A comprehensive, population level evaluation of previously reported drug triggers of pemphigus highlights immunomodulatory capacity as a common characteristic.

QUESTION: Can previously reported, largely anecdotal associations between exposure to any of a comprehensive list of putative trigger drugs and the development of pemphigus be reproduced using population level data? FINDINGS: In this series of observational, retrospective, case-control, pharmacovigilance analyses of the FDA Adverse Event Reporting System, the odds of reporting the adverse event pemphigus were significantly elevated among individuals exposed to 11/36 previously reported trigger drugs namely, gold sodium thiomalate, penicillamine, piroxicam, rifampin, hydroxychloroquine, imiqui

Pemphigus Vulgaris Autoantibodies Induce an Endoplasmic Reticulum Stress Response.

Desmosomes are intercellular junctions that mediate cell-cell adhesion and are essential for maintaining tissue integrity. Pemphigus vulgaris (PV) is an autoimmune epidermal blistering disease caused by autoantibodies (IgG) targeting desmoglein 3, a desmosomal cadherin. PV autoantibodies cause desmosome disassembly and loss of cell-cell adhesion; however, the molecular signaling pathways that regulate these processes are not fully understood. Using high-resolution time-lapse imaging of live keratinocytes, we found that endoplasmic reticulum (ER) tubules make frequent and persistent contacts wi

Spontaneous resolution of pemphigus vulgaris-induced desquamative gingivitis. A case report and brief review of the literature.

Desquamative gingivitis (DG) is important in oral medicine because it is not a pathologic entity itself; rather, it is a clinical manifestation of dermatological disease most frequently associated with chronic inflammatory (e.g., oral lichen planus) and autoimmune diseases, mainly pemphigus vulgaris and mucous membrane pemphigoid, and several cases have been reported till date. Herein, we describe a case of pemphigus vulgaris-associated DG with unusual clinical behavior, in which the gingival lesions spontaneously resolved after consistently showing no response to various treatments. Additiona

Antidesmoglein 1 and 3 serum IgG and positivity by direct immunofluorescence microscopy is associated with relapse in pemphigus in a prospective bicontinental study.

BACKGROUND: Prospective studies identifying immunological parameters that can predict clinical relapse in pemphigus are scarce. OBJECTIVE: To periodically assess immunological parameters in patients with pemphigus vulgaris and foliaceous in remission to understand immunological events preceding clinical relapse. METHODS: A total of 105 patients were included. Baseline assessment included direct immunofluorescence (DIF), serum IgG against desmoglein (Dsg) 1, IgG, IgG1, and IgG4 against Dsg 3, IgG against the extracellular domains 1 and 2 of Dsg 3, IgG against muscarinic (M3)-AchR, and periphe

Comparative analysis of demographics and clinical outcomes in patients with pemphigus vulgaris and foliaceus with high versus normal anti-desmoglein levels post-complete remission.

Some patients with pemphigus continue to have elevated levels of desmoglein autoantibodies (Dsg Ab) even after achieving remission. Thus, it is crucial to gain a deeper understanding of these patients' demographics and clinical patterns to improve prognosis, prevent complications, and devise individualized management strategies. This study utilized electronic patient records from a dermatology registry, focusing on PV and PF patients (diagnosed based on clinical and histopathologic diagnosis which included DIF) who achieved complete remission. We analyzed demographic and clinical data, includi

Autoimmune pemphigus: difficulties in diagnosis and the molecular mechanisms underlying the disease.

Recently recognised as a desmosomal disorder, autoimmune pemphigus remains severe in some of its forms, such as pemphigus vulgaris. This review is divided into four parts. "Cellular and molecular mechanisms in autoimmune pemphigus" discusses in detail antigenic targets, antibodies, immunological and genetic mechanisms of apoptosis and the involvement of cells and organelles (keratinocytes, lymphocytes, eosinophils and neutrophils) in different forms of pemphigus. These advances have led to today's first-line biologic therapy for pemphigus. The section "Specific features in the diagnosis of imm

Epitomic profiling and functional characteristics of pemphigus vulgaris autoantibody binding to keratinocyte M3 muscarinic acetylcholine receptor.

Patients with pemphigus vulgaris (PV) develop IgG autoantibodies (AuAbs) binding to keratinocyte desmogleins (Dsg), acetylcholine (ACh) receptors, mitochondrial proteins, and some other self-antigens. In this study, we identified linear and discontinuous peptide tetrameric epitope segments (ES) of M3 muscarinic ACh receptor (M3AR) targeted by different anti-M3AR AuAbs. As positive controls, we identified Dsg1 and Dsg3 ES targeted by PV sera. Healthy individuals also possessed natural antibodies targeting M3AR, Dsg1 and Dsg3 epitopes that were different from those targeted by AuAbs produced by

Lipid Profile Scores Predict Severity of Pemphigus: A Cross-Sectional Study.

BACKGROUND: There is little evidence about the relationship between lipid indices and the severity of pemphigus vulgaris (PV) disease. In this cross-sectional study, we try to find out the exact relationship between three dietary lipid indices (PUFA/SFA ratio and ω-6/ω-3 index) and the severity of PV disease. METHODS: In this hospital-based cross-sectional study, a total of 138 pemphigus vulgaris cases were studied, of which 108 had PDAI ≤15, and 30 had PDAI>15. Dietary intakes were measured a valid 168-item FFQ. To calculate the lipid indices, the data received from diet were used. RESULTS:

Plasmapheresis for Facilitating Readministration of Rituximab After Paradoxical Exacerbation in Pemphigus Vulgaris: A Case Report.

Rare reports of post-rituximab exacerbation of pemphigus vulgaris (PV) have been published, with some therapeutic protocols proposed for managing this condition. A 40-year-old female with PV experienced an exacerbation of mucocutaneous lesions following rituximab administration. She failed to respond to conventional immunosuppressive agents, intravenous immunoglobulin, and ocrelizumab, so plasmapheresis was administered. Then, further doses of rituximab were prescribed, and mucocutaneous lesions improved significantly. We propose plasmapheresis as a possible treatment for post-rituximab PV exa
