Drug-Induced Blaschkitis Associated With Dipeptidyl Peptidase-4 Inhibitor: A Case Report and Review of the Literature.

Blaschkitis is an acquired Blaschko-linear inflammatory dermatosis similar to classic lichen striatus. Although the pathomechanisms have not yet been fully elucidated, it has been reported that some cases were triggered by viral infections, vaccines, or drugs. We recently encountered a patient who developed multiple broad bands of erythematous papular plaques accompanied by vesicles along Blaschko lines on the left side of the trunk while taking sitagliptin, a dipeptidyl peptidase-4 inhibitor (DPP-4i) for type 2 diabetes mellitus. Histopathology showed necrotic keratinocytes in the epidermis a

Dermatologic outcomes associated with glucagon-like peptide-1 receptor agonists in patients with type 2 diabetes: a large-scale target trial emulation.

BACKGROUND: While dipeptidyl peptidase-4 inhibitors (DPP-4is) are linked to autoimmune skin diseases, comparative data on glucagon-like peptide-1 receptor agonists (GLP-1 RAs) remain limited. METHODS: We emulated a randomized active-comparator trial using the TriNetX US Collaborative Network. Adults with type 2 diabetes (T2DM) initiating GLP-1 RAs or DPP-4is between January 1, 2018, and December 31, 2022, were identified, excluding prior users of either class. Balanced cohorts (n=169,630 each) were created using 1:1 propensity score matching. To address protopathic bias, outcomes occurring wi

Case Report: Lung transplant recipient with bullous pemphigoid and antibody-mediated rejection: an overlapping immunologic phenomenon?

We present what is, to our knowledge, the first reported case of concurrent bullous pemphigoid (BP) and antibody-mediated rejection (AMR) in a lung transplant patient. Similar associations have been described in other solid organ transplant settings, most notably in renal transplantation, where explantation of the allograft has frequently been followed by resolution of the skin disease. These observations suggest a broader potential connection between autoimmune blistering disorders and allograft rejection mechanisms, especially as emerging evidence highlights the role of non-HLA antibodies. Diagnostic challenge: Epidermolysis bullosa acquisita of the palms and soles.

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disorder that can be difficult to diagnose, especially when its presentation mimics other blistering conditions. We present the case of a 56-year-old male who developed widespread blistering on flexural surfaces, palms, soles, and target lesions, with clinical features resembling bullous pemphigoid (BP), erythema multiforme (EM), and infectious aetiologies. A skin biopsy showed subepidermal bullae with linear IgG and C3 deposition at the basement membrane, with additional focal IgM and IgA positivity, and ELISA testing confi

Regulatory T cells in bullous pemphigoid: biological characteristics, dysfunction, and pathogenic roles.

Bullous pemphigoid (BP) is an autoimmune blistering disorder that predominantly affects the elderly. Its pathogenesis involves the disruption of the basement membrane zone (BMZ), driven by pathogenic autoantibodies and a pronounced inflammatory response. Current first-line therapies, primarily based on glucocorticoids and immunosuppressants, are limited by substantial side effects and frequent disease recurrence. Regulatory T cells (Tregs) play a central role in maintaining immune homeostasis and peripheral tolerance, exerting control over effector immune cells through transcriptional regulati

Corticosteroid-Sparing Control of Bullous Pemphigoid with Dupilumab and Tripterygium Glycosides: A Real-World Cohort with Longitudinal Transcriptomics.

BACKGROUND: Bullous pemphigoid (BP) often requires long-term corticosteroids with substantial adverse effects. Effective corticosteroid-sparing treatments could improve safety and quality of life for patients with BP. OBJECTIVE: To evaluate the clinical efficacy, safety, and transcriptomic correlates of dupilumab combined with tripterygium glycoside (TG) in moderate-to-severe BP. MATERIALS AND METHODS: We conducted a clinical cohort study at Peking Union Medical College Hospital. Twelve consecutive BP patients (bullous pemphigoid disease area index [BPDAI] ≥20) who had received dupilumab com

The Secretome of Bullous Pemphigoid IgG-Treated Keratinocytes Induces a Pro-Inflammatory Eosinophil Response.

Bullous pemphigoid (BP) is an autoimmune blistering disease whereby the cutaneous antigens BP180 and BP230 are targeted by autoantibodies. Skin lesions in BP are characterized by an abundance of eosinophils. We recently demonstrated that the treatment of keratinocytes with antibodies from patients with BP induces a robust inflammatory response with release of numerous cytokines, chemokines, complement factors, and proteases. We thus questioned whether this keratinocyte inflammatory response was capable of directly inducing an inflammatory response in eosinophils. We therefore treated human eos

Stapokibart's clinical effectiveness in treating moderate-to-severe bullous pemphigoid in the elderly.

OBJECTIVE: Bullous pemphigoid (BP) is the most common autoimmune blistering disease in the elderly, for whom conventional systemic corticosteroids and immunosuppressants pose significant safety risks due to age-related comorbidities, leading to infections, osteoporosis, and metabolic disorders. While stapokibart, a novel IL-4Rα inhibitor, holds promise for type 2 inflammatory diseases, clinical evidence regarding its efficacy in BP is currently lacking. Therefore, this study aims to observe the clinical efficacy and long-term safety of stapokibart in the treatment of moderate-to-severe BP in t

Optimizing Local Treatment in Pemphigus and Pemphigoid: Current Evidence and Unmet Needs.

Autoimmune blistering diseases (AIBDs), including pemphigus vulgaris (PV) and bullous pemphigoid (BP), are characterized by painful erosions and chronic wounds that significantly impair patient quality of life. While systemic therapies remain the cornerstone of treatment, optimal strategies for local management, including wound care and topical therapies, are not well defined. The aim of this study is to review and synthesize current evidence on topical treatments and wound care strategies in AIBDs, with a focus on their role in disease control, infection prevention, and enhancement of healing

Rare Presentation of TEN-Like Subacute Cutaneous Lupus Erythematosus Occurring in the Setting of Donepezil Exposure and COVID-19 Infection: A Diagnostic Challenge.

Toxic epidermal necrolysis (TEN) is a life-threatening mucocutaneous reaction characterized by rapidly progressive epidermal necrosis within hours to days, extensive skin detachment, and high mortality. Rarely, patients present with TEN-like eruptions that evolve slowly, over days to weeks, and have skin changes mostly confined to photodistributed areas, with possible mucosal involvement. The latter scenario is compatible with TEN-like lupus erythematosus. We report a 65-year-old man without prior lupus history who developed a widespread, photodistributed erythematous patchy eruption within 24

The Changing Trends in the Incidence and Clinical Presentation of Immunobullous Disorders Before and During the COVID-19 Era in a Tertiary Healthcare Centre in Andhra Pradesh.

Background Autoimmune immunobullous disorders (AIBDs) represent a heterogeneous group of chronic blistering dermatoses mediated by autoantibodies against desmosomal and hemidesmosomal adhesion molecules. The COVID-19 pandemic has generated significant interest in the interaction between SARS-CoV-2 infection, vaccination-related immune modulation, and the clinical behaviour of autoimmune diseases. Objective This study's primary objective is to compare hospital-based temporal trends in the frequency, subtype distribution, and clinical presentation of AIBDs before and during the COVID-19 era in a
