Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Lamotrigine-associated SJS/TEN overlap during valproate co-therapy managed with rhTNFR-Fc and therapeutic plasma exchange: A case report.

RATIONALE: Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, life-threatening severe cutaneous adverse reaction. Evidence for escalation therapy in relapsing disease remains limited. PATIENT CONCERNS: A 49-year-old man developed fever and a rapidly progressive generalized erythematous maculopapular eruption after lamotrigine was initiated directly at 50 mg once daily during concomitant valproate therapy. DIAGNOSES: At admission on August 3, 2023, no epidermal detachment was observed and SJS/TEN was not clinically recognized. On August 9 (hospital day 7), epidermal frag

Clinical Study of Bullous Pemphigoid With Oral Mucosal Symptoms After Oral Administration of Dipeptidyl Peptidase-4 Inhibitors.

Dipeptidyl peptidase-4 (DPP-4) inhibitors are widely used for diabetes treatment. However, reports of DPP-4 inhibitor-associated bullous pemphigoid (BP) have been increasing. This study examined 12 cases of BP with oral mucosal lesions associated with DPP-4 inhibitor therapy. Six cases presented with only oral mucosal lesions, while the other six patients had non-inflammatory skin lesions. Discontinuation of DPP-4 inhibitors led to symptom improvement in nine cases, whereas three cases did not improve, with lesion progression to the pharynx. Remission was achieved after a mean of 20.8 ± 24.6 m

Upadacitinib for the treatment of refractory bullous pemphigoid: a case report and literature review.

Bullous pemphigoid (BP) is a common autoimmune subepidermal blistering disorder that mainly affects elderly individuals aged ≥70 years. Systemic glucocorticoids serve as first-line treatment, and immunosuppressants or biological agents such as dupilumab are added for patients with suboptimal responses. However, a proportion of patients exhibit suboptimal responses or cannot sustain therapeutic benefits under these standard therapeutic regimens, representing an important unmet clinical need. We describe a 74-year-old patient with refractory BP who did not achieve adequate disease control despit

GLP-1R expression and dermatological diseases: a mendelian randomization and real-world study.

BACKGROUND: Although glucagon-like peptide-1 receptor (GLP-1R) is widely expressed in multiple tissues and organs including skin and subcutaneous tissue, with diverse physiological roles in metabolism and immunity, the potential causal association between GLP-1R expression and dermatological diseases remains unclear. METHODS: Available cis-expression quantitative trait loci (cis-eQTLs) were selected as genetic instruments for GLP-1R expression. A two-sample Mendelian randomization (MR) analysis was employed to assess the potential causal association between GLP-1R expression and dermatologica

Correlation of Serum and Tissue Eosinophils and Immunoglobulin E with Disease Severity in Bullous Pemphigoid: A Prospective Observational Study.

BACKGROUND: Bullous pemphigoid (BP) is a subepidermal autoimmune blistering disorder in which eosinophils and immunoglobulin E (IgE) are implicated in disease pathogenesis and are potential markers of disease activity. AIM AND OBJECTIVE: To evaluate the association of serum and tissue eosinophils and IgE levels with disease severity in BP. PATIENTS AND METHODS: A prospective observational study was conducted from October 2023 to December 2024 at a tertiary care center in South India. Thirty-five patients with clinically and histopathologically confirmed BP were included, disease severity was

Case Report: Nicotinamide monotherapy for infantile bullous pemphigoid with long-term follow-up.

Infantile bullous pemphigoid (IBP) is a rare autoimmune bullous skin disease that occurs in infants, for which standardized targeted therapeutic regimens remain unavailable. Herein, we report two infants with IBP treated with nicotinamide monotherapy. Complete clinical remission was observed within 3 weeks and 10 days after treatment initiation, respectively. Following clinical improvement, nicotinamide was gradually tapered and discontinued in approximately 2 months. No recurrence was observed during long-term follow-up. However, given the limited number of cases, these observations do not es

Dentofacial Characteristics in Patients With Epidermolysis Bullosa-A Cross-Sectional Study.

OBJECTIVES: Epidermolysis bullosa (EB) is a rare genetic disease characterized by skin and mucosal fragility caused by pathogenic variants in structural protein genes. Malocclusions have been previously described in patients with EB, mainly in case reports, with no cohort studies characterizing dentofacial characteristics across all EB types. This study aims to describe and compare the dentofacial characteristics of patients with EB across all types in a nationally representative cohort. MATERIAL AND METHODS: A cross-sectional study examined 101 patients with genetically confirmed EB diagnosi

Targeted therapies in pemphigus vulgaris: Emerging horizons beyond rituximab.

Pemphigus vulgaris is a potentially life-threatening autoimmune blistering disorder driven by pathogenic IgG autoantibodies targeting desmoglein 3 and 1. While rituximab has transformed management-achieving complete remission off-therapy in 70-90% of patients-approximately 20-30% of patients relapse or fail to respond, defining a critical unmet need. The key drivers of rituximab-refractory disease include post-depletion B-cell repopulation fuelled by elevated BAFF and APRIL, persistence of long-lived plasma cells in bone marrow niches that evade anti-CD20 depletion, IgG4-dominant autoantibodie

Epidermolysis bullosa dystrophica in a goat kid.

In a dairy herd of Thuringian Forest goats, a male goat kid of around three weeks of age was detected with skin lesions, particularly on the distal limbs, chest and ears. These skin lesions were characterized by alopecia, pus-filled blisters, incrustations and erosions. The goat kid was referred to the clinic for treatment but had to be euthanized despite intensive treatment due to increasing deterioration in its general condition and was subsequently referred to necropsy. Histologic investigation of the altered areas of skin and mucosa revealed a separation of epithelium from the underlying d

Suprabasal Blistering as a Diagnostic Pitfall in Bullous Pemphigoid: Histopathologic-Immunofluorescence Discordance Complicated by Sepsis.

Bullous pemphigoid is the most common autoimmune subepidermal blistering disease, typically diagnosed through a combination of clinical features, histopathology, and direct immunofluorescence (DIF). However, histopathological findings can be misleading when re-epithelialization shifts the blister plane from subepidermal to suprabasal, mimicking pemphigus vulgaris. We present the case of a 58-year-old woman with type 2 diabetes mellitus, diabetic neuropathy, and a sacral pressure ulcer admitted with a three-week history of generalized tense bullae in a symmetric distribution across the trunk an

Blistering rash in an elderly patient.

An 87-year-old man presented with a 6-week history of painful erosions on the left arm and both lower limbs requiring daily wound dressings. Examination showed multiple sharply demarcated, polygonal erosions with a striking geometric configuration. Although the morphology initially suggested traumatic or contact-related injury associated with wound care, direct immunofluorescence of perilesional skin demonstrated linear C3 deposition along the dermoepidermal junction, and serum anti-BP180 antibodies were positive, establishing the diagnosis of bullous pemphigoid. This case highlights that bull

Cranial Subdural Empyema Due to Citrobacter koseri in a 96-Year-Old Immunocompromised Patient: Case Report.

Subdural empyema (SDE) is a life-threatening condition. Reports on SDE are scarce. We report the case of a 96-year-old immunocompromised patient on steroids for pemphigus who received repeat burrhole trephination and antibiotics to treat SDE. We identified Citrobacter koseri as the causative pathogen. Our patient made a good recovery at 2 months. We conclude that surgical treatment may yield a favorable outcome after cranial SDE, even in an elderly immunocompromised patient.

Short-term disease control with alternate-day maintenance upadacitinib in refractory Hailey-Hailey disease: a case report.

Hailey-Hailey disease (HHD), also known as benign familial pemphigus, is a rare autosomal dominant genodermatosis characterized by recurrent erosive lesions in intertriginous areas. Previous case reports have described clinical improvement with upadacitinib, but post-response maintenance strategies remain poorly defined. We report a 62-year-old woman with a 7-year history of refractory HHD who showed marked improvement after 2 months of upadacitinib 15 mg once daily. After 6 months of daily treatment, the lesions had completely cleared and disease control was stable. The regimen was then chang

Exacerbation of Bullous Pemphigoid following a Closed Bone Fracture in an Elderly Patient: A Case Report.

INTRODUCTION: Bullous pemphigoid (BP) is the most common autoimmune blistering disorder affecting the elderly population. While the etiology is predominantly idiopathic, physical trauma has been identified as a potential exacerbating factor. Most trauma-induced cases reported in the literature are associated with surgical incisions, burns, or open wounds. The development of generalized BP following a closed fracture, where the skin integrity remains initially intact, is an exceptionally rare phenomenon. The patient experienced a pre-bullous phase starting in June 2025, which significantly prog
