Demographic characteristics and comorbidities in patients with oral mucous membrane pemphigoid and pemphigus.

OBJECTIVES: Oral autoimmune blistering diseases (AIBDs) are a group of rare, chronic disorders causing different symptoms on the oral mucosa. The most common oral AIBDs are mucous membrane pemphigoid (MMP) and pemphigus vulgaris (PV). The aim of this study was to describe the demographic and clinical characteristics, as well as comorbidities, of patients with oral AIBDs in Southern and Western Finland. MATERIALS AND METHODS: This retrospective two-center study included patients diagnosed with AIBD with oral symptoms treated at Helsinki University Hospital and Turku University Hospital between

[Severe cutaneous adverse reactions].

Severe cutaneous adverse reactions are rare, potentially life-threatening T-cell-mediated drug reactions. They present with rapid onset skin lesions and may involve the mucosa and multiple organs. This review finds that early identification, drug withdrawal, supportive care, and immunomodulatory therapy are essential. JAK inhibitors have shown promise in Stevens-Johnson's syndrome/toxic epidermal necrolysis, IL-5 blockade is being tested for drug reaction with eosinophilia and systemic symptoms; and IL-36 inhibition, may be a future option for acute generalised exanthematous pustulosis.

Research Progress on Targeted Therapies for Pemphigus Vulgaris and Bullous Pemphigoid: Mechanisms, Opportunities and Challenges.

Autoimmune bullous diseases (AIBDs) are a group of severe disorders characterized by autoantibodies that attack structural proteins within the skin/mucosa, leading to blister and bullae formation. Pemphigus vulgaris (PV) and bullous pemphigoid (BP) are among the most prevalent representative AIBDs. Traditional treatments involving corticosteroids and immunosuppressive agents are often associated with substantial adverse effects and a high rate of relapse. In recent years, the therapeutic landscape has evolved toward precision targeted therapies, encompassing biological agents, small-molecule d

Erythema Multiforme after Liver Transplant: A Rare Pediatric Case Report.

This case report presents a 3-year-old female patient with erythematous lesions in the mouth and lips. She initially showed symptoms such as abdominal pain, vomiting, pale stools, and dark urine. Imaging revealed a large hepatic mass, diagnosed as alveolar rhabdomyosarcoma via biopsy. Chemotherapy began while metastasis was under evaluation. After 3 cycles of chemotherapy, she underwent tumor resection and liver transplant, followed by continued chemotherapy. Oral lesions that shifted from yellow-white to purple-brown appeared during treatment and were preliminarily diagnosed as erythema multi

A Mechanistic Map of the Phenotypic Complexity in Recessive Dystrophic Epidermolysis Bullosa Provides Basis for Therapeutic Target Discovery and Drug Repurposing.

Recessive dystrophic epidermolysis bullosa (RDEB) is a genodermatosis caused by loss-of-function mutations in the COL7A1 gene, producing a complex and heterogeneous phenotype. Clinical manifestations include skin fragility, local and systemic inflammation, impaired wound healing, dysbiosis, pain, and pruritus, increasing the risk of skin cancer. This work aimed to develop an actionable model of RDEB's molecular mechanisms to identify and prioritize therapeutic targets for drug repurposing. Transcriptomic data of skin biopsies from pediatric RDEB patients and unaffected individuals were used to

Anesthetic Blister Formation for Biopsy-Site Identification Prior to Mohs Micrographic Surgery.

Confirming biopsy sites before Mohs micrographic surgery can be difficult when wounds have healed clinically and documentation is limited, risking wrong-site surgery. We describe a simple intraoperative adjunctive technique: a focal blister that may form at a recent biopsy site during slow intradermal local anesthetic infiltration. This finding may reflect transient immaturity of anchoring fibrils during early wound remodeling and appears to occur most often within the first few weeks after biopsy. Although it requires no extra equipment and can help confirm a carefully chosen presumptive site

Development and Content Validation of the Epidermolysis Bullosa Oral Health Assessment Form (EB-OHAF).

OBJECTIVES: Epidermolysis Bullosa (EB) is a rare skin disorder with significant phenotypic variations according to the primary type and subtype of the condition. As a rare disease with uncommon oral features and high phenotypic variability, identifying all characteristics is important to define a patient-specific treatment strategy and monitor the impact of different strategies on the oral outcomes. This project aims to develop and validate an EB-specific oral health assessment tool that helps clinicians to identify every specific oral feature that can be presented in patients with different t

Case Report: dupilumab for severe pruritus in pediatric recessive dystrophic epidermolysis bullosa-a longitudinal clinical assessment and literature review.

Recessive dystrophic epidermolysis bullosa (RDEB) results from biallelic COL7A1 variants and causes trauma-induced blistering, scarring, and progressive extracutaneous complications. Severe pruritus may intensify skin injury through an itch-scratch cycle. We report a 7-year-old, 25-kg girl with infantile-onset generalized blistering, severe pruritus, mucosal and nail involvement, scarring, and digital contractures. Histopathology showed a subepidermal blister. Sequencing identified heterozygous COL7A1 c.5605G>C (p.Gly1869Arg) and c.5532+5G>A, inherited from different parents and therefore occu

Upadacitinib for the treatment of refractory bullous pemphigoid: a case report and literature review.

Bullous pemphigoid (BP) is a common autoimmune subepidermal blistering disorder that mainly affects elderly individuals aged ≥70 years. Systemic glucocorticoids serve as first-line treatment, and immunosuppressants or biological agents such as dupilumab are added for patients with suboptimal responses. However, a proportion of patients exhibit suboptimal responses or cannot sustain therapeutic benefits under these standard therapeutic regimens, representing an important unmet clinical need. We describe a 74-year-old patient with refractory BP who did not achieve adequate disease control despit

GLP-1R expression and dermatological diseases: a mendelian randomization and real-world study.

BACKGROUND: Although glucagon-like peptide-1 receptor (GLP-1R) is widely expressed in multiple tissues and organs including skin and subcutaneous tissue, with diverse physiological roles in metabolism and immunity, the potential causal association between GLP-1R expression and dermatological diseases remains unclear. METHODS: Available cis-expression quantitative trait loci (cis-eQTLs) were selected as genetic instruments for GLP-1R expression. A two-sample Mendelian randomization (MR) analysis was employed to assess the potential causal association between GLP-1R expression and dermatologica

Case Report: Nicotinamide monotherapy for infantile bullous pemphigoid with long-term follow-up.

Infantile bullous pemphigoid (IBP) is a rare autoimmune bullous skin disease that occurs in infants, for which standardized targeted therapeutic regimens remain unavailable. Herein, we report two infants with IBP treated with nicotinamide monotherapy. Complete clinical remission was observed within 3 weeks and 10 days after treatment initiation, respectively. Following clinical improvement, nicotinamide was gradually tapered and discontinued in approximately 2 months. No recurrence was observed during long-term follow-up. However, given the limited number of cases, these observations do not es

Epidermolysis bullosa dystrophica in a goat kid.

In a dairy herd of Thuringian Forest goats, a male goat kid of around three weeks of age was detected with skin lesions, particularly on the distal limbs, chest and ears. These skin lesions were characterized by alopecia, pus-filled blisters, incrustations and erosions. The goat kid was referred to the clinic for treatment but had to be euthanized despite intensive treatment due to increasing deterioration in its general condition and was subsequently referred to necropsy. Histologic investigation of the altered areas of skin and mucosa revealed a separation of epithelium from the underlying d

Short-term disease control with alternate-day maintenance upadacitinib in refractory Hailey-Hailey disease: a case report.

Hailey-Hailey disease (HHD), also known as benign familial pemphigus, is a rare autosomal dominant genodermatosis characterized by recurrent erosive lesions in intertriginous areas. Previous case reports have described clinical improvement with upadacitinib, but post-response maintenance strategies remain poorly defined. We report a 62-year-old woman with a 7-year history of refractory HHD who showed marked improvement after 2 months of upadacitinib 15 mg once daily. After 6 months of daily treatment, the lesions had completely cleared and disease control was stable. The regimen was then chang

Clinical improvement of pemphigus-like mucocutaneous disease in a patient with CLL following initiation of zanubrutinib: a case report.

Pemphigus is an autoimmune bullous disease. PNP/PAMS represents a rare subtype of autoimmune bullous disease, and typically associated with an underlying neoplasm, particularly lymphoproliferative disorders. Current management of PNP primarily involves glucocorticoids, immunosuppressants, and rituximab. This article reports a 67-year-old male who presented with generalized erythema, blisters, and oral erosions with pain for one month. Initially diagnosed with pemphigus, he received methylprednisolone treatment, which led to slight improvement in skin lesions but minimal resolution of oral ulce

Microfluidic synthesis of alumina particle-laden beads via capillary number tuning for varying particle concentrations.

The mechanisms underlying the antifertility effects of hyperprolactinemia have yet to be established in an appropriate experimental model. Hyperprolactinemia is a known side effect of fluphenazine, a broad spectrum, long-acting phenothiazine known to be dopamine type-D2 receptor antagonist. In our earlier study in adult male rats, we reported that fluphenazine at a dose of 3 mg/kg/day suppressed serum FSH but not testosterone (T) through increasing dopamine (DA) metabolism in the pituitary gland, within 60 days. Fluphenazine treatment affected sperm quality and male rats treated with fluphenaz

The efficacy-invasiveness trade-off: a retrospective cohort comparison of surgical operation and endobronchial valves for refractory lung bullae disease.

BACKGROUND: The optimal intervention for refractory bullae lung disease remains debated, with a choice between invasive surgical operation (SO) and the minimally invasive endobronchial valve (EBV). A comprehensive comparison of their long-term efficacy, safety, and cost is crucial for clinical decision-making. OBJECTIVE: To compare perioperative outcomes, 1-year efficacy, long-term recurrence-free survival, and economic impact between surgical bullectomy (SO) and endobronchial valve (EBV) placement. METHODS: In this comparative study, 673 patients with refractory bullae disease from two Chin
