Dapsone use in dermatology: indication, efficacy and tolerability experience of a UK tertiary dermatology department.

Dapsone has been a quick and affordable treatment option for a diverse range of dermatological pathologies. Although licensed for the management of leprosy and dermatitis herpetiformis, its uses expand to a range of neutrophil- and eosinophil-driven pathologies. This retrospective study examined the use of dapsone at a large tertiary dermatology department in the UK serving a diverse population. In our report, we assess indications for use and treatment efficacy, and provide 3-year follow-up data on clinical outcomes and adverse events, and their frequency. These data demonstrate the unpredict

Cohort study of neurological and psychiatric morbidity in dermatitis herpetiformis and celiac disease.

BACKGROUND: Neurological and psychiatric morbidity has been associated with celiac disease but has been scarcely studied in dermatitis herpetiformis (DH), a cutaneous manifestation of celiac disease. Hence, this cohort study aimed to investigate neurological and psychiatric morbidity in patients with DH and celiac disease. METHODS: The study comprised 368 DH patients and 1,072 celiac disease patients without DH and their 1,099 and 3,197 refences, matched 1:3 on age, sex, calendar period and place of residence. Their neurological and psychiatric morbidity was studied using the Care Register fo

Multifocal motor neuropathy secondary to gluten intolerance: a case report.

BACKGROUND: Peripheral nervous system manifestations of gluten sensitivity usually present as distal symmetric axonal polyneuropathy, small fiber neuropathy, or sensory neuropathy, often accompanied by ataxia or painful symptoms. By contrast, motor neuropathies are extremely rare, with only a few cases of multifocal motor neuropathy reported. Notably, the most recent guidelines of the European Society for the Study of Coeliac Disease do not recognize multifocal motor neuropathy as a classical neurological manifestation of gluten-related disorders. The main differential diagnoses include amyotr

Isolated Hand Involvement as the Presenting Sign of Dermatitis Herpetiformis.

Dermatitis herpetiformis (DH) is an autoimmune blistering disease associated with celiac disease, classically presenting as intensely pruritic erythematous vesicles and papules on the elbows, knees, buttocks, and scalp. Involvement of the dorsal hands is extremely rare, complicating diagnosis. We report a unique case of DH confined solely to the dorsal hands in a 68-year-old woman with a five-year history of recurrent, pruritic vesicles initially misdiagnosed as hand eczema. Direct immunofluorescence confirmed the diagnosis of DH with granular immunoglobulin (Ig) A deposition along the dermo-e

Beyond the codes: radiological and neurosensorial nuances in the celiac-dermatitis herpetiformis spectrum.

Background: The recent cohort study by Nilsson et al. provides the first registry-based comparison of neurological and psychiatric morbidity between dermatitis herpetiformis (DH) and celiac disease (CD) without DH, reporting divergent risk profiles. Methods: A critical appraisal was conducted focusing on methodological design, particularly the reliance on ICD-coded administrative data and its implications for phenotypic ascertainment in neurological and neuropsychiatric outcomes. Results: The study reports a modest neurological burden in DH (HR 1.27, 95% CI 0.94-1.71) compared with significant

Pemphigus Herpetiformis Presenting With Mucosal Involvement: A Rare Clinical Presentation.

Pemphigus herpetiformis (PH) is a rare autoimmune blistering disorder, considered a variant of pemphigus, characterized by clinical features resembling dermatitis herpetiformis and immunopathologic findings consistent with pemphigus. Patients typically present with pruritic, urticarial, papular, or vesiculobullous eruptions, while mucosal involvement is uncommon. Due to its overlapping clinical and histopathologic features with those of other blistering diseases, the diagnosis can be challenging. We report a case of a 49-year-old female who presented with itchy and painful, well-defined erythe

Mortality, cardiovascular disease, and cancer in coeliac disease and dermatitis herpetiformis: a matched cohort study.

BACKGROUND: Recent data on mortality risk in coeliac disease (CeD) have been contradictory. Large-scale research on mortality, cardiovascular disease (CVD), and cancer in dermatitis herpetiformis (DH) remains scarce. METHODS: Using the U.S. TriNetX database we identified 204,056 adults with CeD and 6896 with DH between 2005 and 2025, and individually matched 1:1 with comparators without CeD or DH. Cox regression estimated hazard ratios (HRs) for mortality, CVD, and cancer, matching cohorts based on age, sex, nicotine dependence, and comorbidities. FINDINGS: The mean age of patients with CeD

Fibrillar-Type Dermatitis Herpetiformis: An Underrecognized Variant That Is Rarely Associated With Celiac Disease and Lacking Epidermal Transglutaminase Deposition: A Report of Two Cases.

Dermatitis herpetiformis (DH) is a rare, intensely pruritic cutaneous manifestation of celiac disease (CD), typically presenting with excoriated papulovesicular lesions on extensor surfaces. DH is associated with IgA autoantibodies against epidermal transglutaminase (eTG), which form granular immune deposits in the papillary dermis and induce subepidermal blistering. Histopathology usually shows papillary dermal microabscesses and granular IgA deposits on direct immunofluorescence. We describe two patients with atypical DH featuring fibrillar, rather than granular, IgA deposits and no evidence

Spectrum of Gluten Intolerance.

The spectrum of gluten intolerance spans celiac disease, dermatitis herpetiformis, nonceliac gluten sensitivity, and wheat allergy. These conditions share gluten as the driver perpetuating symptoms and pathology. This review seeks to compare and contrast the main conditions on the spectrum of gluten intolerance with respect to definition, epidemiology, pathophysiology, clinical presentation, diagnosis, and treatment.

Exuberant Dermatitis Herpetiformis With Oral Involvement Mimicking Behçet Disease and Disseminated Herpetic Infection: The Crucial Role of Direct Immunofluorescence.

Dermatitis herpetiformis (DH) is a chronic autoimmune blistering dermatosis associated with gluten-sensitive enteropathy and characterized by intensely pruritic papulovesicular eruptions, typically distributed symmetrically on extensor surfaces. Although DH classically presents with grouped vesicles and excoriated papules, atypical and exuberant manifestations may represent a substantial diagnostic challenge, frequently mimicking infectious, inflammatory, and autoimmune blistering disorders. We report the case of a 30-year-old woman presenting with a one-year history of progressive, intensely

A Case of Pemphigus Herpetiformis in a 10-Year-Old Child.

Pemphigus herpetiformis (PH) is a rare variant of pemphigus. Pediatric PH is exceedingly rare, and its clinical manifestations in children overlap with those of dermatitis herpetiformis and linear IgA bullous dermatosis (LABD), often leading to clinical misdiagnosis. Here, we describe a 10-year-old girl presenting with isolated bilateral lower limb erythema, clustered tense vesicles, and severe pruritus, without mucosal involvement. The diagnosis was confirmed by elevated desmoglein 1 antibodies, subcorneous vesicles, and epidermal intercellular IgG/C3 deposition. Histopathology revealed an at

From photosensitivity to autoimmunity: the role of AI metabolomics in PCT-celiac disease overlap.

Porphyria cutanea tarda (PCT), caused by hepatic uroporphyrinogen decarboxylase deficiency, may rarely overlap with undiagnosed celiac disease (CD), complicating diagnosis due to shared hepatic and dermatologic features. Both conditions present with skin manifestations such as photosensitivity, blistering, or dermatitis herpetiformis-like lesions, often leading to misclassification. AI-driven metabolomics provides a novel strategy to identify metabolic biomarkers, predict CD with high accuracy through machine learning, and uncover hidden comorbidities in PCT patients. Despite challenges includ

European S2k guidelines on management of autoimmune blistering diseases in children and adolescents.

INTRODUCTION: Autoimmune blistering diseases (AIBDs) in children and adolescents present a major challenge in diagnosis and management because of their rarity and the presence of other differential diagnoses with blistering. They are potentially serious, and timely diagnosis and optimal management can help prevent morbidity related to both the diseases and their treatments. METHODS: The Autoimmune Blistering Diseases Task Force of the European Academy of Dermatology and Venereology (EADV) has initiated evidence-based, expert-informed clinical guidelines for the diagnosis, treatment and monito

The risk of inflammatory bowel disease is increased in celiac disease but not in dermatitis herpetiformis.

BACKGROUND: Although an association between celiac disease and inflammatory bowel disease (IBD) has been recognized, there remains limited research on the subject. Comorbidities vary between the phenotypes of celiac disease, but the risk of IBD in the extraintestinal manifestation of celiac disease, dermatitis herpetiformis (DH), has been previously studied mainly through case reports. This study aimed to compare the risk of IBD between celiac disease and DH patients and references. METHODS: The study population consisted of 1,072 celiac disease and 368 DH patients with biopsy-proven diagnosi
