Pemphigus vegetans: insights from a systematic review of the literature.

BACKGROUND: Pemphigus vegetans (PVg) is a rare variant of pemphigus vulgaris with distinct clinical and histological features, with fewer than 200 cases of patients with the condition reported. OBJECTIVES: To systematically review cases of patients with PVg and summarize clinical, histopathological, immunological features, treatment and outcomes. METHODS: Following PRISMA guidelines, PubMed, Embase and Web of Science were searched (1889 to January 2025) using 'pemphigus AND vegetans'. Eligible studies included case reports, series and observational studies. Data on demographics, subtypes, hi

Pre-Referral Provisional Diagnoses and Final Diagnostic Subtypes of Pemphigus at a Regional Tertiary Dermatology Center in Japan: A Single-Center Retrospective Study.

Pemphigus comprises autoimmune blistering diseases with heterogeneous clinical presentations, but pre-referral provisional diagnoses in regional tertiary dermatology practice have been less well characterized. We retrospectively reviewed 35 consecutive patients diagnosed with pemphigus at Toyama University Hospital, a regional tertiary dermatology center in Japan, between January 2011 and May 2026. Final diagnoses were established using clinical, histopathological, immunopathological, and serological findings. We extracted referral characteristics, pre-referral provisional diagnoses, final dia

Isolated oral pemphigus vegetans - A case report.

Pemphigus vegetans, a rare subtype of pemphigus, is characterized by vegetative plaques, primarily affecting intertriginous areas. We report a case of a 55-year-old female who presented with a three-month history of multiple blisters on the tongue. The blisters ruptured spontaneously within 2-3 days, subsequently forming persistent vegetative plaques over the affected areas. Clinical, hematological and histopathological findings were compatible with pemphigus vegetans. The patient responded favorably to systemic and topical corticosteroids and adjunctive therapies, with marked improvement obse

Desquamating dermatological conditions: The burns surgeon's guide to diagnosis and management.

Desquamating or blistering skin conditions may be referred to burns and plastic surgeons owing to the morphological similarity to burn injuries, given an increasing shortage of dermatology inpatient beds within the context of NHS care. Desquamating skin conditions, similar to burn injuries, carry the potential to be severe, debilitating and potentially fatal; however, they present more complexity owing to their underlying aetiologies. These conditions may present with similar physiological challenges, dressing requirements and need for extensive multidisciplinary input as burn injuries, and th

Novel Low-y⁺ Roughness Correlation for Hydrogen-Fired Jet Burners Validated Under Isothermal and Reacting Conditions.

The objective of the present work was to study the effects of contamination on the reflective properties of groundcovers used for enhancing fruit colouration in the orchard. Contamination also affects longevity and possible sustainable re-use of materials. A white, woven textile (polypropylene Lumilys™) and silver aluminium foil were experimentally contaminated with soil, similar to the situation after an autumn storm in a fruit orchard. Clean material served as control.Using a spectrophotometer (StellarNet; Tampa, FL, USA), vertically directed (0°) and diffuse (45°) light reflection in the ra

Case report: Atypical SJS/TEN-like fulminant pemphigus vulgaris: diagnostic challenges and the potential impact of anti-Dsg1 titers and CMV reactivation.

Pemphigus vulgaris (PV) is a chronic autoimmune blistering disease characterized by intraepidermal flaccid blisters of the skin and mucous membranes. Although typically chronic and relapsing, rare fulminant variants have been described. We report a life-threatening case of PV in a previously healthy 55-year-old woman who developed a rapidly progressive, Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN)-like phenotype with extensive epidermal detachment and mucocutaneous involvement. Histopathological analysis demonstrated suprabasal acantholysis, and direct immunofluorescence showe

Non-COVID-19 Vaccinations and the Induction of Autoantibodies in Pemphigus Diseases: A Review of the Speculative Issue and Our Clinical-Laboratory Experience.

Background: Pemphigus diseases are rare autoimmune blistering disorders mediated by pathogenic autoantibodies directed mainly against desmoglein 1 and desmoglein 3. Although most cases are considered idiopathic, external triggers that can disrupt immune tolerance have been described. Vaccination has been discussed as a potential precipitating factor in autoimmune skin diseases. However, the relationship between vaccination and the induction of pemphigus-related autoantibodies has not been comprehensively summarized. Methods: We conducted a narrative review of all available studies published in

Drug-Induced Pemphigus Vulgaris Following Ceftriaxone Use.

Pemphigus vulgaris (PV) is a rare, potentially life-threatening autoimmune blistering disorder characterized by intraepidermal acantholysis caused by IgG autoantibodies directed against desmoglein 1 and 3. Although most cases are idiopathic, certain medications, including antibiotics, have been implicated as potential triggers in susceptible individuals. We report the case of a 61-year-old man with a history of nonischemic cardiomyopathy, chronic kidney disease, and substance use disorder who developed widespread flaccid bullae six days after initiation of intravenous ceftriaxone for a suspect

Early-Onset Neutropenia Induced by Rituximab in a Patient With Pemphigus Vulgaris: A Case Report and Literature Review.

Rituximab (RTX), a chimeric anti-CD20 monoclonal antibody, is increasingly used in autoimmune diseases. Although generally safe, it may rarely induce cytopenias, particularly early-onset neutropenia (EON) or late-onset neutropenia (LON), with LON being more frequent. We report the case of a 48-year-old female patient with pemphigus vulgaris who developed EON 21 days after RTX initiation. After the second RTX infusion, she presented with severe neutropenia, anemia, fever, respiratory symptoms, and Staphylococcus aureus sepsis. Despite antibiotics and intensive care, she died of refractory septi

Pneumonia caused by co-infection with Mycobacterium tuberculosis and Pneumocystis jirovecii leading to acute respiratory distress syndrome in an HIV-negative immunocompromised patient: a case report and literature review.

BACKGROUND: Pulmonary tuberculosis (PTB) and Pneumocystis jirovecii pneumonia (PJP) often occur in immunosuppressed populations, particularly in individuals with human immunodeficiency virus (HIV) infection. However, co-infection with these two pathogens resulting in acute respiratory distress syndrome (ARDS) has been less frequently reported, especially in HIV-negative patients. CASE PRESENTATION: We report the case of a 68-year-old immunosuppressed male patient with pneumonia caused by co-infection with Mycobacterium tuberculosis and Pneumocystis jirovecii, leading to ARDS, who was successfu

Longitudinal assessment of physiological and mental health changes following acute COVID-19 in Uganda.

BACKGROUND: Understanding synapse-specific effects of neuroinflammation can provide mechanistic and therapeutically relevant insights across the spectrum of neurological diseases. METHODS: We applied neuron-specific proteomic biotinylation in vivo, differential centrifugation of brain for crude synaptosome enrichment (P2 fraction) and mass spectrometry (MS) analysis of biotinylated proteins to derive native-state proteomes of Camk2a-positive neurons and their corresponding P2 synaptic compartments. Next, in an in vivo model of systemic lipopolysaccharide (LPS) dosing, we examined the effects

Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Correlation of Clinical and Histopathological Features With Anti-Desmoglein Antibody Profile in Pemphigus Patients.

BACKGROUND: The desmoglein compensation hypothesis (DCH) proposes that clinical and histopathological features of pemphigus correlate with differential expression of desmoglein 1 (Dsg1) and desmoglein 3 (Dsg3) in skin and mucosa, and corresponding autoantibody profiles. While DCH explains many classical patterns, increasing exceptions have challenged its universality. AIM: This study aimed to correlate clinical phenotype and histopathological split level with anti-Dsg1 and anti-Dsg3 antibody profiles in pemphigus vulgaris (PV) and pemphigus foliaceus (PF), and evaluate consistency with DCH.

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Surgical Methods for the Management of Intra-articular Distal Humerus Fracture: A Systematic Review and Bayesian Network Meta-analysis.

BACKGROUND: Intra-articular distal humerus fractures (DHF) carry a technical challenge for orthopedic surgeons, owing to the complex geometry of the distal humerus. Surgical management is the standard of care for these fractures. The aim of this network meta-analysis was to compare the clinical outcomes of the different open reduction and internal fixation (ORIF) and arthroplasty techniques for the management of intra-articular DHFs. METHODS: The databases Medline, Embase, and CENTRAL were searched for relevant studies. Randomized controlled trials (RCTs) were included if they matched the fol
