Herpes zoster, also known as shingles, is caused by the reactivation of the Varicella-Zoster Virus (VZV), which remains dormant in the cranial sensory nerve ganglia and spinal dorsal root ganglia after the initial VZV infection is cured in a patient. When it reactivates, it travels down the nerve to the skin, causing a painful rash and blisters. The pathophysiology involves viral replication in nerve cells, inflammation, and subsequent skin lesions. Herpes zoster can have a significant impact on an individual's physical, psychological, and social well-being. While the condition is usually not

Varicella-zoster virus (VZV) establishes lifelong latency following primary infection and can reactivate later in life to cause herpes zoster (HZ) and its debilitating complication postherpetic neuralgia (PHN). The implementation of varicella vaccinations has profoundly altered the epidemiological landscape of VZV. This narrative review examines the current literature to evaluate the impact of these vaccination programs on the incidence of HZ and PHN. Early mathematical models based on the "exogenous boosting" hypothesis predicted a substantial surge in adult HZ due to reduced exposure to circ

Herpes zoster (HZ), caused by the reactivation of varicella zoster virus, presents a major diagnostic challenge in dentistry, particularly when involving the trigeminal nerve. The prodromal symptoms of HZ can closely resemble endodontic pain, leading to misdiagnosis and delayed appropriate treatment. This case report described a 54-year-old male who presented to the emergency dental clinic with severe unilateral, radiating maxillary pain originating from posterior to anterior teeth. Initially diagnosed with symptomatic apical periodontitis on a previously endodontically treated tooth #3, the p

Varicella is usually a mild childhood disease; however, primary infection in adults may lead to severe complications, particularly varicella pneumonia. We report the case of a 32-year-old previously healthy male admitted to the intensive care unit with acute hypoxemic respiratory failure occurring three days after the onset of a diffuse vesicular rash. Chest CT demonstrated bilateral ground-glass opacities with focal consolidation consistent with viral pneumonia. The diagnosis was established based on typical cutaneous findings, epidemiological exposure, and compatible imaging results. Early t

Varicella-zoster virus (VZV) is well-known for causing primary chickenpox infection; however, it remains dormant in the central nervous system and can be reactivated during periods of decreased cell-mediated immunity. In addition to shingles, which is the most common form of reactivation, other rare secondary complications have been observed, including meningitis and encephalitis. Immunocompromised patients, such as those with acquired immunodeficiency syndrome (AIDS), are at an increased risk of such complications. We present the case of a 58-year-old male with AIDS (CD4 111 cells/mm3) who de

Herpes zoster ophthalmicus results from the reactivation of latent varicella zoster virus involving the ophthalmic branch of the trigeminal nerve and may lead to severe ocular complications. Hutchinson's sign, defined by vesicular or crusted lesions on the nasal tip, is a key predictor of ocular involvement. We describe a 57-year-old woman with no relevant past medical history who presented with a 15-day history of pruritus and burning in the right eye. She was initially misdiagnosed with blepharitis and later periorbital cellulitis, receiving topical tobramycin and systemic antibiotics witho

We present the case of a seven-year-old fully immunized male who developed an intensely pruritic, generalized maculopapular rash that progressed to vesicles. The patient's rash was initially diagnosed by his primary care provider as possible scabies and treated with topical permethrin. Our examination revealed a diffuse maculopapular and vesicular exanthem with lesions at various stages of evolution, eventually progressing to secondary cellulitis. The patient's presentation, consistent with varicella (chickenpox), highlights the diagnostic challenges of breakthrough varicella infection and pot

Breakthrough varicella (BTV), defined as varicella occurring more than 42 days after vaccination, is typically milder than primary varicella infection. However, its clinical course in immunocompromised children remains incompletely characterized. We report two pediatric patients undergoing chemotherapy for acute leukemia who developed BTV despite prior two-dose varicella vaccination. Both patients initially presented with atypical and sparse skin eruptions, which delayed diagnosis. During chemotherapy of profound immunosuppression, the rashes rapidly progressed, accompanied by fever and neutro

Disseminated cutaneous herpes zoster (DCHZ), defined as lesions involving three or more dermatomes, is strongly associated with immunocompromised states and remains exceptionally rare in young immunocompetent adults. We present a case of DCHZ in a previously healthy 25-year-old male from Guatemala who presented with an eight-day history of a painful vesicular rash involving the right V2/V3, C3-C5, and T2-T3 dermatomes. Comprehensive evaluation for occult immunocompromise, including HIV testing, complete blood count, hemoglobin A1c, and urinalysis, was unrevealing. Treatment with high-dose intr

Ramsay Hunt Syndrome (RHS) arises from reactivation of varicella-zoster virus (VZV) in the geniculate ganglion, clinically manifesting as facial palsy and an auricular rash. Involvement of cranial nerves (CN) beyond the facial nerve (CN VII) occurs in up to half of cases, complicating the clinical presentation. Current knowledge regarding polycranial involvement in RHS remains fragmented, largely drawn from isolated case reports and small series. This review synthesizes available evidence on the epidemiology, clinical spectrum, differential diagnosis, and mechanisms of cranial nerve involvemen

Background and objective Herpes zoster infection is due to reactivation of the varicella zoster virus, usually at the dorsal root ganglia, leading to classic skin lesions and neuralgic pain. Concurrent involvement of motor axons/cells may cause segmental paralysis. We sought to describe the clinical and electrodiagnostic (EDX) findings in a cohort of patients with zoster segmental paralysis (ZSP). Methods This is a retrospective study of 17 patients with historical and clinical evidence of ZSP of the upper extremities (UE) or lower extremities (LE), confirmed by EDX studies. The clinical metri

INTRODUCTION: Chemo-immunotherapy has reshaped the treatment of metastatic non-small cell lung cancer (NSCLC), yet data on varicella-zoster virus (VZV) reactivation under these regimens are limited. Major immunotherapy trials do not specifically describe herpes zoster events in patients receiving combined chemotherapy and immune checkpoint inhibitors.
METHODS: We describe a case of metastatic lung adenocarcinoma (ADC) treated with first‑line Pemetrexed-Carboplatin-Pembrolizumab plus Denosumab in which herpes zoster occurred during treatment, detailing clinical course, management, and outcomes

BACKGROUND: Periorbital necrotizing fasciitis (NF) is a severe form of soft-tissue infection characterized by necrosis of the subcutaneous tissue, muscle, and other deep soft tissue around the fascia in the periorbital region. Although rare, such a condition can occur following predisposing factors, such as injury from previous trauma, procedures, or infection. In this report, we highlight the role of preoperative computed tomography (CT) scan assessment in a patient with periorbital NF following herpes zoster ophthalmicus. Immunocompromised conditions, particularly human immunodeficiency viru

Varicella-Zoster Virus (VZV) causes primary varicella and can reactivate later in life to induce herpes zoster with severe complications, posing a persistent threat to human health. Previous studies of VZV immunogens have focused primarily on the major envelope glycoproteins (e.g., gE, gH, and gB), whereas other potential immunogenic components remain unexplored. To broaden the repertoire of anti-VZV targets, we established a biotin-avidin membrane-protein capture strategy to screen for surface-accessible viral antigens that elicit neutralizing antibodies. Using this approach, we identified th

Herpes zoster ophthalmicus (HZO) is a manifestation of varicella-zoster virus reactivation involving the ophthalmic division of the trigeminal nerve and carries a risk of significant ocular complications. Early antiviral therapy may alter the clinical course of infection and occasionally produce atypical or abortive presentations. This report describes the case of a 34-year-old primigravid woman at 33 weeks of gestation with primary antiphospholipid antibody syndrome on anticoagulation therapy who presented with acute severe burning pain over the right forehead and scalp followed by grouped er

Herpes simplex virus type 1 (HSV-1) is a common DNA virus that typically causes orolabial lesions. Atypical presentations, such as dermatomal vesicular eruptions, are rare and may mimic varicella-zoster virus (VZV) reactivation, complicating diagnosis. Here we present a case of HSV-1 with recurrent vesicular eruptions in the C4-C5 dermatomes, initially posited to be herpes zoster. Viral culture confirmed HSV-1, and the patient's symptoms were successfully managed with valacyclovir and topical corticosteroids.

The reactivation of the varicella zoster virus (VZV) is the cause of herpes zoster (HZ), usually referred to as shingles. Once an individual has been previously infected with chickenpox, the virus stays dormant in their nervous system and may reactivate later in life, leading to HZ. HZ poses a significant burden on the healthcare system due to its high prevalence. Older adults and individuals with compromised immune function are at higher risk of viral reactivation. Vaccination against HZ is a highly effective strategy in preventing viral recurrence and its associated complications. However, r

Hepatic duct perforation (HDP) is a rare and life-threatening condition in adults, typically caused by direct injury or increases in intraductal pressure. The risk of HDP following cardiac surgery is poorly documented, although predisposing factors, such as bilirubin overload and ischemia from cardiopulmonary bypass, share common pathophysiological mechanisms with postoperative cholecystitis. We report the case of an 80-year-old woman who underwent mitral valve replacement for infectious endocarditis. Preoperatively, she was asymptomatic with mild common hepatic duct dilatation but no stones o

Immunoglobulin A (IgA)-mediated vasculitis is a small-vessel vasculitis defined by IgA-dominant immune complex deposition. Although common in children, adult-onset cases are rare and may signal underlying malignancies. In this case report, we present a 60-year-old male with chronic lymphocytic leukemia (CLL) under observation for his painful purpura and non-healing leg ulcers, with a biopsy confirming IgA vasculitis. His course was further complicated by steroid-induced diabetes, poor wound healing, recurrent infections, and fatal respiratory failure. He remained steroid-dependent with recurre

Varicella-zoster virus (VZV), a neurotropic α-herpesvirus, establishes lifelong latency in the nervous system after primary infection. Reactivation leads to herpes zoster (HZ), which can result in severe neurologic complications including postherpetic neuralgia, vasculopathy, encephalitis, meningitis, myelopathy, cranial neuropathy, and optic neuritis. These complications may occur even in the absence of a characteristic rash, making early recognition crucial for timely antiviral therapy to maximize the likelihood of a good outcome.
