pipeline for variant calling,

Primary Varicella-Zoster Virus Infection Complicating the Third Trimester of Pregnancy.

Varicella-zoster virus, colloquially known as chickenpox in the United States, complicating pregnancy has become a rare diagnosis following the introduction of the varicella-zoster vaccine. Despite a lower incidence of chickenpox than in the past, those providing prenatal care should keep a high index of suspicion for varicella-zoster among patients presenting with a vesicular rash and no history of having received the varicella-zoster vaccine. This report reviews the case of a 28-year-old gravida three at 35 weeks gestation who presented with concern for a full-body vesicular rash and pruritu

PD-L1-Binding Antigen Presenters: Redirecting Vaccine-Induced Antibodies for Cancer Immunotherapy.

The efficacy of immunotherapy in enhancing antitumor immunity in solid tumors remains limited, primarily due to the insufficient immunogenicity of tumor cells. In contrast, vaccination and natural viral infections can generate durable, high-titer antiviral antibodies. A modular Programmed Death-Ligand 1 (PD-L1)-binding antigen presenter (PBAP) has been engineered to tether varicella-zoster virus (VZV) glycoprotein E (gE) to PD-L1 expressed on tumor cell surfaces. This innovative construct leverages pre-existing anti-gE antibodies to trigger antibody-dependent effector mechanisms. PBAP-gE effec
