Neutrophilic dermatoses are a group of rare heterogenous inflammatory disorders that share the presence of a sterile neutrophilic infiltrate on histopathology due to an underlying systemic illness, which can be a malignancy. This group of disorders can have diverse clinical presentations, including papules, plaques, nodules, vesiculopustular lesions, or ulcerations. We review the clinical and histopathologic features of several neutrophilic dermatoses that can be cutaneous manifestations of an underlying malignancy, including Sweet syndrome (acute febrile neutrophilic dermatosis), pyoderma gan

Pyoderma gangrenosum (PG) is a rare, sterile neutrophilic dermatosis characterized by rapidly progressive, painful cutaneous ulcerations and frequently associated with systemic diseases, including inflammatory bowel disease, rheumatologic disorders, and hematologic malignancies. In particular, recognition of PG as a cutaneous manifestation of an underlying hematologic neoplasm can be critical for timely diagnosis of the systemic disease. Postoperative PG after total hip arthroplasty may closely mimic early periprosthetic joint infection and can lead to unnecessary surgical revisions and prolon

Pyoderma gangrenosum is a rare neutrophilic dermatosis characterized by painful ulcerative skin lesions and a lack of definitive diagnostic tests. It is frequently mistaken for infectious conditions, often resulting in inappropriate antibiotic use and delayed immunosuppressive therapy. To describe the clinical and demographic characteristics of patients with pyoderma gangrenosum initially assessed in an infectious diseases setting and identify patterns that may aid in earlier diagnosis and management. A retrospective case series was conducted at a Cellulitis Clinic. Patients with a PARACELSUS

Malignancies can elicit a vast array of systemic findings in patients that are not directly driven by tumor invasion. These paraneoplastic syndromes may result from mechanisms ranging from ectopic hormone expression to crossreactive immune responses. Many of these paraneoplastic syndromes can even resemble autoimmune dermatoses. Paraneoplastic dermatoses are especially common in patients with hematologic malignancies. We review the fundamental mechanisms underlying immune-mediated paraneoplastic dermatoses in patients with hematologic malignancies and describe several common paraneoplastic der

S pseudintermedius is a leading cause of canine pyoderma and an important pathogen in small animal skin, soft tissue, otic, wound, and postoperative infections. The emergence of methicillin-resistant S pseudintermedius (MRSP) has made recurrent infections more difficult to manage by combining multidrug resistance, biofilm-associated persistence, treatment failure, and repeated antimicrobial exposure. This article reviews MRSP emergence and epidemiology, mechanisms of resistance and recurrence, and the limitations of current treatment options. It discusses emerging therapeutic strategies, inclu

BACKGROUND: Postsurgical pyoderma gangrenosum (PSPG) is a rare neutrophilic dermatosis that occurs following surgical procedures and frequently mimics postoperative infection. Misdiagnosis may lead to repeated surgical debridement and disease progression due to pathergy. Reports describing safe reoperation following PSPG remain limited, creating uncertainty regarding subsequent reconstructive procedures.
CASE: We report the case of an 18-year-old woman with well-controlled perinatal HIV who developed PSPG following bilateral reduction mammaplasty. On postoperative day (POD) 2 she developed re

Pyoderma gangrenosum is a rare neutrophilic dermatosis that presents as rapidly progressive ulcerative lesions, often associated with systemic diseases. Pyoderma gangrenosum remains an uncommon and challenging diagnosis particularly in a pediatric population. We report a 3-year-old male with myelodysplastic syndrome who developed a nonhealing ulcer of the right forearm following minor trauma. The lesion was misdiagnosed as an infection, leading to prolonged antibiotic therapy. The patient underwent surgical debridement and closure of the ulcer with bilateral advancement flaps with biopsies tak

We report a complex case of neutrophilic dermatosis (ND) in a 61-year-old woman with a 19-year history of recurrent, pruritic, and painful vesiculopustular eruptions. Her clinical course was marked by evolving diagnoses, from pustular vasculitis to IgA pemphigus and pyoderma gangrenosum (PG). Concurrent investigations revealed an IgA-κ type monoclonal gammopathy of undetermined significance (MGUS). This case highlights the rare association between neutrophilic dermatoses (ND) and haematological disorders, and illustrates the therapeutic challenges posed by comorbidities such as osteoporosis an

Postoperative pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that frequently masquerades as a surgical site infection. Because its clinical presentation closely resembles postoperative cellulitis or even necrotizing soft tissue infection, diagnosis is often delayed. Misdiagnosis may result in unnecessary antibiotic escalation, repeated surgical interventions, and progressive tissue destruction due to pathergy. This case highlights the importance of early recognition of PG and serves as a reminder for surgeons to consider this uncommon diagnosis when postoperative wounds fail to re

BACKGROUND: Systemic inflammation is increasingly linked to atrial arrhythmogenesis, yet arrhythmic risk and downstream cardiovascular complications remain poorly quantified for inflammatory diseases managed outside cardiology, including neutrophilic dermatoses such as pyoderma gangrenosum (PG).
METHODS: We performed a global, large retrospective cohort study, including 147 healthcare organizations. Adults were included in three separate 1:1 propensity score-matched cohorts: (1) PG vs non-PG to assess incident AF and relevant arrhythmogenic heart disease; (2) PG with vs without AF to examine

A variety of skin conditions may be important diagnostic indicators for disorders of the digestive tract and liver. In inflammatory bowel diseases, the most common cutaneous manifestations are erythema nodosum and pyoderma gangrenosum. Autoimmune liver diseases are often associated with cutaneous manifestations such as hidradenitis suppurativa, vitiligo, psoriasis and alopecia areata. Lichen planus or porphyria cutanea tarda can also be induced by infectious hepatitis. Enteral malabsorption can lead to deficiencies and characteristic skin changes, such as celiac disease, acrodermatitis enterop

IMPORTANCE: Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis characterized by sterile ulcerative skin lesions. Multiple comorbidities and clinical features have been associated with PG, but no standardized guidelines exist for classifying PG phenotypes.
OBJECTIVE: To develop an expert-established classification framework for PG phenotypes to inform treatment guidelines and future research endeavors.
EVIDENCE REVIEW: In this modified Delphi consensus study that included 23 board-certified dermatologists and Medical Dermatology Society members with expertise in PG, panelists complet

IL-36γ is highly expressed in psoriatic skin lesions and promotes neutrophil infiltration through the induction of neutrophil chemotactic chemokines. While IL-36γ has been established to play an important role in the pathogenesis of psoriasis, its localization in the psoriatic epidermis has not been fully elucidated, and the difference in its expression patterns among other inflammatory skin diseases remains unclear. We investigated IL-36γ localization using immunohistochemistry in skin specimens from patients with psoriasis vulgaris (n = 36), generalized pustular psoriasis (n = 11), pyoderma

BACKGROUND: Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis characterized by painful, rapidly progressive cutaneous ulcers. Postoperative PG (PPG) represents a distinct variant that develops at surgical or traumatic sites due to the pathergy phenomenon. Early diagnosis is often challenging, as it requires the exclusion of other causes such as infection, and misdiagnosis may lead to inappropriate surgical interventions and treatment delays.
METHODS: We report the case of a 69-year-old man who developed extensive ulcerative lesions at both surgical sites following bilateral high lig

BACKGROUND: Staphylococcus pseudintermedius is the main etiological agent of canine pyoderma and exhibits increasing multidrug resistance. Due to the reduced efficacy of antimicrobials, topical combinations of antimicrobial agents-such as chlorhexidine (CHX) and clotrimazole (CLT)-have gained attention as an alternative.
OBJECTIVE: To evaluate the in vitro antibacterial activity and pharmacological interaction of a 2:1 CHX-CLT combination against S. pseudintermedius isolates from canine pyoderma.
MATERIALS AND METHODS: Sixteen isolates of S. pseudintermedius were obtained from canine pyoderm

BACKGROUND: Severe cutaneous ulceration in antiphospholipid syndrome (APS) with concurrent infection and thrombocytopenia presents a therapeutic dilemma: standard immunosuppression risks worsening infection, while anticoagulation risks hemorrhage. We present a high-risk case successfully managed using intravenous immunoglobulin (IVIG) as a bridging immunomodulatory strategy.
CASE PRESENTATION: A 73-year-old man with triple-positive APS and immune thrombocytopenic purpura (ITP) developed rapidly progressive leg ulcers with clinical features consistent with pyoderma gangrenosum-like disease, co
