Assessment of three major intestinal protozoan infections in Kermanshah, Iran: a pre- and post-COVID-19 study.

Intestinal parasitic infections continue to be a significant public health concern worldwide. This study examines the impact of the COVID-19 pandemic on the prevalence of three intestinal protozoa—Blastocystis, Entamoeba coli, and Giardia lamblia—among patients at the Imam Reza Health Complex in Kermanshah, Iran, from March 2017 to March 2024. Patient data were obtained from the Health Information System and analyzed across seven age groups using descriptive and inferential statistics. A total of 15,018 individuals participated in the survey, consisting of 7169 males and 7849 females. The over

A challenging diagnosis: The overlap of hepatorenal polycystic disease and amebiasis.

A 71-year-old retired man returning from Senegal was found to have multiple hepatorenal cysts associated with a colonic mass on CT. Initially misdiagnosed, stool and abscess puncture analyses revealed intestinal and extraintestinal amebiasis. On the other hand, genetic testing suggested hepatorenal polycystic disease, demonstrating the coexistence of these two pathologies.

Structural basis for carbohydrate recognition by the Gal/GalNAc lectin of Entamoeba histolytica involved in host cell adhesion.

Intestinal amoebiasis is caused by Entamoeba histolytica, one of the deadliest human-infective parasites. Central to its pathogenicity is its binding to mucosal carbohydrates, which precedes tissue damage by trogocytosis. Carbohydrate binding is mediated by a single adhesin, the galactose/N-acetylgalactosamine (Gal/GalNAc) lectin, which is the leading vaccine candidate for amoebiasis. We present the structure of the native heterodimeric lectin, revealing an ordered core containing the light chain and the N-terminal region of the heavy chain. Structures obtained in the presence of ligand show t

Cervico-vaginal Amoebiasis Masquerading as Malignancy-A Case Series Highlighting Rare Clinical Presentation of a Common Parasitic Infection.

Amoebiasis is a common parasitic infection with a global disease burden, and usually presents with intestinal disease. Cervical and vulvovaginal amoebiasis are rare extraintestinal manifestations that closely mimic malignancy, owing to overlapping symptoms like vaginal bleeding or discharge and ulcerated necrotic mass-like lesions on clinical examination. A high index of suspicion, particularly in women from endemic regions, combined with timely pathologic examination, is crucial for accurate diagnosis and appropriate management. Definitive diagnosis relies on cytologic or histopathological ex

Trafficking and organization of cyst wall components into a robust biphasic structure in Entamoeba.

Entamoeba histolytica, a protozoan parasite, causes amebiasis. Amebiasis is mainly transmitted by oral ingestion of cysts. Cysts are produced in the large intestine of humans from proliferative trophozoites by a cell differentiation process called encystation. The Entamoeba cyst wall consists of chitins and proteins that include chitinase and Jacob and Jessie lectins. During encystation, these components are synthesized and layered around encysting Entamoeba cells. The structures of these components are well studied; however, the detailed timings of their synthesis (the transcription of the en

An atypical venus fly trap domain receptor regulates motility and phagocytosis in the protozoan parasite Entamoeba histolytica.

Amebiasis is a parasitic infection of the human intestines, primarily caused by Entamoeba histolytica. Its pathogenesis relies on the environmental sensing-induced cytoskeletal remodeling as the basic mechanism for motility and tissue invasion. We identified and characterized an atypical Venus Fly-Trap (VFT) receptor protein, EhVFT (CL6EHI_096680). While it shares homology with the ligand-binding domain of class C GPCRs, it is phylogenetically related to the Periplasmic Binding Protein (PBP) superfamily. This protein is uniquely lacking a transmembrane domain. Instead, the glycosylphosphatidyl

Fulminant amebic colitis complicated by appendiceal perforation and massive abdominal hemorrhage: a case report and literature review.

INTRODUCTION: Fulminant amebic colitis complicated by intestinal perforation or massive intra-abdominal hemorrhage is uncommon but associated with extremely high mortality. In non-endemic regions, diagnosis is frequently delayed because early manifestations resemble bacterial appendicitis or perforated peritonitis. CASE PRESENTATION: We report a fatal case of Entamoeba histolytica infection presenting with appendiceal perforation, septic shock, and recurrent intra-abdominal bleeding. Surgery revealed extensive transmural necrosis, deep ulcers, and exposure of submucosal vessels. Metagenomic n

Intestinal entamoebiasis in a captive colony of Anderson's crocodile newts Echinotriton andersoni.

Entamoeba protozoa are known to cause enteric and sometimes systemic disease in both domestic and non-domestic animals. In amphibians, Entamoeba spp. are primarily documented as incidental gastrointestinal inhabitants, with rare reports of clinically significant entamoebiasis. In the last decade, mortality outbreaks caused by a recently described protozoan, Entamoeba sp. CT1, have been documented in free-ranging anurans in Australia and managed anurans in the USA associated with gastroenteritis and substantial morbidity and mortality. In 2023, a small population of Anderson's crocodile newts E

A multifaceted model of Entamoeba histolytica KERP2 regulating gene expression and host cell responses.

Entamoeba histolytica causes amoebiasis and damages intestinal epithelium, but how individual parasite factors coordinate parasite gene regulation with host remodeling is unclear. We investigate lysine- and glutamic acid-rich protein 2, a factor linked to the host brush border. Sequence analysis, imaging, and functional assays show that lysine- and glutamic acid-rich protein 2 accumulates in the parasite nucleus, binds AT-rich DNA, and modulates transcriptional programs related to Amoebiasis, including cysteine protease and sulfur metabolism. During parasite-epithelium contact, the protein ent

Association of Blastocystis species subtypes 3 and 4 with intestinal inflammation and response to metronidazole in symptomatic patients.

BACKGROUND: Blastocystis spp. (BC) often passes as a harmless resident of the gut. However, it may be associated with gastrointestinal disturbances, such as abdominal pain and diarrhea. The variation in the clinical spectrum of BC infection is, in part, due to its diverse genotypic profile, where ongoing research continues to discover new BC subtypes. This study aimed at investigating the association between different BC subtypes and intestinal inflammation as assessed by fecal calprotectin (FCP) and tumor necrosis factor alpha (TNF-α) levels. METHODS: Patients complaining of gastrointestinal

Optimized DNA extraction from fecal samples using non-stool kits to improve PCR detection of protozoan parasites: A focus on Blastocystis sp.

PCR-based detection of intestinal protozoa frequently faces challenges due to inadequate DNA recovery or the presence of inhibitors within fecal samples. Therefore, optimizing extraction protocols through pretreatment strategies is crucial to enhance DNA yield and quality, ensuring reliable molecular detection. This study aimed to evaluate whether combining conventional, non-stool-specific extraction kits with pretreatment protocols could enhance DNA yield and purity from fecal samples and enable reliable PCR-based detection of intestinal protozoa, such as Blastocystis sp. Five pretreatment me

Microbiota-derived indole differentially shapes Entamoeba histolytica physiology and promotes host-compatible colonization.

Entamoeba histolytica is a pathogenic amoeba that colonizes the human large intestine and causes amoebiasis. In its natural gut environment, the parasite is exposed to microbiota-derived metabolites, including indole, a tryptophan-derived compound present at millimolar concentrations, whereas laboratory cultures contain negligible levels. We previously showed that bacterial metabolites such as queuine and oxaloacetate modulate parasite stress responses and virulence. Here, we investigated how acute versus long-term exposure to indole shapes E. histolytica physiology and host interactions. Trop

The role of cervids in the transmission of Blastocystis spp. to livestock and humans: a comprehensive review.

Blastocystis spp. is a common intestinal parasite with debated pathogenicity in animals but frequently linked to human disease in clinical studies, particularly ST1. Cervids, uniquely coexisting in wild and captive populations, may bridge transmission to both livestock and humans. This review synthesizes 44 studies (n = 6,137 cervids) to assess their potential transmission role by biological basis, host characteristics, and ecological factors. For biological basis, cervids carry livestock-associated subtypes (ST10/14 for cattle, ST5 for pigs) and human-pathogenic ST1 (124 cases, 91% in captive

Protein interactions and regulatory role of low-molecular-weight protein tyrosine phosphatase in Entamoeba histolytica virulence.

Amoebiasis is a gastrointestinal infection caused by the protozoan parasite Entamoeba histolytica (E. histolytica, Eh) and remains one of the leading parasitic causes of human mortality worldwide. Invasive intestinal disease may present with abdominal cramping, abdominal pain, watery or bloody diarrhea, and weight loss. Understanding the factors that modulate parasite virulence is therefore essential to elucidate disease pathogenesis. The E. histolytica genome encodes two low-molecular-weight protein tyrosine phosphatases (LMW-PTPs), EhLMW-PTP1 and EhLMW-PTP2, whose roles in parasite virulence

[Brain Amoebiasis].

Amoebic infections of the brain can cause brain abscesses and meningoencephalitis. Brain abscess is a rare form of extraintestinal amoebiasis caused by hematogenous spread from intestinal infection with Entamoeba histolytica, which occurs through oral intake of contaminated water and food or fecal-oral transmission. It is mostly associated with amoebic liver abscesses and can be effectively treated with metronidazole. Meningoencephalitis caused by pathogenic free-living amoebae is extremely rare. Primary amoebic meningoencephalitis (PAM), caused by Naegleria fowleri infection of the olfactory
