A Retrospective Analysis of Soft Tissue Point-of-Care Ultrasound (POCUS) in Primary Care.

INTRODUCTION: Soft tissue concerns are common, yet most primary care physicians must refer patients to radiology for further imaging, leading to potential delays in diagnosis and management. Point-of-care ultrasound (POCUS) is increasingly available in primary care and has been shown to improve clinical decision making. However, current Family Medicine POCUS curricula focus primarily on cellulitis and abscess, overlooking other common soft tissue pathologies. This study aims to evaluate the frequency of soft tissue pathologies and associated follow-up recommendations on comprehensive radiology

Systematic literature review and meta-analysis of postoperative complications of surgical management of vulvar cancer: what is the impact of frailty factors?

INTRODUCTION: Vulvar cancer surgery is associated with high rates of morbidity. Preventing or minimizing these morbidities is an important objective, as they impact on patients' quality of life, and are highly deleterious in frail, aged patients, who represent the majority of those affected by this disease. This systematic literature review and meta-analysis assesses post-operative complications in vulvar cancer surgery, and attempts to identify the impact of frailty factors. METHOD: A Pubmed search was conducted to identify studies reporting data on complications of vulvar cancer surgery in

Recipient site selection in vascularized lymph node transfer for secondary lymphedema: A systematic review and meta-analysis.

Vascularized lymph node transfer (VLNT) is an established microsurgical approach for secondary lymphedema; however, the optimal recipient site remains uncertain. This systematic review and meta-analysis aimed to compare outcomes between different recipient sites in upper and lower extremity VLNT. Following the PRISMA guidelines, PubMed, Scopus, and ScienceDirect were searched to October 2024. Outcomes assessed included circumferential reduction rate (CRR), volume-based reduction, cellulitis incidence, and quality of life. Meta-analysis was performed using the generic inverse-variance method wi

Campylobacter fetus Bacteremia Presenting with Bilateral Lower Leg Cellulitis in an Immunocompetent Adult.

Campylobacter fetus is a species of the Campylobacter genus that often causes bacteremia in immunocompromised patients. Although cellulitis due to C. fetus has been previously described, bilateral involvement is rare. We herein report the case of an 87-year-old woman without immunodeficiency, who presented with fever and bilateral leg pain. An examination showed inflammatory changes in both lower legs, and blood cultures showed the growth of C. fetus. Antimicrobial therapy was initiated with meropenem, de-escalated to ampicillin, and then switched to oral amoxicillin, which resulted in a clini

Rivaroxaban and Piperacillin-Tazobactam: Case of Massive Gastrointestinal Bleeding.

Anticoagulated patients are at increased risk of complications from drug interactions, including with antibiotics. Piperacillin-tazobactam (PTZ) has been associated with coagulation disruptions, potentially leading to severe bleeding when combined with anticoagulants like rivaroxaban. This report describes the first known case of massive gastrointestinal (GI) bleeding potentially associated with PTZ in a rivaroxaban-treated patient, successfully managed by discontinuing both drugs. We present a 52-year-old male with deep vein thrombosis on rivaroxaban, admitted for severe left leg cellulitis.

Frostbite in January-Amputate in July: Is Watchful Waiting for Surgical Management Following Frostbite Injury Still Standard? An Examination of the National Readmission Database.

Frostbite injuries account for 2000-4000 hospital admissions annually in the United States, with 30% of severe cases requiring amputation and up to 70% of cases resulting in long-term sequelae or disability. Traditional practice following frostbite injury is to delay surgical intervention 4-6 weeks to preserve limb length, tissue coverage, and reduce complications. This study examines amputation timing among frostbite patients in the United States and identifies factors influencing early versus delayed amputation. This retrospective study used the Nationwide Readmission Database (NRD) from 201

Feasibility of Platelet-Rich Plasma in Uterosacral Ligament Suspension Surgery.

IMPORTANCE: Platelet-rich plasma (PRP) is an autologous biological with the potential to enhance tissue healing. Its use during native tissue pelvic organ prolapse (POP) repair has not been evaluated. OBJECTIVE: The objective of this study was to assess the acceptability and feasibility of PRP injection into the uterosacral ligament during uterosacral ligament suspension (USLS) for POP repair. STUDY DESIGN: This prospective feasibility study was conducted from April to October 2024 at a large academic institution. Patients scheduled for USLS prolapse surgery for stage ≥2 POP were screened fo

Chronic Foreign Body Inflammatory Response in Textiloma Formation: Case Report and Analysis of Current Evidence.

A gossypiboma (GPB), also referred to as a textiloma (TXT), is a mass formed as a result of the body's inflammatory and granulomatous response to retained surgical textile material (RSTM), most commonly gauze or mesh fibers. Its reported incidence ranges from 1 in 1,000 to 1 in 10,000 surgical procedures, although the true frequency is believed to be significantly higher due to underreporting related to medicolegal concerns. This entity is clinically important because it may mimic more common postoperative complications such as abscess (ABS), seroma (SER), or mesh infection (MI), frequently le

The Effect of Surgical Approach on Postoperative Complications Following Pediatric Gastrostomy Tube Placement.

PURPOSE: Gastropexy during laparoscopic gastrostomy tube (GT) placement is commonly achieved using transfascial sutures (TFS), transabdominal sutures (TAS), or T-fasteners (TF). It is unknown whether the method of gastropexy influences the incidence of postoperative complications. We aimed to determine if the gastropexy approach impacts rates of common complications after laparoscopic GT placement. METHODS: A single-center retrospective review of children who underwent laparoscopic GT placement from January 2018 to May 2023 was conducted. Patients with concomitant procedures labeled wound cla

Orbital Cellulitis: An Uncommon Complication of Sinusitis.

This case study describes a school-age child who developed orbital cellulitis following an upper respiratory infection. Orbital cellulitis is an infection of the orbital space, typically caused by an extension of an infection in the sinuses. While complications of upper respiratory infections are rare, they can be serious, and advanced practice nurses need to be alert for signs and symptoms that may indicate a complication.

Abdominal and Back Pain Initially Suspected to Be Transient Intestinal Ischemia in a Case of Streptococcus dysgalactiae subsp. equisimilis Bacteremia.

Streptococcus dysgalactiae subsp. equisimilis (SDSE) has been increasingly recognized as a causative pathogen of skin and soft tissue infections and bacteremia, particularly in elderly individuals or those with underlying comorbidities. It typically presents with fever and localized pain associated with cellulitis; however, it may occasionally manifest with nonspecific symptoms, leading to diagnostic delay. We report the case of an 87-year-old man who presented with abdominal and lower back pain, initially suspected to have intestinal ischemia based on contrast-enhanced computed tomography fin

National Demographic and Treatment Patterns of Finger Cellulitis, Paronychias, and Felons.

BACKGROUND: Finger cellulitis, felon, and paronychia may be treated nonoperatively if diagnosed in a timely manner. Delayed or improper treatment may result in progression to more serious conditions, which necessitate intervention by a hand surgeon. This investigation sought to evaluate national demographics and treatment patterns in finger infections. MATERIALS AND METHODS: The PearlDiver Mariner database, an insurance claims database with 151 million unique patients within the United States, was used to identify patients with finger cellulitis, felon, and paronychia from 2010 to 2015. Effect of prior mastectomy on outcomes following total shoulder arthroplasty.

BACKGROUND: Mastectomy and its associated treatments may alter shoulder biomechanics and soft-tissue integrity, yet their impact on total shoulder arthroplasty (TSA) outcomes remains unclear. This study evaluated short- and long-term complications following TSA in patients with a history of mastectomy. METHODS: A retrospective cohort analysis was performed using the TriNetX Research Network, including patients who underwent primary TSA. Individuals with a history of mastectomy were identified using procedural and diagnostic coding, and those with contralateral mastectomy and TSA were excluded.

Recurrent Penile and Scrotal Edema as An Extracolonic Manifestation of Metastatic Crohn's Disease in an Adult Patient.

We present a case of recurrent penile and scrotal swelling determined to be an extraintestinal manifestation (EIM) of Crohn's disease. Genitourinary complications are a rare EIM, with most cases reported in pediatric populations. Initial treatment assumed cellulitis, delaying appropriate care. Recognition of the swelling as an EIM allowed for targeted treatment with prednisone, leading to rapid resolution. EIMs of Crohn's disease can be difficult to identify, especially rare forms like scrotal and penile edema. Increased awareness and early recognition are essential for streamlined treatment,

Lisinopril-Induced Angioedema Triggered by Tattoo Cellulitis and Acute Kidney Injury: A Case Report.

Angiotensin-converting enzyme inhibitor-induced angioedema is a rare, potentially life-threatening disorder. This mostly manifests with swelling of the lips, tongue, and face with no known reports of angiotensin-converting enzyme inhibitor-induced angioedema from soft-tissue infections. We present herein a case of a 47-year-old male with a history of chronic kidney disease stage III who developed acute upper lip angioedema and severe acute kidney injury while on lisinopril/hydrochlorothiazide, following tattoo-related cellulitis. We propose that inflammatory activation from cellulitis amplifie
