A 75-year-old male with eosinophilic pustulosis Ofuji complicated by superinfection with Pseudomonas aeruginosa and Proteus mirabilis is described. This is the first case of eosinophilic pustulosis with an antiepidermal antibody. This antibody was directed against the intercellular substance of the lower epidermis and it was detected by direct immunofluorescence in the patient's normal and lesional skin. Sixty-four biopsies were obtained from patients with a variety of vulvar disorders for direct immunofluorescence study. Twelve of 24 vulvar biopsies from patients with lichen sclerosus demonstrated deposition of fibrin in the upper dermis, while 7 vulvar biopsies showed fibrin deposition at the dermal-epidermal junction. Eight women had a bullous skin disease that occurred during pregnancy. One patient had clinical, histopathologic, and immunopathologic features of pemphigus. The other seven patients showed the features of herpes gestationis. Oral contraceptives reproduced the disease in the pemphigus patient and in one of the others. When the pemphigus patient was free of active disease, local intraepidermal acantholytic blister formation with deposition of IgG and C3 in the intercellular substance of the epidermis was induced by intradermal injection of progesterone.

Behçet's disease is a chronic inflammatory disorder of undetermined cause. The most frequent manifestations are aphthous ulceration of the upper aerodigestive tract, genital ulcers, ocular inflammation, and arthritis. Less frequent features include involvement of the CNS, thrombophlebitis, and large-vessel arteritis. Of utmost importance is the fact that aphthous ulceration of the upper aerodigestive tract will develop in all patients who have Behçet's disease; thus, they may come to the otorhinolaryngologist for evaluation. Desmosomes of a patient suffering from pemphigus vegetans were studied by electron microscopy and analysed by morphometry, The desmosomes were fewer in the patient's skin than in healthy individuals. The desmosomes of the patient were thin and short, showing pathological ultrastructures, viz. lack of median lines, single-sided attachment plaques and direct cell contact with some thickened cell membranes. These desmosomal changes were more manifest in the involved than in the uninvolved skin. The changes suggest deterioration or defective formation of desmosomes in pemphigus vegetans.

Numerous vegetating plaques developed in a 63-year-old man followed up for the management of pemphigus vulgaris of 5 years' duration. These lesions occurred over the trunk and right upper arm with no involvement in the intertriginous area, and were controlled with systemic steroid and ODT. Direct immunofluorescence test showed heavy deposition of fibrinogen in the papillary dermis. To our knowledge, the occurrence of pemphigus vegetans in the non-intertriginous area is rare.

A 69-year-old man with recurrent eruptions of pyoderma gangrenosum for 4 years is described. The patient also suffered from paraproteinemia (increased IgA with M-component) and transient acantholytic dermatosis resembling pemphigus erythematosus. He had no intestinal symptoms.

The association of pemphigus and thymoma has been investigated in a recent personally observed case and data from nineteen further cases published in the literature have been reviewed. From these twenty cases several prominent features emerge. Clinically, pemphigus of Senear-Usher type (pemphigus erythematosus) is most commonly found (11/20 cases). The association of HLA antigens with pemphigus in Jewish patients of various ethnic origins was investigated. Of 45 Ashkenazi patients, 31 (68.6%) had A26 (p less than 10(-5) and 32 (7175) had BW38 ( less than 10(-5)0. Among the 11 non-Ashkenazi patients, 7 (63.6%) had A26 and only 1 had BW38. A review of canine pemphigus vulgaris, pemphigus vegetans, pemphigus foliaceus, pemphigus erythematosus, and bullous pemphigoid shows them to share many clinicopathologic similarities with their human counterparts. Because of their numerous analogies in clinical presentations, histopathologic and immunopathologic findings, and therapeutic results, canine and human pemphigus and pemphigoid may represent a valuable area for comparative dermatologic research.

A variety of diseases of diverse etiology may produce oral pathology in a spectrum ranging from blisters through erosion to frank ulcers. The morphologic similarity of such lesions makes precise diagnosis difficult. Some of these diseases also produce cutaneous lesions which suggest or confirm a specific diagnosis. Selected diseases which produce oral ulcers are reviewed with an emphasis on those in which coexisting cutaneous lesions can significantly aid in diagnosis.

Twenty cases of pemphigus extracted from the literature and 19 patients from UCLA with pemphigus were studied in detail regarding the possible correlation of pemphigus titer and disease activity. A statistically significant relationship between titer and disease activity, as well as change in titer and change in disease activity, was found. However, in spite of this relationship, serial titers were not found to be consistent enough to be used reliably as a guide to therapy or prognosis in pemphigus.

The history of keratoprosthetics and the pioneer work of Cardona et al. is described and the technical improvements in this method over the past two decades are reviewed. In addition, the results achieved in 17 personal cases are recorded. Visual acuity was definitely improved in 7 cases, transient improvement was obtained in 5 cases, 3 patients retained poor vision, and 2 cases showed deterioration. Implantation of artificial keratoprostheses is still at a pioneer stage and further progress can be expected in this field.

A 62 year old female was treated for several years with DDS because of Duhring-disease like symptoms. She developed erythro-melanoderma, generalized lymphadenitis and pachyderma. Histologically there were eosinophilic spongiosis and intraepidermal vesiculation, blood eosinophilia up to 42%, and intercurrent streptococcal septicemia, immunohistologically in vivo bound pemphigus antibody, but no papillary or linear IgA. Indirect immunofluorescence was negative. Paraproteinemia was excluded.

The technique of direct immunofluorescence has been applied to skin biopsy specimens fixed in formalin and embedded in paraffin wax. The results have been compared with those obtained by using snap-frozen biopsy specimens from the same patients. Trypsinisation of the dewaxed material allowed subsequent detection of immunoglobulins, complement, and fibrinogen. When compared to the fluorescence in the snap-frozen specimens, the staining in the paraffin sections was less bright and there was a higher rate of negatives. Even so, it was possible to establish the diagnosis in most cases of pemphigus
