A case series of immune checkpoint inhibitor-induced bullous pemphigoid successfully treated with dupilumab and evidence for the BP180 midportion epitope as a preferential autoantigenic target.

Bullous pemphigoid (BP) is the most common autoimmune bullous disease and represents a recognized cutaneous immune-related adverse event associated with immune checkpoint inhibitors (ICIs). Management of ICI-induced BP is challenging, as conventional systemic corticosteroids may interfere with antitumor immunity, highlighting the need for effective steroid-sparing therapies. We report a case series of four elderly male patients who developed BP during treatment with ICIs and were successfully managed with dupilumab, an interleukin-4 receptor alpha antagonist. The median age was 72 years, and B

Comparative Clinical and Histological Analysis of Bullous Pemphigoid: Immunotherapy-Associated versus Spontaneous Onset Cases.

INTRODUCTION: Immune checkpoint blockade (ICB) therapy can be associated with the development of bullous pemphigoid (BP). To characterize clinical, histological, and serological characteristics, we compared immune-related bullous pemphigoid (irBP) and spontaneous bullous pemphigoid (sBP) considering clinical, serological, and histopathological character. METHODS: 149 BP patients treated at the University Hospital Zurich between 2015 and 2024 were identified retrospectively. Demographic and serological data were obtained from electronic records. Hematoxylin-eosin-stained biopsies were semi-qua

Comorbidities of atopic dermatitis: Emerging evidence and clinical considerations.

Atopic dermatitis (AD) is the most common chronic inflammatory skin disease and has traditionally been viewed as a disorder confined to the skin. However, emerging epidemiologic, genetic, and mechanistic evidence increasingly supports the concept that AD represents a systemic disease associated with a broad spectrum of immune-related comorbidities. This review synthesizes current literature on the comorbidities associated with AD, including dermatologic, respiratory, gastrointestinal, rheumatologic, neurologic, ophthalmologic, and endocrine disorders. Well-established associations include othe

Bullous pemphigoid following adjuvant radiotherapy for breast cancer: a case report.

BACKGROUND: Bullous pemphigoid (BP) is an autoimmune cutaneous blistering disease that predominantly affects elderly patients. Radiotherapy represents a rare triggering factor and may clinically mimic radiation-induced dermatitis, potentially leading to delayed diagnosis. CASE REPORT: An 85-year-old woman underwent breast-conserving surgery and adjuvant radiotherapy for multifocal invasive lobular carcinoma of the right breast. The immediate post-radiotherapy course was unremarkable. Approximately 7 weeks after completion of radiotherapy, the patient developed acute-onset pruritic blistering

Noninvasive Imaging Techniques as Modern Diagnostic Tools in Desquamative Gingivitis: Focus on RCM, OCT, and LC-OCT.

BACKGROUND: Desquamative gingivitis is a clinical condition characterized by erythema, blisters, and erosions within the gingiva. It may be a manifestation of oral lichen planus or autoimmune bullous diseases, such as pemphigus vulgaris and mucous membrane pemphigoid. The identification of the cause of desquamative gingivitis typically involves the analysis of clinical manifestations, histopathology, and immunopathology evaluation. This article reviews available data on the application of noninvasive imaging techniques (reflectance confocal microscopy, optical coherence tomography, and line-fi

Influenza Vaccine as a Potential Trigger for Urticarial Phase Bullous Pemphigoid: A Case Report.

INTRODUCTION: Bullous pemphigoid (BP) is a chronic autoimmune blistering disorder that primarily affects older adults. While typically associated with tense bullae, its urticarial phase presents diagnostic challenges due to its nonspecific features, often mimicking other pruritic dermatoses. CASE PRESENTATION: This report presents a unique case of urticarial phase BP triggered by influenza vaccination in a 58-year-old male, with a clinical course marked by widespread intensely pruritic annular erythematous plaques. Notably, no vesicles, bullae, or erosions were observed during the early phase

A clinically actionable stratification of bullous pemphigoid using a disease activity score reveals a targetable inflammatory endotype.

BACKGROUND: Bullous pemphigoid (BP) exhibits significant clinical heterogeneity. The molecular drivers underlying these distinct phenotypes remain poorly understood, hindering the development of precision medicine approaches. OBJECTIVES: To correlate clinical phenotypes, defined by the Bullous Pemphigoid Disease Area Index (BPDAI), with molecular endotypes using serum proteomic profiling. METHODS: This cohort study enrolled 143 patients with BP hospitalized at a tertiary centre between January 2024 and June 2025. Patients were stratified as having inflammatory BP (I-BP; BPDAI urticaria/eryth

A neuroimmune axis linking S100A8/9 to itch sensitization in both bullous pemphigoid and atopic dermatitis.

Bullous pemphigoid and atopic dermatitis are both inflammatory skin disorders characterized by intense pruritus and type 2 immune responses. Previous studies identified S100A8 and S100A9 as highly upregulated genes in perilesional skin of both conditions. Here, we demonstrate that S100A8/9 is highly expressed in the keratinocytes and infiltrating immune cells in bullous pemphigoid and atopic dermatitis skin and elevated in the serum of patients with bullous pemphigoid. Notably, infiltrating neutrophils in the perilesional skin, one of the major sources of secreted S100A8/9, are in close proxim

Mucous membrane pemphigoid with oral involvement: Two cases reports.

These cases present a fifty-four- and sixty-four-year-old females, one of whom had the second episode of the disease. Clinical and cytological examinations of the oral lesions were performed, and the Oral Health Impact Profile-14 (OHIP-14) questionnaire was administered. Additionally, histopathological examination and direct immunofluorescence microscopy were conducted on the patient's biopsied lesions. This article presents two case reports of patients with mucous membrane pemphigoid involving the oral cavity. As an autoimmune bullous disease, mucous membrane pemphigoid is often characterized

Treg cells as potential therapeutic targets for bullous pemphigoid: A review.

Bullous pemphigoid (BP) is an autoimmune disease characterized by Bullous pemphigoid antigen 180(BP180) and Bullous pemphigoid antigen 230(BP230) autoantibodies production. Regulatory T cells (Tregs) are key cells for the maintenance of immune tolerance. Increasing evidence suggests that Treg dysfunction leads to autoantibody production and skin rashes in mice and humans, making immunomodulatory therapies targeting Tregs a potential therapeutic strategy for BP. Low-dose Interleukin-2 treatment and adoptive cell therapy are promising methods to modulate Tregs' numbers and function. Herein, we r

Expert consensus on oral management in autoimmune bullous diseases, erythema multiforme and SJS/TEN.

BACKGROUND: Autoimmune bullous diseases (AIBD), Stevens-Johnson syndrome and toxic epidermal necrolysis (SJS-TEN) and erythema multiforme (EM) often present with clinically similar features in the oral mucosa: classically widespread mucosal erosions or ulceration. Despite this, there are no formalized guidelines for the specific management of the oral manifestations of these conditions. OBJECTIVES: We sought to establish an consensus on the management of oral involvement of AIBD, SJS-TEN and EM using the Delphi method. METHODS: Participants were sent a survey comprising 62 statements organiz

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) in dermatology: cutaneous adverse events and emerging efficacy in inflammatory skin diseases.

INTRODUCTION: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) and next-generation incretin therapies are increasingly used for diabetes and obesity, yet cutaneous adverse events remain incompletely characterized. New-onset dermatological manifestations after GLP-1RA initiation such as rash or alopecia may be misattributed to baseline inflammatory skin disease rather than drug-induced toxicity. AREAS COVERED: Searches of PubMed, Embase and Cochrane from inception to 4 October 2025 yielded 3,006 records; 80 primary studies were included. Small studies suggest improvement in psoriasis and h

Stoma-Related Bullous Pemphigoid: Case Report and Literature Review.

Peristomal skin complications are common and occur in most stoma recipients. Typically, these complications present as contact dermatitis due to the adhesive or moisture associated with the leakage of the pouching system. However, they can also present as atypical peristomal skin complications presenting with vesicular eruptions that are triggered by surgical trauma of stoma creation. The authors present a case of bullous pemphigoid in a 73-year-old patient, initially localized in the peristomal area, which subsequently progressed to a generalized eruption affecting the trunk and extremities.

Oral Microbial Dysbiosis and Pathogenic Functional Shifts in Mucosa-Dominant Pemphigus Vulgaris and Low-Risk Mucous Membrane Pemphigoid.

BACKGROUND: Mucosal-dominant pemphigus vulgaris (MD-PV) and low-risk mucous membrane pemphigoid (LR-MMP) are autoimmune bullous diseases primarily affecting the oral mucosa, yet their oral microbiome profiles remain inadequately characterized. METHODS: Using 16S rRNA sequencing of saliva from 21 MD-PV patients, 26 LR-MMP patients, and 14 healthy controls (HC), we analyzed microbial diversity, differential taxa (LEfSe), and functional potential (PICRUSt2/BugBase). RESULTS: Both patient groups showed significant microbial restructuring without major richness changes, featuring increased Firmic

HCA2 Receptors in Lymphocytes and Keratinocytes Affect Murine Contact Allergic Inflammation.

The hydroxycarboxylic acid receptor HCA2 is expressed in keratinocytes and immune cells. In mice, the anti-inflammatory potential of HCA2 receptor signalling in the skin was first described in experimental models of psoriasis-like inflammation and bullous pemphigoid-like epidermolysis bullosa acquisita. We examined contact allergic immune responses to the obligate contact sensitiser DNFB in Hcar2-/- and wild-type C57BL/6 mice. Basal mRNA levels of pro-inflammatory mediators like IFNγ were already increased in the ear tissue of naïve Hcar2-/- animals. After sensitisation and challenge with DNFB

Bullous Diseases in the Perioperative Setting: Anesthetic Considerations.

Bullous skin disorders, including epidermolysis bullosa, pemphigus, and bullous pemphigoid, pose distinctive anesthetic challenges due to extreme cutaneous fragility, mucosal involvement, fluid imbalances, an increased risk of infection, and multisystem comorbidities. Although these patients frequently require surgical or procedural care, evidence guiding perioperative management is limited to isolated case reports and small series. This systematic review aimed to synthesize contemporary data on anesthetic and perioperative strategies for patients with autoimmune and inherited bullous diseases

CD19×CD3 bispecific T cell engager treatment induces remission in experimental pemphigoid disease.

Pemphigoid diseases (PDs) are autoimmune disorders marked by autoantibodies (Aabs) against skin and mucous membrane proteins, causing mucocutaneous blistering in predominantly elderly patients. Since current therapeutics are often insufficient, PDs impose a significant morbidity and mortality burden. CD19×CD3 bispecific T cell engagers (TCEs)-originally developed for B cell malignancies-have shown promise in treatment-refractory autoimmune diseases such as systemic sclerosis and rheumatoid arthritis. To explore their potential in PDs, we tested a murine CD19×CD3 TCE in a mouse model of epiderm
