Immunofluorescence studies were made on light (L) chain of immunoglobulins in patients with pemphigus. These patients fell into two groups: a subcorneal bulla group, consisting of 10 patients with Senear-Usher syndrome, and one with pemphigus foliaceus, and a suprabasal bulla group consisting of 8 patients with pemphigus vulgaris. Five skin and two oral biopsies from patients with South American pemphigus foliaceus (SAPF) were studied by electron and immunoelectron microscopy for the ultrastructural localization of bound immunoglobulin in epidermal and oral lesions. Electron microscopy showed the tonofilament-desmosome complex to be preserved in the various layers of the epidermis. A 78-year-old man had an acute eruption of lesions that clinically resembled seborrheic keratoses. Microscopic examination of the lesions indicated acantholysis and dyskeratosis primarily in the granular layer. The patient's serum contained antibodies to intercellular substance. Forty-eight biopsy samples submitted to a diagnostic immunofluorescence service have been stored at -20 degrees C and sequentially re-tested at monthly intervals to ascertain the duration for which immunoglobulins and complement can be identified in these samples in typical and diagnostic patterns of distribution. Two female patients are reported in whom the clinical picture closely corresponded to dermatitis herpetiformis Duhring or bullous pemphigoid, while acantholytic changes were present histologically. Two cases of pemphigus vegetans are presented. One, an example of the Hallopeau type of the disease, was found to have early lesions that simulated eosinophilic spongiosis clinically and histologically. The other had lesions that were largely confined to the oral mucosa and was also considered to be the Hallopeau type of the disease in a limited form. The cases of these two patients were compared with a typical case of eosinophilic spongiosis. All were controlled with dapsone therapy.

Seborrheic pemphigoid first described by Schnyder is considered to be a clinical variant of bullous pemphigoid which resembles pemphigus erythematosus. Ruptured bullae and erosions covered with crusts involving predominantly the median parts of the trunc characterize the clinical picture. Histologic and immunofluorescence findings substantiate the diagnosis of pemphigoid. The clinical, histopathologic and immunofluorescence features as well as the course of the disease in a 74 year old woman are described.
