BACKGROUND: Autoantibodies in the blistering skin disease pemphigus primarily target desmosomal cadherins and cause loss of keratinocyte adhesion and epidermal blistering via signalling events. Pemphigus vulgaris (PV) is associated with autoantibodies (PV IgG) against desmoglein (Dsg)1 and Dsg3 and pemphigus foliaceus (PF) with antibodies against Dsg1 only. In previous studies, protein kinase C (PKC) inhibition was protective in murine but not in human epidermis.
OBJECTIVES: To investigate the roles of PKC subtypes in in PV IgG-induced epidermal blistering in human skin.
METHODS: We applied

Pemphigus represents a family of acantholytic autoimmune vesiculobullous diseases. It is classified into multiple subtypes, the two most common of which are pemphigus vulgaris and pemphigus foliaceus. The transition between these two subtypes is uncommon. In this report, we discuss a unique case of biopsy-proven pemphigus foliaceus transitioning to pemphigus vulgaris.

UNLABELLED: <p>Introduction: Pemphigus is an autoimmune bullous disease caused by autoantibodies against desmoglein (DSG) 1 and/or 3 and comprises two main subtypes: pemphigus vulgaris (PV) and pemphigus foliaceus (PF). PV affects the skin and/or mucosa and includes two forms: mucocutaneous PV (mcPV; anti-DSG1 and anti-DSG3) and mucosal PV (mPV; anti-DSG3). PF, characterized by anti-DSG1 autoantibodies, is limited to the skin. Pemphigus is effectively treated with rituximab, a B-cell-depleting therapy. However, about half of patients relapse, with a subset exhibiting a shift in clinical subtyp

INTRODUCTION: Autoimmune bullous diseases (AIBDs) are rare, tissue-specific autoimmune diseases of the skin, and corresponding autoantibodies have been proved to be pathogenic. Recently, we reported that variations in platelet-related parameters may reflect the fluctuations of circulating AIBD-correlated antibody titers during the disease progression of 1 patient with AIBD. The purpose of this article is to further investigate the possible correlation between autoantibody titers and platelet-related parameters in patients with AIBD.
METHODS: This study collected data on autoantibody titers an

Autoimmune blistering diseases (AIBDs) are mediated by autoantibodies targeting structural proteins of the epidermis or dermo-epidermal junction. Since the introduction of COVID-19 mRNA vaccines, isolated cases of pemphigus temporally associated with vaccination have been described, although causal relationships remain uncertain. We report two patients who developed pemphigus following COVID-19 mRNA vaccination and showed an exceptionally favorable clinical course. A 66-year-old man developed pemphigus foliaceus one week after his second vaccine dose, and a 45-year-old woman developed pemphigu

BACKGROUND: Thiopurine methyltransferase (TPMT) is responsible for the inactivation of azathioprine, and is widely used to treat cutaneous autoimmune diseases. TPMT activity is inherited as an autosomal codominant trait and ranges from high to undetectable levels in different individuals. Low TPMT activity may result in a higher risk of adverse effects (AEs), whereas high TPMT activity may result in potential treatment failure.
OBJECTIVE: To assess TPMT levels in patients with cutaneous autoimmune diseases and evaluate the correlation between TPMT levels and AEs of azathioprine and clinical r

INTRODUCTION: While rituximab has been approved for moderate-to-severe pemphigus vulgaris, data on pemphigus foliaceus (PF) remain limited because of the small number of patients included in therapeutic trials. However, current European guidelines recommend rituximab for the first-line treatment of patients with moderate-to-severe PF. This systematic review and meta-analysis aimed to evaluate the efficacy and safety of rituximab in PF.
METHODS: This systematic review and meta-analysis was conducted according to PRISMA guidelines and registered in PROSPERO (CRD42023428459). Studies published i

Pemphigus encompasses autoimmune blistering diseases driven by autoantibodies to desmosomal proteins. Disruption of keratinocyte adhesion produces acantholysis and epidermal fragility. Anti-CD20 B-cell depletion and IgG lowering strategies have transformed care, but many patients relapse, some variants respond incompletely, and durable drug-independent tolerance remains uncommon across variants. These gaps highlight that pemphigus is not a simple linear B-cell disorder, but an autoantibody disease sustained by a network linking the innate immunity, the adaptive lymphocytes, and the stromal nic

Although population-based studies have reported an increased risk of bullous pemphigoid and pemphigus associated with dipeptidyl peptidase-4 (DPP-4) inhibitors, there is limited, well-designed epidemiological evidence for pemphigus vulgaris (PV), which has only been supported by case reports thus far. This study aimed to assess whether DPP-4 inhibitor use in type 2 diabetes patients is associated with PV and pemphigus foliaceus (PF) development. A retrospective cohort study was conducted using the Korean National Health Insurance databases. The study included 1 228 526 patients newly diagnosed

Autoimmune bullous diseases (AIBDs), including pemphigus vulgaris (PV), pemphigus foliaceus (PF), and bullous pemphigoid (BP), are mediated by autoantibodies against desmogleins (DSG1, DSG3) or hemidesmosomal proteins (BP180, BP230). While enzyme-linked immunosorbent assay (ELISA) is commonly applied for antibody detection, chemiluminescent immunoassay (CLIA) offers advantages such as a broader dynamic range and higher automation. To compare the diagnostic performance of CLIA and ELISA for detecting autoantibodies in patients with AIBDs, we collected 255 serum samples (92 controls, 84 BP, 69 P

BACKGROUND: Pemphigus vulgaris (PV) and pemphigus foliaceus (PF) are rare, chronic, potentially life-threatening, immunoglobulin (Ig)G-mediated, autoimmune skin and/or mucosal blistering diseases. Efgartigimod, a human IgG1 antibody Fc fragment, blocks the neonatal Fc receptor, decreasing IgG recycling and reducing both healthy and pathogenic IgG autoantibody levels.
OBJECTIVES: To investigate the efficacy, safety and impact of subcutaneous efgartigimod PH20 (co-formulated with recombinant human hyaluronidase PH20) in conjunction with prednisone in the treatment of pemphigus.
METHODS: Adult

BACKGROUND: Although an increasing number of cases of new-onset pemphigus have been reported following drug exposure or vaccination, the available evidence remains fragmented and distinguishing true trigger-associated disease from coincidental onset continues to represent a major clinical challenge. We performed a systematic review to evaluate the available evidence on drug- and vaccine-associated new-onset pemphigus and to compare their clinical characteristics, management, and outcomes.
METHODS: This systematic review was conducted according to the PRISMA 2020 statement and prospectively re

INTRODUCTION: Pemphigus vulgaris (PV) is a rare autoimmune blistering disorder that primarily affects the skin and mucous membranes. Drug-induced pemphigus (DIP) is an uncommon variant that is triggered by certain medications. The introduction of novel hypoglycemic agents, such as glucagon-like peptide-1 receptor agonists (GLP-1RA), has been associated with an increase in drug-induced bullous diseases. However, reports of pemphigus induced by these agents remain exceedingly rare.
CASE PRESENTATION: We report the first case of liraglutide-induced PV in a 40-year-old female patient with type 2

Pemphigus Vulgaris (PV) is a chronic autoimmune blistering disease. Although pharmacological agents, particularly those containing thiol groups, have been identified as precipitating factors, reports of non-thiol drugs, such as nonsteroidal anti-inflammatory drugs (NSAIDs), causing PV remain limited. We report a case of a 41-year-old Myanmar female who developed generalized bullous eruptions, oral ulcers, and ocular pain after the concurrent use of ibuprofen and penicillin V. She presented with extensive cutaneous erosions complicated by sepsis and malnutrition. Although penicillin V was initi

Pemphigus vegetans (PVeg) is a rare variant of pemphigus vulgaris characterized by pustules and/or vegetating plaques, preferentially affecting the flexural areas. The diagnosis of PVeg depends on clinical grounds, histopathology, and direct immunofluorescence. Systemic corticosteroid is the first-line therapy for PVeg, and immunosuppressive treatments such as azathioprine, mycophenolate mofetil, and intravenous immunoglobulins can be used to improve remission rates. Also, there have been a few reports on the use of rituximab for treating PVeg. Here, we present a rare case of PVeg successfully
