Common Skin Diseases and Metabolic Syndrome: A Proinflammatory Chemokine Perspective.

Skin diseases frequently coexist with other disorders, such as metabolic syndrome, diabetes mellitus, depression, psoriatic arthritis, and cardiovascular disease. Altered levels of distinct chemokines, like CCL5/RANTES, CXCL12/SDF-1a, CCL7/MCP-3, CCL2/MCP-1, CXCL1/GROa, and the eotaxin family, contribute to the development and/or exacerbation of inflammation, which is a common feature of numerous skin diseases as well as metabolic syndrome. The pathological and molecular connections between chronic inflammatory skin diseases and metabolic syndrome are increasingly recognized as being driven by

Patient Perspectives on Treatment Outcomes and Priorities in Autoimmune Bullous Diseases: An Exploratory Survey among Dutch Patients.

INTRODUCTION: Autoimmune bullous diseases (AIBDs), comprising pemphigoid and pemphigus diseases, have seen limited therapeutic advances beyond rituximab for pemphigus vulgaris. As novel therapies are evaluated in clinical trials, well-defined, uniform, and relevant outcomes with patient involvement are essential. To date, however, patient-reported outcomes remain underrepresented, leaving uncertainty about whether trial results genuinely reflect patients' expectations. This study examines perspectives of patients with AIBD on treatment outcomes and priorities to ensure that future research foc

Risk of inflammatory dermatoses following COVID-19 infection: a cohort study of 204 241 patients.

BACKGROUND: The long-term impact of SARS-CoV-2 infection on inflammatory dermatoses remains incompletely defined. OBJECTIVES: To evaluate the association between COVID-19 infection and a broad range of inflammatory dermatoses. METHODS: In this cohort study, electronic health-record data were analysed for 204 241 patients seen at a tertiary medical centre between March 2020 and May 2023, including 22 368 with confirmed COVID-19 and 181 873 controls with negative testing. Diagnoses of 16 inflammatory dermatoses were extracted from standardized documentation fields and verified diagnostic codes

Cyclophosphamide is associated with long-term treatment-free remission in patients with pemphigus.

BACKGROUND AND OBJECTIVE: Pemphigus treatment has seen significant advancements, yet long-term remission remains a challenge. Comparative data on the long-term effectiveness of available therapies are limited. We compared 10-year relapse rates in pemphigus patients treated with azathioprine or cyclophosphamide plus high-dose IV corticosteroids PATIENTS AND METHODS: In this retrospective single-center cohort study, we analyzed 91 patients with pemphigus vulgaris/foliaceus treated between 1985 and 2022. 43 patients received oral azathioprine, 48 received intermittent intravenous and oral cycloph

Postsurgical pemphigus vulgaris following thyroidectomy: a case report and review of the literature.

Rare diseases are often overlooked due to their low prevalence and may be classified as "idiopathic" once more common etiologies have been excluded. Nevertheless, rare conditions do occur, and clinicians must remain vigilant in considering them. Pemphigus vulgaris is a well-established autoimmune blistering disorder involving the skin and mucous membranes, with recognized triggering factors such as medications, infections, paraneoplastic processes, and trauma. Postsurgical pemphigus, however, represents an exceptionally rare variant, arising specifically at sites of surgical scarring. Unfortun

Genetic and immunological determinants of Pemphigus vulgaris: integrative analysis of HLA-DRB1 and FCGR2B variants.

Pemphigus vulgaris (PV) is a rare autoimmune blistering disease mediated by pathogenic autoantibodies. Although both HLA and non-HLA loci contribute to disease susceptibility, their combined roles in immune regulation remain incompletely understood. This study investigated the joint contribution of HLA-DRB1 and FCGR2B variants to PV susceptibility within an integrative immunogenetic framework. Genotype data from 286 individuals (200 controls and 86 PV patients) were analyzed using bias-reduced association models, two-locus genotype combination analyses, and cumulative genetic risk modeling. NK cell hyperactivation drives macrophage repolarization and limits M2 bias in pemphigus vulgaris.

BACKGROUND: Macrophages and natural killer (NK) cells are innate immune components proven to be either pathogenic, protective, or both in autoimmune disorders. Nonetheless, the function of macrophages in the pathogenesis of pemphigus vulgaris (PV), an autoimmune skin disorder, is lacking. OBJECTIVES: We aimed to investigate macrophages and their crosstalk with NK cells in PV, with a particular focus on how these interactions influence their functionalities. METHODS: Flow cytometry and immunofluorescence was utilised to assess macrophage profile along with expression of receptors on NK cells

Examining the association of dipeptidyl peptidase-4 inhibitors with autoimmune blistering disorders in the U.S. Military veterans.

BACKGROUND: Dipeptidyl peptidase 4 inhibitor (DPP4i) are commonly used for management of diabetes. Although there are reports of increased risk of autoimmune skin blistering disorders (AIBD) with DPP4i, data is scarce. Although the exact mechanism through which DPP4i may cause AIBD is unknown, DPP4i may induce anti-basement membrane zone antibodies. Inhibition of DPP4 enzyme may also result in the recruitment of eosinophils into the dermis. OBJECTIVES: Examine the incidence of AIBD in DPP4i users compared to non-users. METHODS: This retrospective cohort study included US veterans who initiat

CD19 CAR T-cell therapy is feasible for patients with pemphigus vulgaris treated without lymphodepletion in the RESET-PV trial.

Lymphodepleting preconditioning (LD) is essential for the efficacy of chimeric antigen receptor (CAR) T-cell therapy in hematologic malignancies. However, in the setting of autoimmune diseases (ADs), the contribution of LD for efficacy is unclear. Here, we report on the early safety, efficacy, and correlative data of the first 4 patients with pemphigus vulgaris (PV) who received resecabtagene autoleucel (rese-cel), a fully human CD19 CAR T-cell therapy, without LD in the RESET-PV trial, a substudy of the DesCAARTes trial. Following infusion, pemphigus disease area index scores improved signifi

Protocol for evaluating the effects of the intelligent multimodal symptom assessment and response system in patients with pemphigus vulgaris: a randomised controlled trial.

INTRODUCTION: Pemphigus vulgaris is a rare autoimmune blistering disease characterised by recurrent mucocutaneous erosions, high symptom burden and unpredictable relapse. Current management relies mainly on pharmacological therapy and hospital-based follow-up, with limited real-time monitoring and individualised support in home-based disease management. To address these challenges, this trial aims to evaluate the effectiveness of an Intelligent Multimodal Symptom Assessment and Response System, integrating patient-reported outcomes, wearable physiological data and image-based lesion assessment

Editor's Highlights-July 2026.

Targeted therapeutic advances in autoimmune blistering diseases have renewed attention to diagnostic precision, disease classification, and clinically relevant subgroups. In this issue, we focus on two aspects of this evolving field: recent developments in the diagnostics of autoimmune blistering diseases and the clinical features of checkpoint inhibitor-associated autoimmune blistering diseases and their treatment options. These contributions illustrate how therapeutic innovation, diagnostic progress, and recognition of emerging clinical contexts are closely interconnected.

Drug therapies and immune pathways in ocular manifestations of inflammatory skin diseases.

INTRODUCTION: Inflammatory skin diseases and ocular disorders frequently coexist due to shared embryological origins, overlapping immune pathways, and common barrier functions. The increasing use of targeted systemic therapies in dermatology has further highlighted the skin-eye interface, introducing both therapeutic benefits and iatrogenic ocular complications. AREAS COVERED: This review outlines the shared embryological and immunological mechanisms linking the skin and ocular surface, with emphasis on major inflammatory pathways, including TNF-α, Th17, and JAK-STAT signaling. We summarize t
