Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Alleles of CYP3A5 and their association with renal function in chronic kidney disease.

BACKGROUND: The cytochrome P450 family 3 subfamily A polypeptide 5 (CYP3A5) gene plays an important role in renal function through its product's involvement in metabolizing endogenous substances and drugs, including immunosuppressants used following kidney transplantation. A single-nucleotide polymorphism, CYP3A5*3 (rs776746), produces a non-functional variant that may influence progression of chronic kidney disease (CKD) by impairing renal filtration. However, the frequency of the CYP3A5*3 allele in the Thai population and its association with renal parameters remain underexplored. This study

Autochthonous amebiasis in Spain.

Amoebiasis is an infection caused by the intestinal protozoan Entamoeba histolytica. It is transmitted via the faecal-oral route and primarily affects the intestinal tract and the liver. It is prevalent in tropical countries but is uncommon in Europe, where it is typically diagnosed in immigrants and tourists from endemic regions. However, an increase in autochthonous cases is being observed in Spain. A case is presented of a 56-year-old Spanish male with no relevant epidemiological history who attended the Emergency Department due to right hypochondrial pain and fever. An abdominal CT scan re

Exploring the complex interplay of Blastocystis, morbid obesity, and bariatric surgery on gut microbial dynamics.

This study examines Blastocystis dynamics in 15 individuals undergoing sleeve gastrectomy. Molecular detection involved DNA extraction, RT-PCR, and sequencing, while 16S rRNA sequencing via Illumina MiSeq analyzed the intestinal microbiome. Statistical analysis through SPSS considered a significance level of p < 0.05. Postoperative colonization of Blastocystis was observed in previously negative individuals, revealing subtypes and shifts in microbial taxa. Blastocystis-positive participants post-bariatric surgery showed a significant increase in Lachnospira, alongside higher abundances of Bact

Assessment of Lactobacillus acidophilus (L. acidophilus) therapeutic and prophylactic role in rats experimentally infected with Blastocystis subtype 3 (ST3).

Blastocystis, an eukaryote, inhabits the intestinal tract of humans and animals worldwide. Lactobacillus acidophilus (L. acidophilus), a probiotic, has been reported to be effective against blastocystosis. The present study evaluated the therapeutic and prophylactic effectiveness of L. acidophilus compared to metronidazole (MTZ) in rats experimentally infected with Blastocystis subtype 3 (ST3). Four groups of Blastocystis ST3-infected rats received MTZ, L. acidophilus, both MTZ and L. acidophilus, or prophylactic L. acidophilus. Non-infected and infected control groups were included. The effec

Adaptive approach to diagnosis of amebic liver abscess in non-endemic areas.

Entamoeba histolytica infections range from asymptomatic intestinal amebiasis to more severe syndromes like invasive colitis or hepatic abscess. While diagnostic tests available for assessing these infections have evolved, the optimal use of newer diagnostics like enteric multiplex Polymerase Chain Reaction (PCR) panels has not been fully established. Here we describe the case of a 34-year-old male with an amebic liver abscess (ALA) that was diagnosed via multiplex Gastrointestinal (GI) PCR panel on formed stool and subsequently confirmed by testing liver abscess fluid (off-label) on the same

Molecular prevalence and associated risk factors of Entamoeba spp. in donkeys in Shanxi Province, North China.

BACKGROUND: The intestinal protozoa Entamoeba spp. can infect humans and various animals, including donkeys, causing diarrhea and malabsorption and presenting significant risks to animal husbandry and public health. Most Entamoeba species are not pathogenic except for Entamoeba histolytica. China has among the highest rates of donkey farming worldwide. Donkey (Equus asinus) farming is increasingly important in China because of their draft and medicinal value; however, epidemiological data on Entamoeba spp. in donkeys remains limited globally. This study aimed to investigate the prevalence of E

Molecular characterization and subtype analysis of Blastocystis sp. in captive wildlife in Henan, China.

Blastocystis sp. is a zoonotic intestinal protozoan that is ubiquitous globally, residing in the gastrointestinal tracts of both humans and various animals. In the present study, a PCR-sequencing tool based on the SSU rRNA gene was employed to investigate the prevalence and subtypes of Blastocystis spp. in 204 fresh fecal samples collected from 20 captive wildlife species from a bird park in Henan Province, Central China. Overall, Blastocystis was present in 13.73% (28 out of 204) of the samples and 25% (5 out of 20) of the species. A total of four zoonotic subtypes of Blastocystis sp. were fo

Spatial and Genetic Diversity of Clinical Isolates of Blastocystis in Italy: A Network Analysis.

Blastocystis is a common intestinal protist with a global distribution, frequently found in humans and various animals. Despite its prevalence, its role in human health remains debated, oscillating between being a harmless commensal and a potential pathogen. It has also been associated with gastrointestinal disorders such as irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD). In Italy, the genetic and spatial diversity of Blastocystis remains understudied, despite the country's diverse urbanized and environmental landscapes. This study investigates the haplotypic and spatial d

A new autochthonous case of amebiasis in Italy in a patient without risk factors: an alert for clinicians and laboratorians.

We describe a rare case of intestinal amebiasis of unknown source, in an immunocompetent patient with no risk factors. The detection of an autochthonous Entamoeba histolytica strain causing ulcerative colitis (UC) in a healthy individual was unexpected, but it was the second time it had occurred in Italy in a few months therefore, it highlighted the urgency to establish a new approach in the diagnosis of UC and more generally in the diagnosis of inflammatory bowel disease, involving routine parasitological investigation even in a non-endemic area.

Entamoeba gingivalis induces gingival cell death, collagen breakdown, and host immune response via VAMP8/-3-driven exocytosis pathways.

The protozoan Entamoeba gingivalis commonly colonizes anaerobic periodontal pockets, induces a severe innate immune response, invades gingival mucosa, and kills epithelial cells. E. gingivalis infection is associated with the common oral inflammatory disease periodontitis. DNA variants in vesicle-associated membrane proteins (VAMP) -3 and -8 genes are linked to increased periodontitis risk. These genes mediate host-pathogen interactions, including mucin exocytosis to form protective barriers and matrix metalloproteinase (MMP) secretion in intestinal amoebiasis caused by Entamoeba histolytica.

Human Mucin-2-Producing Colonic Goblet-Like Cells Secrete the Chemokine CXCL8 by Activating Multiple Proinflammatory Pathways in Response to Entamoeba histolytica.

The mucus layer produced by highly stressed goblet cells forms a protective shield in the gut to protect the underlying mucosal epithelial cells from external threats. Hypersecretion and depletion of mucin-2 (MUC2) mucin from goblet cells is characteristic of symptomatic Entamoeba histolytica infections. It was hypothesized that MUC2 depleted goblet cells could mount a second line of innate host defense by producing proinflammatory cytokines. To investigate this, whether E. histolytica could stimulate proinflammatory responses in wild-type (WT) high MUC2 mucin-producing goblet-like cells and i

Blastocystis colonization among inland and forest periphery Negritos in Malaysia: The crucial role of sanitation and hygiene practices.

There has been an increasing trend in the study of Blastocystis due to its ubiquitous nature, and potential impact on human health. In Malaysia, its prevalence among the undeserved indigenous Orang Asli (OA) population varies widely from 1.7 %- 80.0 %. These huge differences were suggested to be attributed to disparities in living standards, including sanitation and access to treated water status. We investigated the prevalence, risk factors, and subtypes of Blastocystis among the OA Negritos residing in forest periphery villages and the inland jungle settings. A cross-sectional study was cond

Amebicidal Activity of Escherichia coli Nissle 1917 Against Entamoeba histolytica.

Amebiasis is a globally prevalent infection that can lead to fatal outcomes if not adequately treated. Conventional treatment with imidazoles often fails due to side effects and resistance, emphasizing the need for alternative therapies. The probiotic Escherichia coli Nissle 1917 (EcN) has shown potential in combating intestinal pathogens. This study aimed to evaluate the amebicidal activity of EcN in vitro and its effect on the production of reactive oxygen species (ROS). Trophozoites of Entamoeba histolytica (2.5 × 10⁴ cells/mL) were cultured in 96-well plates and exposed to varying concentr
