C5-Predominant Segmental Zoster Paresis Presenting With Right Shoulder Abduction Weakness Before Cutaneous Eruption: A Case Report.

Segmental zoster paresis (SZP) is an uncommon motor complication of herpes zoster (HZ) characterized by focal, asymmetric weakness in a myotomal distribution. Motor weakness usually follows the cutaneous eruption, and pre-eruptive weakness can mimic cervical radiculopathy, brachial plexopathy, neuralgic amyotrophy, or other causes of acute focal limb weakness. A 57-year-old woman developed right shoulder pain and, five days later, became unable to abduct her right arm. A vesicular eruption appeared on the lateral aspect of the right upper arm three days after the onset of weakness, and HZ was

Varicella-zoster virus encephalitis in an elderly patient with influenza A infection.

Varicella-zoster virus (VZV) encephalitis is a rare but severe condition associated with significant morbidity and mortality. Its clinical presentation is often nonspecific and may overlap with other neurological conditions, including complications of influenza infection. We report a case of a 76-year-old man with confirmed influenza A infection who developed progressive neurological deterioration. Notably, no cutaneous manifestations suggestive of herpes zoster were observed. Cerebrospinal fluid analysis revealed lymphocytic pleocytosis, and polymerase chain reaction confirmed the presence of

Brief report: Effectiveness and safety of recombinant zoster vaccine in inflammatory bowel disease patients aged 18 years and older: A matched cohort and self-controlled case series study.

Individuals with inflammatory bowel disease (IBD) are at increased risk of herpes zoster (HZ). Using a matched cohort design, we evaluated the vaccine effectiveness (VE) of recombinant zoster vaccine (RZV) against HZ among IBD patients aged ≥18 years. Within the Kaiser Permanente Southern California population, individuals who received 2 RZV doses were matched up to 1:3 to RZV-unvaccinated individuals. Individuals were followed from 31 days after the receipt of the 2nd RZV dose until the occurrence of a censoring event. VE was calculated using Cox proportional hazards regression. A self-contro

[Ultrasound diagnosis of fetal infections: current possibilities and limitations].

INTRODUCTION: Fetal infections, particularly those caused by TORCH agents (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes), may significantly affect fetal growth and development, resulting in a substantial risk of morbidity and mortality. Since infections are mostly preventable causes of congenital anomalies, early diagnosis relies on serological and molecular testing and fetal ultrasound. OBJECTIVE: The aim of this study was to assess the prevalence of fetal infections and their detectable ultrasound features. METHOD: This retrospective s

[TORCH infections during pregnancy: clinical significance, diagnosis, and treatment].

TORCH infections (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes) constitute the classic group of congenital infections that may result in severe fetal and neonatal consequences. Transplacentally transmitted infections have been a critical component of prenatal care for decades. Although screening for this group of infections has been performed in Hungary since 1971, diagnostic approaches and screening strategies have undergone significant transformation. Current international recommendations, as well as national professional guidelines, no

Breast Cancer Screening Patterns Following National Crises: Experience from a Lebanese Tertiary Center.

BACKGROUND: Lebanon's recent compounded crises-including economic collapse, the COVID-19 pandemic, and the Beirut Port explosion-have disrupted healthcare access, including breast cancer (BC) screening. This study assesses adherence to BC screening after the onset of these crises and evaluates associated imaging outcomes. METHODS: This retrospective, single-center study included women aged ≥40 who underwent screening mammography (± ultrasound) at Lebanese Hospital Geitaoui (LHG) between October 2018 and October 2019, during the year that preceded the crisis. Exclusions included known BC, symp

TLR9 and STING agonists adjuvanted varicella-zoster virus glycoprotein E induced a potent and durable nasal vaccine response.

Herpes zoster is caused by the reactivation of latent varicella-zoster virus (VZV), primarily affecting middle-aged and older individuals and often accompanied by severe pain and other complications. Vaccination is an effective preventive measure against this disease. In the present study, we evaluated the adjuvant activity of Toll-like receptor (TLR)9 and TLR7 agonists in combination with stimulator of interferon genes (STING) agonists for a VZV-glycoprotein E (gE)-based vaccine. Among the tested combinations, the CpG-2722 (TLR9 agonist) and 2'3'-c-di-AM(PS)2 (STING agonist) pairing exhibited

The Emerging Threat of Monkeypox: An Updated Overview.

Monkeypox (MPOX) is an emerging zoonotic disease caused by monkeypox virus (MPXV), an orthopoxvirus closely related to smallpox. Initially confined to endemic regions in Central and West Africa, MPOX has recently gained global significance with outbreaks reported across multiple continents. MPXV is maintained in animal reservoirs but is increasingly transmitted from person to person, facilitated by close contact, respiratory droplets, and, in some cases, sexual transmission. Clinically, MPOX presents with fever, lymphadenopathy, and a characteristic vesiculopustular rash, though atypical manif

Secondary Deinococcus proteolyticus Infection Following Varicella-Zoster Virus Infection in a Child: A Case Report Describing a Rare Medical Condition.

Deinococcus proteolyticus belongs to Deinococcaceae, a family of bacteria characterized by an exceptional ability to withstand the lethal effects of DNA-damaging agents, including ionizing radiation, ultraviolet (UV) light, and desiccation. It was isolated from the Sahara surface sands, an extreme and nutrient-poor environment, regularly exposed to intense UV radiation, cycles of extreme temperatures, and desiccation. D. proteolyticus infection has seldom been implicated in human infections. The child presented with multiple necrotic skin lesions following a varicella infection, later complica

Herpes Zoster of the Larynx: A Diagnostic and Therapeutic Challenge.

Herpes zoster of the larynx is an exceptionally rare condition that may mimic more common causes of laryngitis. Reactivation of varicella-zoster virus in the vagus nerve can lead to odynophagia, dysphonia, and dysphagia, often without the typical cutaneous rash. Early recognition is essential, as delayed diagnosis may result in serious complications, including postherpetic neuralgia, persistent dysphagia, chronic voice impairment, and even laryngeal paralysis. We describe a 66-year-old woman on long-term low-dose corticosteroid therapy who presented with acute odynophagia, dysphagia, dysphonia

Primary Varicella-Zoster Virus Infection Complicating the Third Trimester of Pregnancy.

Varicella-zoster virus, colloquially known as chickenpox in the United States, complicating pregnancy has become a rare diagnosis following the introduction of the varicella-zoster vaccine. Despite a lower incidence of chickenpox than in the past, those providing prenatal care should keep a high index of suspicion for varicella-zoster among patients presenting with a vesicular rash and no history of having received the varicella-zoster vaccine. This report reviews the case of a 28-year-old gravida three at 35 weeks gestation who presented with concern for a full-body vesicular rash and pruritu

Disseminated Herpes Zoster Leading to Orbital Apex Syndrome: A Case of MRI-Negative Cranial Neuropathy.

Varicella-zoster virus (VZV) establishes latency in neuronal ganglia following primary infection and can later reactivate as herpes zoster. Disseminated zoster may occur in some cases and is characterized by vesicular lesions appearing beyond the initially affected dermatome. When the ophthalmic branch of the trigeminal nerve is involved, herpes zoster ophthalmicus can develop and may lead to a range of ocular complications. Neurological sequelae are less common, and orbital apex syndrome represents a rare but potentially severe manifestation. This syndrome involves dysfunction of the optic ne

Virus-induced onychomadesis: Exploring the role of viral infection in nail shedding.

Onychomadesis, characterized by proximal detachment of the nail plate due to temporary arrest of matrix proliferation, has been increasingly recognized as a complication following viral infections. Enterovirus-associated hand-foot-and-mouth disease (HFMD) is the most frequently reported cause. Recent studies demonstrate that some enteroviruses, including Coxsackievirus A10, utilize the host receptor KREMEN1 (KRM1) to impair Wnt/β-catenin signaling and suppress nail stem cell differentiation, thereby providing a molecular basis for infection-induced nail shedding. Additionally, cases of onychom

Disseminated Cutaneous Herpes Zoster in an Immunocompetent Patient: A Diagnostic Challenge.

Disseminated cutaneous herpes zoster (DCHZ) typically occurs in immunocompromised individuals. However, rare cases arise in immunocompetent adults. These cases may initially resemble benign dermatoses or be difficult to distinguish from other systemic rashes when overlapping risk factors obscure diagnosis. Early recognition is essential to prevent visceral or neurologic complications.  A 32-year-old immunocompetent male with childhood varicella presented with a 10-day history of a pruritic vesicular rash, right ear fullness, and viral prodromal symptoms. He had recent diving exposure five days

Rapidly progressive varicella zoster virus vasculopathy in a chemotherapy- and steroid-immunosuppressed patient with refractory diffuse large B-cell lymphoma: diagnostic and therapeutic challenges.

Varicella zoster virus (VZV) vasculopathy is a rare but severe neurological complication that frequently results in a reversible cerebral vasoconstriction syndrome (RCVS). Diagnosis is often delayed because early manifestations are nonspecific and radiologic findings often overlap with those of other cerebrovascular disorders. We describe the fatal case of a heavily pretreated 60-year-old man with refractory diffuse large B-cell lymphoma undergoing evaluation for chimeric antigen receptor (CAR) T-cell therapy. The patient developed multifocal intracranial arterial stenosis and an acute infarct

Immunogenicity and safety of an investigational varicella zoster vaccine in toddlers aged 12-15 months: results of a phase II, controlled, randomized trial.

Varicella is an acute, highly contagious infectious disease that primarily impacts young children with approximately 90% of cases occurring under the age of 15. Due to the high incidence of varicella infection, the potential for serious complications, and the significant economic and public health burden, vaccination of toddlers against varicella is included in routine vaccination programs in many countries. This study assessed the immunogenicity and safety of the investigational varicella vaccine (VNS) compared to a licensed, current standard-of-care vaccine. In this phase II, observer-blind,
