The effect of methylene blue infiltrating injection on anal pain after Milligan-Morgan surgery: A randomized controlled clinical study.

BACKGROUND: To investigate the efficacy and safety of methylene blue infiltrating injection (MBI) in alleviating postoperative pain following hemorrhoidectomy. METHODS: In a randomized clinical trial, 60 patients with mixed hemorrhoids undergoing Milligan-Morgan surgery were divided into 2 groups: a study group (n = 30) and a control group (n = 30). Upon surgical completion, patients in the study group received an intraoperative MBI to the surgical incisions. All patients, in both groups, then received the same standard postoperative on-demand intravenous lornoxicam protocol. The primary outc

Sickle Cell Crisis-Induced Painless Bilateral Orbital Bone Infarction: A Case Report.

INTRODUCTION: Sickle cell disease is associated with painful vaso-occlusive crises, bone infarction, avascular necrosis, and osteomyelitis. Sickle cell orbitopathy is a rare manifestation with signs similar to periorbital cellulitis and has subtle radiologic features. CASE PRESENTATION: We report a case of a five-year-old girl with sickle cell who presented with bilateral orbital infarctions and subperiosteal hematomas, notably without periorbital pain, and was treated with antibiotics and steroids. CONCLUSION: Physicians should be aware of this rare manifestation, especially in the pediatri

Behind the Swelling: Primary Mediastinal Large B-cell Lymphoma Masquerading as an Infection.

We present the case of a 26-year-old female patient with a history of asthma who initially presented with symptoms suggestive of a common infection. Her constellation of facial swelling, productive cough, and fatigue, coupled with imaging findings, first led to a diagnosis of right upper lobe pneumonia and suspected cellulitis. Despite appropriate antibiotic therapy, her symptoms persisted, and her clinical picture became more complex with the identification of Actinomyces in her sputum, shifting the differential diagnosis toward a more indolent cervicofacial infection. However, a progressivel

A Case of Chronic Elephantiasis Nostra Verrucosa Highlighting the Progression of Chronic Lymphedema.

Elephantiasis nostra verrucosa (ENV) is a rare form of secondary, nonfilarial chronic lymphedema, typically linked to conditions such as morbid obesity, congestive heart failure, and chronic venous insufficiency (CVI). Bacterial infection is thought to initiate a vicious cycle of inflammation, limb swelling, and fibroblast proliferation that results in the "cobblestone" appearance of the affected extremity. A 67-year-old male with a past medical history of end-stage renal disease (ESRD) on hemodialysis, morbid obesity, type 2 diabetes mellitus, and ENV presented to our ED with cellulitis of hi

"GLP-1 Agonists vs. Bariatric Surgery: A Global Health Network Analysis of Panniculectomy Outcomes".

BACKGROUND: With the rise in medically assisted weight loss, procedures like panniculectomy are increasingly common. The surgical impact of glucagon-like peptide-1 (GLP-1) receptor agonists remains unclear, particularly compared to bariatric surgery. OBJECTIVES: To compare panniculectomy outcomes and perioperative lab values in patients with prior GLP-1 use versus bariatric surgery. METHODS: TriNetX Global Research Network was queried to identify patients who underwent panniculectomy following either GLP-1 use or bariatric surgery. Cohorts were formed using codes such as CPT and ICD-10 and p

Surgical Treatment for Extremity Lymphedema Reduces Frequency of Cellulitis Episodes: A Cohort Study and Meta-Analysis.

Dysfunction of the lymphatic system causes lymphedema, a chronic disease that can lead to cellulitis. However, the optimal treatment modality for preventing cellulitis has not yet been established. This cohort study investigated the outcomes of cellulitis by treatment modality in lymphedema of the extremities. A meta-analysis was conducted to evaluate the change in frequency of cellulitis episodes according to the surgical procedure. This single-center retrospective cohort study included patients with extremity lymphedema who were treated at our department. We performed a comprehensive search o

A diagnostic dilemma in pediatric proptosis-from suspected malignancy to benign orbital cavernous venous malformation: a case report.

BACKGROUND: Rapidly progressive proptosis in children can be caused by multiple causes. Distinguishing between these is challenging, especially when presentations mimic malignancy. CASE PRESENTATION: A healthy Malay 3-year-old boy presented with a 1-week history of painless, progressive left eye proptosis. Initial imaging showed intraconal and extraconal orbital masses with prominent vascular channels, suggesting a vascular lesion. Two weeks later, he developed fever, lethargy, and worsening proptosis. Repeat imaging revealed lesion enlargement with intralesional hemorrhage, displacement of o

Elephantiasis Nostra Verrucosa Complicated by Cellulitis and Hemorrhagic Bullae: A Rare and Severe Clinical Presentation.

Elephantiasis nostras verrucosa (ENV) is a rare and debilitating manifestation of chronic nonfilarial lymphedema characterized by progressive dermal fibrosis, hyperkeratosis, and papillomatous skin changes. It arises from long-standing lymphatic obstruction and is most commonly associated with risk factors such as obesity, venous insufficiency, recurrent soft tissue infections, and conditions impairing lymphatic drainage. The true prevalence of ENV is poorly defined due to its rarity, though limited institutional data suggest it is uncommon even among high-risk populations. While ENV typically

Avian Metapneumovirus: A Review of Pathogenesis and Its Effect on Diagnosis.

Avian metapneumovirus (aMPV) is a highly contagious upper respiratory pathogen of poultry globally that emerged in the United States in January 2024 and rapidly spread to flocks throughout North America. In turkeys, the disease (turkey rhinotracheitis) initiates as an upper respiratory infection with high morbidity and frequent complication by bacterial coinfection that can lead to high mortality and significant economic loss. In chickens (swollen head syndrome), uncomplicated clinical disease is less severe than in turkeys. Chickens are less susceptible to the virus, although, once infected,

Point-of-care ultrasound for the evaluation of dental abscesses in the pediatric emergency department.

BACKGROUND: Facial cellulitis is a common presentation at pediatric emergency departments. Prompt diagnosis of dentoalveolar abscesses is crucial for initiating appropriate treatment and minimizing morbidity. While imaging modalities such as computed tomography (CT) and a panoramic X-ray are available, they are not often used for pediatric patients given the known risks of exposure to radiation and the need for sedation. METHODS: We conducted a prospective diagnostic accuracy study at a tertiary pediatric emergency department to evaluate point-of-care ultrasound (POCUS, index test) for diagno

Infection post strabismus surgery: A review and case series.

Surgical site infection is a relatively rare complication of strabismus surgery; however, if it occurs, it can have permanent impact on the patient. Prior existing case reports and questionnaire-based studies have described different types of surgical site infections that can occur post strabismus surgery. It is not only important to know how to identify and treat such cases, but also to know how to prevent them, in addition to identifying the causative risk factors. In this review, we discuss 10 such cases of various types of infection that occurred post strabismus surgery and how they were m

Acute versus chronic mitral regurgitation: a case of perioperative cardiogenic shock in an athlete.

Mitral regurgitation (MR) poses challenges in distinguishing acute from chronic aetiologies in compensated patients. A 48-year-old male rugby player presented with 2 weeks of worsening dyspnoea on a background of chronic bilateral lower limb cellulitis. Found in atrial fibrillation, transoesophageal echocardiography revealed flail P2 with severe MR, ejection fraction 35%, and severe left ventricular dilation-indicating chronicity despite acute symptoms. He underwent urgent mitral valve repair. Post-induction instability necessitated emergency cardiopulmonary bypass. Repair consisted of P2 tria

Snake envenomation and acute kidney injury: a systematic review and meta-analysis.

BACKGROUND: Snakebite envenomation remains a health problem in tropical regions and is a recognized cause of acute kidney injury (AKI). Despite numerous regional studies, the global burden and predictors of snakebite-induced AKI have not been systematically quantified. This review aimed to estimate the pooled incidence of AKI following snake envenomation, identify key clinical and laboratory features, and determine the mortality rate and proportion of patients requiring renal replacement therapy (RRT). METHODS: A systematic search of PubMed, Embase, and Cochrane Library was conducted for studi
