IgA Pemphigus in a Patient With Systemic Lupus Erythematosus (SLE): A Novel Association, With Combined Epidermal Intercellular IgA and Nuclear IgG Deposition on Skin Direct Immunofluorescence.

We describe the novel occurrence of IgA pemphigus in a patient with long-standing systemic lupus erythematosus (SLE). SLE can be associated with the development of other autoimmune conditions, including cutaneous immunobullous disorders. To our knowledge, the co-occurrence of IgA pemphigus and SLE has not been previously reported in the literature. Potential common pathophysiological mechanisms are discussed.

Demographic characteristics and comorbidities in patients with oral mucous membrane pemphigoid and pemphigus.

OBJECTIVES: Oral autoimmune blistering diseases (AIBDs) are a group of rare, chronic disorders causing different symptoms on the oral mucosa. The most common oral AIBDs are mucous membrane pemphigoid (MMP) and pemphigus vulgaris (PV). The aim of this study was to describe the demographic and clinical characteristics, as well as comorbidities, of patients with oral AIBDs in Southern and Western Finland. MATERIALS AND METHODS: This retrospective two-center study included patients diagnosed with AIBD with oral symptoms treated at Helsinki University Hospital and Turku University Hospital between

Endoplasmic reticulum stress in autoimmune skin diseases: immune activation, cutaneous cell damage, and therapeutic advances.

Endoplasmic reticulum stress (ERS) arises when the protein-folding load, or perturbations in calcium and redox homeostasis, outpace the adaptive capacity of the endoplasmic reticulum (ER). The unfolded protein response (UPR) initially reduces secretory load and restores proteostasis. Persistent stress can redirect the same pathways toward inflammatory signaling, altered antigen processing, and potentially immunogenic cell death. We term this context-dependent transition ERS-immune tolerance breaking, and propose it as a unifying framework for understanding how unresolved ERS may contribute to

Successful Treatment of Refractory Pemphigus Vulgaris with Ultra-Low-Dose Rituximab in a High-Risk Patient: A Case Report.

Pemphigus vulgaris is a rare autoimmune blistering disease that causes painful erosions on the skin and mucous membranes. Standard treatments include corticosteroids, immunosuppressants, and rituximab. However, managing pemphigus vulgaris in elderly patients with comorbidities remains challenging. We report a 73‑year‑old male patient who had pemphigus vulgaris poorly controlled by glucocorticoids and a history of malignancy after surgical resection. He was then treated with add-on ultra‑low‑dose rituximab (100 mg) combined with methylprednisolone. The patient achieved complete clinical remissi

CD74 as a Potential Adjunctive Tissue Biomarker for Bullous Pemphigoid Lesions: A Retrospective Study Integrating Bioinformatics Analysis and Experimental Validation.

BACKGROUND: Bullous pemphigoid (BP), an autoimmune blistering disorder that primarily afflicts the geriatric population, poses diagnostic and therapeutic challenges, particularly in patients with comorbidities. This retrospective study aimed to identify and validate potential adjunctive tissue biomarkers for BP and explore their association with immune-related mechanisms through integrated bioinformatics analysis and experimental validation. METHODS: Transcriptomic data from the GEO dataset were analyzed to identify differentially expressed genes (DEGs), which were further cross-referenced wi

Pemphigus herpetiformis: association with malignancies and therapeutic challenges.

This case series examines the histopathological, immunological, and therapeutic characteristics of 7 patients diagnosed with pemphigus herpetiformis (PH) at a tertiary hospital between 1989 and 2025. The most common clinical manifestation was pruritic annular lesions. Direct immunofluorescence was positive in all cases, whereas serological testing (enzyme-linked immunosorbent assay, indirect immunofluorescence, and immunoblotting) performed at 2 independent referral centers detected anti-desmoglein 1 antibodies in only 2 patients, with negative antigen-specific results in the remaining patient

Real-World Outcomes of Rituximab in Pemphigus: An Australian Tertiary Cohort.

Rituximab is endorsed internationally as first-line therapy for moderate-to-severe pemphigus, but Australian guidelines position it as a rescue agent and local outcome data are limited. In this single-centre retrospective cohort, 24 adults with pemphigus vulgaris (n = 16) or pemphigus foliaceus (n = 8) received rituximab as part of routine care; median follow-up from first infusion was 24.9 months. Disease control was documented in all patients and complete remission in 20 (83%), of whom half subsequently relapsed despite most receiving more than one cycle. Adverse events occurred in eight pat

Research Progress on Targeted Therapies for Pemphigus Vulgaris and Bullous Pemphigoid: Mechanisms, Opportunities and Challenges.

Autoimmune bullous diseases (AIBDs) are a group of severe disorders characterized by autoantibodies that attack structural proteins within the skin/mucosa, leading to blister and bullae formation. Pemphigus vulgaris (PV) and bullous pemphigoid (BP) are among the most prevalent representative AIBDs. Traditional treatments involving corticosteroids and immunosuppressive agents are often associated with substantial adverse effects and a high rate of relapse. In recent years, the therapeutic landscape has evolved toward precision targeted therapies, encompassing biological agents, small-molecule d

From Empiricism to Precision: Century of Paradigm Shifts in Medical Dermatology.

Over the past century, advances in scientific understanding and therapeutic development have fundamentally reshaped dermatology. Diseases once managed with empiric, toxic, or largely ineffective treatments can now be approached with therapies that target specific pathways, achieve sustained disease control, and, in some cases, alter prognosis. Herein, we reviewed landmark therapeutic advances across infectious, inflammatory, autoimmune, vascular, and oncologic dermatology, focusing on interventions that changed the standard of care or substantially improved patient outcomes. Transformative adv

Immunoglobulin deposition and plasma cell infiltration define distinct immunopathological patterns in skin diseases.

Immunoglobulin deposition in the skin is an important diagnostic and etiological feature in many skin diseases. However, the relationship between deposited immunoglobulins and the local presence of antibody-producing cells, such as B cells and plasma cells, remains poorly understood. In several autoimmune diseases, including systemic lupus erythematosus and bullous pemphigoid, marked immunoglobulin deposition is observed despite limited B-cell infiltration into the skin, suggesting that the deposited antibodies primarily originate as circulating autoantibodies. In contrast, diseases such as di

Disease-specific and therapy-driven alterations in immunoglobulin G heavy-chain repertoire in peripheral blood mononuclear cells from pemphigus vulgaris and bullous pemphigoid patients.

BACKGROUND: Pemphigus vulgaris (PV) and bullous pemphigoid (BP) are two subtypes of autoimmune bullous disorders (AIBD), characterized by abnormal B cell activation and autoantibody production. Glucocorticoids (GC) and rituximab (RTX) are main treatment agents for AIBD. However, the effects of GC and RTX therapies on B cell receptor (BCR) repertoire need clarification. METHODS: The study included 18 AIBD patients (13 PV, 5 BP) and 9 healthy controls (HC). Of 13 PV, 9 were newly diagnosed/relapsed (4 responsive to GC monotherapy), 4 were GC-refractory but responsive to RTX-GC therapy. All BP we

Complete remission after rituximab therapy of pemphigus: Real-world evidence on dosing regimens and retreatment strategies.

Although rituximab is clinically approved for pemphigus, real-world data on predictors of treatment success and optimal dosing remain limited. We conducted a retrospective cohort study of patients with pemphigus treated with rituximab with ≥1 year of follow-up at NewYork-Presbyterian Hospital. Patients initially received two 1000 mg doses 2 weeks apart or 4 weekly weight-adjusted (375 mg/m2) infusions. Factors associated with complete remission (CR) off oral immunosuppressive therapy were analyzed using Kaplan-Meier and Cox proportional hazards analyses. Among 105 patients, 44.7% achieved CR o

Temporal outcomes and biomarkers of rituximab treatment response in pemphigus: Validation of a historical control dataset.

Rituximab and corticosteroids remain the only clinically approved therapies for pemphigus vulgaris, although novel therapeutics are entering clinical development. A historical control dataset previously described clinical and serologic outcomes after rituximab therapy in pemphigus vulgaris, but whether these data are representative of other patient cohorts is currently unclear. A contemporary pemphigus vulgaris cohort (n = 97) was compared with the historical cohort (n = 107) to assess clinical outcomes as well as desmoglein 3 ELISA predictive performance for achieving complete remission off o

Targeted therapies in pemphigus vulgaris: Emerging horizons beyond rituximab.

Pemphigus vulgaris is a potentially life-threatening autoimmune blistering disorder driven by pathogenic IgG autoantibodies targeting desmoglein 3 and 1. While rituximab has transformed management-achieving complete remission off-therapy in 70-90% of patients-approximately 20-30% of patients relapse or fail to respond, defining a critical unmet need. The key drivers of rituximab-refractory disease include post-depletion B-cell repopulation fuelled by elevated BAFF and APRIL, persistence of long-lived plasma cells in bone marrow niches that evade anti-CD20 depletion, IgG4-dominant autoantibodie
