Early Outcomes of a Simple Intramedullary Fixation Technique for Fifth Metacarpal Neck and Distal Diaphyseal Fractures.

Fifth metacarpal neck and distal diaphyseal fractures with severe angulation, displacement, or malrotation may benefit from surgery. Fixation approaches include antegrade and retrograde intramedullary fixation, transmetacarpal Kirschner wires (K-wire), and plate fixation. K-wires are a particularly promising option due to their simple, non-invasive, and cost-efficient nature. This study describes a novel K-wire technique for fracture fixation and presents early outcomes and complications. 21 patients with fifth metacarpal neck or distal shaft fractures were retrospectively reviewed after treat

Antibiotics in Septorhinoplasty: Routine Necessity or Unnecessary Precaution? A Nationwide Propensity-Score Matched Retrospective Cohort Study.

BACKGROUND: The nasal cavity's natural bacterial flora poses a risk for postoperative infections, prompting surgeons to administer prophylactic antibiotics during septorhinoplasty. However, this practice's effectiveness is debated due to concerns about antibiotic resistance and variability in practice. This study leverages a large nationwide database to evaluate the impact of perioperative antibiotics on infection, cellulitis, and sepsis rates following septorhinoplasty. METHODS: A retrospective cohort study was conducted to identify patients undergoing septorhinoplasty using the TriNetX netw

Hospital-at-home versus hospital admission for acute care in Singapore: a prospective quasi-experimental study on cost, utilisation and clinical outcomes.

BACKGROUND: Hospital-at-home (HaH) is increasingly adopted to expand hospital capacity, however, data from Asian healthcare remain limited. The study compares cost, utilisation, and clinical outcomes of HaH program compared to traditional hospital care in Singapore. METHODS: This prospective quasi-experimental study compared patients enrolled in the HaH programme with a comparator group of clinically eligible patients who received ward-based care because they lived outside the programme's catchment area or when HaH capacity was full. Adults admitted to two hospitals within an academic health

Orbital cellulitis with emm1-type M1UK sublineage group A Streptococcus detected in a blood culture: a case report.

Invasive group A streptococcal infection (iGAS) increases in Europe and it causes skin and soft-tissue infection, including orbital cellulitis. In this study, we report a case of a 7-year-old girl with orbital cellulitis and invasive emm1-type M1UK sublineage group A Streptococcus (GAS) detected in a blood culture. The patient was initially treated with a combination of cefotaxime (CTX) and clindamycin (CLDM). We changed antibiotics from CTX to ampicillin after susceptibility results were known. After discharge, oral clavulanic acid/amoxicillin was continued as an oral medication, and antibiot

A retrospective, real-world study of IV iron use to treat iron deficiency anemia during acute infection.

The administration of IV iron to treat anemia during acute infection remains controversial owing to concerns of exacerbating the infection. We conducted a retrospective cohort study using the TriNetX Research Network (2000 to June 2025) to evaluate the safety and efficacy of IV iron administration in adults with iron deficiency anemia and infection (methicillin-resistant Staphylococcus aureus [MRSA] bacteremia, pneumonia, urinary tract infection [UTI], colitis, or cellulitis). Patients must have received antibiotic agents within 2 days of infection for inclusion and were stratified by IV iron

Quantifying the impact of inequality on traditional and emerging diabetes complications: A registry study of cause-specific hospital admissions and deaths in Australia.

BACKGROUND AND AIMS: Our aim was to quantify the relationship of a large range of diabetes complications with socioeconomic disadvantage. METHODS: This study included 621,114 people with type 2 diabetes from the Australian National Diabetes Services Scheme, with follow-up from July 2010 to June 2022. These data were linked to hospital admission and death datasets. The association of socioeconomic disadvantage with cause-specific outcomes (first occurrence of either admission or death) was assessed using Poisson regression, adjusted for age, sex, and year. RESULTS: Most diabetes-related compl

Staphylococcus spp. Epidemiology, Virulence, Genomic Adaptability and Coinfection in Broiler Chickens.

Staphylococcus species are saprophytic, opportunistic, and nosocomial pathogens that frequently co-infect with other microorganisms, causing severe infections in birds. Some of the notable examples include bacterial chondronecrosis with osteomyelitis (BCO), cellulitis, dermatitis, and systemic infections. Understanding of how Staphylococcus spp. cause infections evading the host immune system is crucial for helping farmers and veterinarians develop long-term solutions for poultry production system management. The aim of this review is to broaden the understanding of Staphylococcus spp. epidemi

Lymphovenous anastomosis: microsurgical innovation and clinical outcomes in breast cancer-related lymphedema care.

Lymphovenous anastomosis (LVA) has emerged as an important physiologic microsurgical procedure for patients with breast cancer-related lymphedema (BCRL) with the goal of restoring lymphatic drainage rather than providing just palliative care for symptoms of swelling. A multicenter randomized controlled trial (RCT) in 2024 (N-LVA) found improvements in the Lymph-ICF physical and mental function domains, and decreased use of compression garments, despite modest changes in total quality of life (QoL) and limb volume at 6 months. Meta-analyses have found average reductions of 30%-35% in excess lim

Role of ultrasonography in pediatric infectious diseases.

Infectious diseases remain a leading cause of pediatric consultations despite significant advances in their prevention and treatment. Clinical ultrasound has emerged as a valuable diagnostic tool in the pediatric setting due to its non-ionizing nature, the absence of need for sedation, its feasibility at the point of care, and its safety, reproducibility, and current availability. In this article, we review pediatric infectious conditions in which clinical ultrasound contributes significantly to diagnosis, follow-up, and clinical decision-making. This is a narrative review based on both the av

20/20 to NLP due to Blowing the Nose and Orbital Emphysema: A Case Report.

INTRODUCTION: Orbital emphysema is characterized by presence of air in the soft tissues of the orbits. In severe cases, this can lead to orbital compartment syndrome, a medical emergency that can result in severe permanent vision loss without urgent intervention. Generally, the severity of presenting vision loss and longer time to intervention have been associated with worse visual outcome. CASE PRESENTATION: We present the case of a 26-year-old healthy male who developed an acute orbital compartment syndrome with orbital emphysema following nose blowing in the setting of a bacterial orbital
