Ultraviolet light applied to the normal appearing skin of a patient with pemphigus erythematosus produced clinical and histological changes, typical of the disease, in the irradiated skin. The action spectrum, studied with monochromatic light, was in the range of UVB.

A patient affected by rheumatoid arthritis developed pemphigus erythematosus after 14 months of treatment with thiopronine. The lesions healed spontaneously after 4 weeks' withdrawal of the drug. Whereas the pemphigus erythematosus induced by D-penicillamine is a quite rare but well documented side effect, we believe our case to be the first one reported as being due to thiopronine. It is very interesting to note that the two drugs are very similar with regard to chemical structure, mechanism of action, therapeutic indications and also side effects.

Biopsies from the oral mucosa of three patients (two with pemphigus vulgaris and one with pemphigus erythematosus) were taken for both freeze-fracturing and thin-sectioning studies. D-Penicillamine is a chelating agent which is effective in the treatment of Wilson's disease, cystinuria, and lead poisoning. The association of pemphigus and thymoma has been investigated in a recent personally observed case and data from nineteen further cases published in the literature have been reviewed. From these twenty cases several prominent features emerge. Clinically, pemphigus of Senear-Usher type (pemphigus erythematosus) is most commonly found (11/20 cases). The association of HLA antigens with pemphigus in Jewish patients of various ethnic origins was investigated. Of 45 Ashkenazi patients, 31 (68.6%) had A26 (p less than 10(-5) and 32 (7175) had BW38 ( less than 10(-5)0. Among the 11 non-Ashkenazi patients, 7 (63.6%) had A26 and only 1 had BW38. A review of canine pemphigus vulgaris, pemphigus vegetans, pemphigus foliaceus, pemphigus erythematosus, and bullous pemphigoid shows them to share many clinicopathologic similarities with their human counterparts. Seborrheic pemphigoid first described by Schnyder is considered to be a clinical variant of bullous pemphigoid which resembles pemphigus erythematosus. Ruptured bullae and erosions covered with crusts involving predominantly the median parts of the trunc characterize the clinical picture. Histologic and immunofluorescence findings substantiate the diagnosis of pemphigoid. The clinical, histopathologic and immunofluorescence features as well as the course of the disease in a 74 year old woman are described.

A sixty year old woman was found to have a benign thymoma and pemphigus erythematosus. Her skin disease was controlled with oral and topical corticosteroids. The association between thymoma and pemphigus erythematosus is discussed, and the literature reviewed. Most previous cases have also had myasthenia gravis and skeletal muscle antibodies. This is the first case in which neither of these was present.

This report describes the immunofluorescence (IF) findings in sequential skin biopsies and serum obtained during a follow-up study of a patient who developed pemphigus lesions after D-penicillamine treatment of arthritis. The distribution in the skin of IgG, IgM and C3 varied with the date, type and site of the biopsy sample. Thus IF patterns showed features of (a) pemphigus vulgaris, (b) pemphigus foliaceus and (c) pemphigus erythematosus. Indirect IF examination indicated the presence of circulating intercellular epithelial antibody of both the IgG and IgM class.
