This retrospective study evaluated quantification of skin fluorescein delivery by fiberoptic fluorometry as a means of predicting the healing potential of an amputation site. Fluorometry uses a dual-channel fiberoptic light guide--one channel transmits blue light to excite the fluorescein in the skin under study, and the other transmits emitted fluorescence from the skin to a photomultiplier tube where it is measured. The presence of beta-lactamase producing bacteria (beta LPB) was investigated in specimens obtained from 1469 children who presented with infections of the skin and soft tissue (648), upper respiratory tract (514), pulmonary sites (137), surgical sites (113), and other (57). A 10-year retrospective study of age-frequency, sites of infection, and pre-existing conditions in 297 children with Hemophilus influenzae (HI) disease seen at Grady Memorial Hospital from 1974 through 1984 is described. The majority of the patients were black (73%) and of lower socioeconomic status and were less than 2 years of age. Manifestations of HI disease were similar to those described in reports from other centers, with meningitis being the most common (56.7%). The changing microbiology of bacteremia among narcotic addicts in Detroit raised concerns about current presumptive antimicrobial therapy. In a one-year study of incidence, microbiology, sites, and risk factors, 180 bacteremic addicts (15% of addict-related admissions) were followed prospectively. Facial and periorbital cellulitis require emergent recognition and treatment. Hemophilus influenzae type B is the most common pathogen, with Streptococcus pneumoniae being cultured less frequently. Infections following injury are usually caused by Staphylococcus aureus and group A streptococci. Initial evaluation must exclude more severe orbital involvement. Appropriate bacteriological specimens should be obtained. Management requires parenteral antibiotics and careful monitoring for a clinical response or dissemination to other sites of infection.

Infective complications were analysed in 74 patients who underwent renal transplantation. Bacterial infections were seen in 69%, viral infections in 48% and fungal infections in 17% of these patients. The urinary tract was involved in 56% of patients, the lungs in 24% and the upper respiratory tract in 17%. Less common were cellulitis (7%), CNS infections (4%), and other sites of infection.

From May 1973 thorugh April 1974, a total of 83 children with severe Haemophilus influenzae infections were treated in three Denver pediatric hospitals.
