Diagnosis of Autoimmune Blistering Diseases: Dermatopathology and Other Laboratory Updates.

Autoimmune blistering diseases (AIBD), an immunologically diverse group associated with intraepithelial or subepithelial blistering, can present with protean clinical or histopathologic findings. Accurate diagnosis is important to predict clinical course, stratify risk for certain associated systemic conditions, and determine patient eligibility for various therapies. While helpful clues can be gleaned from microscopic evaluation of tissue, definitive diagnostic changes are not always present. Therefore, it is important for clinicians and dermatopathologists to recognize the various clinicopat

Real-World Experience With Single Ultra-Low Dose Rituximab for Remission Induction in Pemphigus.

Standard-dose rituximab (SDRTX) is the established treatment for pemphigus, but optimization of cost-effectiveness and safety remains desirable. We retrospectively evaluated the efficacy of a single ultra-low dose RTX infusion (ULRTX; 100 or 200 mg) for remission induction in 12 patients with mild-to-severe pemphigus (median baseline Pemphigus Disease Area Index [PDAI] 29.5). 9 patients (75%) achieved complete remission on minimal therapy (CRMT), and among them 5 (41.7%) subsequently achieved complete remission off therapy (CROT), with a median time to CRMT and CROT of 24 weeks and 61 weeks, r

Successful treatment of paraneoplastic pemphigus associated with Castleman's disease using membrane therapeutic plasma exchange: A case report.

Paraneoplastic pemphigus (PNP) is a rare autoimmune blistering disease. We report a case in which membrane therapeutic plasma exchange (mTPE) effectively improved oral lesions in a patient with unicentric Castleman's disease(UCD)-associated PNP characterized by elevated anti-desmoglein-3 antibodies. As the lesions persisted despite multiple immunosuppressive therapies and tumor resection, mTPE was introduced to eliminate circulating autoantibodies. Plasma was replaced with 5% human albumin equivalent to about 1.1 times the estimated plasma volume. Two courses of mTPE were performed, each compr

Paraneoplastic dermatoses in patients with hematologic malignancy.

Malignancies can elicit a vast array of systemic findings in patients that are not directly driven by tumor invasion. These paraneoplastic syndromes may result from mechanisms ranging from ectopic hormone expression to crossreactive immune responses. Many of these paraneoplastic syndromes can even resemble autoimmune dermatoses. Paraneoplastic dermatoses are especially common in patients with hematologic malignancies. We review the fundamental mechanisms underlying immune-mediated paraneoplastic dermatoses in patients with hematologic malignancies and describe several common paraneoplastic der

Case Report: A rare co-occurrence of IgA pemphigus and pyoderma gangrenosum associated with IgA-κ type monoclonal gammopathy of undetermined significance: a 19-year diagnostic and therapeutic journey.

We report a complex case of neutrophilic dermatosis (ND) in a 61-year-old woman with a 19-year history of recurrent, pruritic, and painful vesiculopustular eruptions. Her clinical course was marked by evolving diagnoses, from pustular vasculitis to IgA pemphigus and pyoderma gangrenosum (PG). Concurrent investigations revealed an IgA-κ type monoclonal gammopathy of undetermined significance (MGUS). This case highlights the rare association between neutrophilic dermatoses (ND) and haematological disorders, and illustrates the therapeutic challenges posed by comorbidities such as osteoporosis an

Increased Mucocutaneous FDG Activity in Paraneoplastic Pemphigus Associated With Follicular Dendritic Cell Sarcoma Transformed From Castleman Disease.

Paraneoplastic pemphigus is a rare and fatal autoimmune mucocutaneous blistering disease associated with underlying tumors. We describe FDG PET/CT findings in a case of paraneoplastic pemphigus associated with pelvic follicular dendritic cell sarcoma transformed from Castleman disease. FDG PET/CT showed intense activity in the oral cavity, diffuse, heterogeneous activity in the skin, linear activity in the upper esophagus, focal activity in the anus, and heterogeneous activity in the pelvic tumor. This case indicates that paraneoplastic pemphigus should be considered as a differential diagnosi

Population-Based and Routinely Collected Health Data Studies of Immunobullous Diseases: A Scoping Review.

BACKGROUND: Validated case definitions are essential for reliable epidemiological research using routinely collected health data. The extent to which population-based studies on autoimmune blistering diseases (AIBDs) use validated case definitions is uncertain, and variability in definitions may limit the generalizability of findings. OBJECTIVE: To describe definitions of AIBDs used in population-based epidemiologic studies and whether these definitions have been validated. METHODS: MEDLINE and Embase were searched for epidemiologic studies on pemphigoid, pemphigus and their variants, dermat

Tumour necrosis factor-like weak inducer of apoptosis (TWEAK) is associated with inflammation and disease severity in pemphigus.

BACKGROUND: Pemphigus vulgaris (PV) is a severe autoimmune blistering disease characterized by immune inflammatory response imbalance. The role of the proinflammatory cytokine tumour necrosis factor-like weak inducer of apoptosis (TWEAK) in the pathogenesis of PV is unclear. OBJECTIVES: To evaluate the inflammatory status in PV and investigate the role of TWEAK in initiating and exacerbating inflammation, as well as its effects on desmoglein (Dsg) internalization at keratinocyte membranes induced by PV cell models and individual PV IgG components. METHODS: We collected serum samples from 60

The Role of Genetic Testing in the Differential Diagnosis of Pustular Dermatoses: A Pediatric Case of Generalized Pustular Psoriasis Associated with the IL36RN Variant.

General pustular psoriasis (GPP) is a rare, potentially life-threatening neutrophilic dermatosis. Pediatric cases are uncommon and often misdiagnosed due to overlapping clinical and histopathological features with other pustular dermatoses. We present a case of an 11-year-old boy, initially diagnosed with Sneddon-Wilkinson syndrome, who presented with disseminated pustular eruptions, with no response to antibiotics, dapsone, and glucocorticosteroids. In histopathology, we observed subcorneal neutrophilic pustules. Due to atypical features and poor treatment response, the patient underwent gene

A Link Between Dietary Modifications Focused on Unsaturated Fatty Acids and the Course of Oral Autoimmune Diseases: A Systematic Review.

Background/Objectives: The aim of this systematic review was to analyze the impact of dietary modifications on unsaturated fatty acids (UFAs) intake on the course of three oral autoimmune conditions: oral lichen planus (OLP), recurrent aphthous stomatitis (RAS), and pemphigus. These acids serve a structural function as key components of cell membrane phospholipids, influencing their fluidity and membrane receptor activity. The proper ratio of these acids in the diet may be crucial for maintaining normal functioning of the human immune system. Methods: The following bases were searched with no

Noninvasive Imaging Techniques as Modern Diagnostic Tools in Desquamative Gingivitis: Focus on RCM, OCT, and LC-OCT.

BACKGROUND: Desquamative gingivitis is a clinical condition characterized by erythema, blisters, and erosions within the gingiva. It may be a manifestation of oral lichen planus or autoimmune bullous diseases, such as pemphigus vulgaris and mucous membrane pemphigoid. The identification of the cause of desquamative gingivitis typically involves the analysis of clinical manifestations, histopathology, and immunopathology evaluation. This article reviews available data on the application of noninvasive imaging techniques (reflectance confocal microscopy, optical coherence tomography, and line-fi

3D organotypic skin models recapitulate autoantibody-driven pemphigus pathomechanisms and targeted therapeutic response.

Advanced three-dimensional (3D) tissue models that recapitulate autoimmune disease progression can enable mechanistic studies and accelerate the development of targeted therapies with reduced reliance on systemic immunosuppression. Here, we present a dynamic biofabrication strategy combining 3D bioprinting and cell self-organization to create multilayered skin constructs with a stratified epidermis atop a vascularized, fibroblast-remodeled dermis. This platform reconstitutes native skin architecture to model pemphigus vulgaris, an autoantibody-driven blistering disorder. Tightly reassembled ke

Expert consensus on oral management in autoimmune bullous diseases, erythema multiforme and SJS/TEN.

BACKGROUND: Autoimmune bullous diseases (AIBD), Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS-TEN) and erythema multiforme (EM) often present with clinically similar features in the oral mucosa: classically widespread mucosal erosions or ulceration. Despite this, there are no formalized guidelines for the specific management of the oral manifestations of these conditions. OBJECTIVES: We sought to establish an consensus on the management of oral involvement of AIBD, SJS-TEN and EM using the Delphi method. METHODS: Participants were sent a survey comprising 62 statements organiz

Oral Microbial Dysbiosis and Pathogenic Functional Shifts in Mucosa-Dominant Pemphigus Vulgaris and Low-Risk Mucous Membrane Pemphigoid.

BACKGROUND: Mucosal-dominant pemphigus vulgaris (MD-PV) and low-risk mucous membrane pemphigoid (LR-MMP) are autoimmune bullous diseases primarily affecting the oral mucosa, yet their oral microbiome profiles remain inadequately characterized. METHODS: Using 16S rRNA sequencing of saliva from 21 MD-PV patients, 26 LR-MMP patients, and 14 healthy controls (HC), we analyzed microbial diversity, differential taxa (LEfSe), and functional potential (PICRUSt2/BugBase). RESULTS: Both patient groups showed significant microbial restructuring without major richness changes, featuring increased Firmic
