The risk of inflammatory bowel disease is increased in celiac disease but not in dermatitis herpetiformis.

BACKGROUND: Although an association between celiac disease and inflammatory bowel disease (IBD) has been recognized, there remains limited research on the subject. Comorbidities vary between the phenotypes of celiac disease, but the risk of IBD in the extraintestinal manifestation of celiac disease, dermatitis herpetiformis (DH), has been previously studied mainly through case reports. This study aimed to compare the risk of IBD between celiac disease and DH patients and references. METHODS: The study population consisted of 1,072 celiac disease and 368 DH patients with biopsy-proven diagnosi

Diagnosis of Autoimmune Blistering Diseases: Dermatopathology and Other Laboratory Updates.

Autoimmune blistering diseases (AIBD), an immunologically diverse group associated with intraepithelial or subepithelial blistering, can present with protean clinical or histopathologic findings. Accurate diagnosis is important to predict clinical course, stratify risk for certain associated systemic conditions, and determine patient eligibility for various therapies. While helpful clues can be gleaned from microscopic evaluation of tissue, definitive diagnostic changes are not always present. Therefore, it is important for clinicians and dermatopathologists to recognize the various clinicopat

Population-Based and Routinely Collected Health Data Studies of Immunobullous Diseases: A Scoping Review.

BACKGROUND: Validated case definitions are essential for reliable epidemiological research using routinely collected health data. The extent to which population-based studies on autoimmune blistering diseases (AIBDs) use validated case definitions is uncertain, and variability in definitions may limit the generalizability of findings. OBJECTIVE: To describe definitions of AIBDs used in population-based epidemiologic studies and whether these definitions have been validated. METHODS: MEDLINE and Embase were searched for epidemiologic studies on pemphigoid, pemphigus and their variants, dermat

Dermatitis Herpetiformis in Celiac Disease: A Systematic Review and Meta-Analysis.

Dermatitis herpetiformis (DH) shares a pathophysiological basis with celiac disease (CD), but its epidemiology remains poorly defined. This systematic review and meta-analysis estimated the prevalence of DH among CD patients. Following the PRISMA 2020 guidelines, PubMed, Embase, Web of Science, and Scopus were searched. Observational studies with extractable DH data in CD patients were pooled using a random-effects model, with subgroup and meta-regression analyses. The risk of bias was assessed using the JBI Checklist. Of 7271 records, 24 studies published between 1996 and 2023, comprising 73,

Causal Relationships Between Oral Microbiota and Inflammatory Skin Diseases.

INTRODUCTION AND AIMS: The oral microbiome has been increasingly linked to systemic inflammation and immune dysregulation, but whether specific oral bacteria causally contribute to inflammatory skin diseases remains unclear due to confounding and reverse causation. This study aimed to assess the causal effects of 43 oral microbiota taxa on the risk of five inflammatory skin diseases using a Mendelian randomization (MR) approach. METHODS: We performed a two-sample MR analysis using genetic instruments for oral microbiota derived from publicly available genome-wide association studies and outco

Connecting Skin and Gut in Dermatitis Herpetiformis: Insights from Gluten Challenge Transcriptomics.

Dermatitis herpetiformis (DH) is a dietary gluten -induced autoimmune blistering skin disease. It is hypothesized to be initiated by a small intestinal mucosal immune response. Gluten-free diet (GFD) is the primary treatment for DHdermatitis herpetiformis and effectively resolves skin lesions. Previous studies show that after gluten reintroduction, peripheral cell responses are detectable already in six6 days, and most patients relapse clinically within months. The relapse presentation may differ between patients, with some experiencing both rash and small intestinal mucosal damage and some on

Immune checkpoint inhibitor-associated autoimmune bullous disease: A clinical review.

Immune checkpoint inhibitors (ICIs) have markedly advanced oncology by enhancing the immune system's ability to combat malignancies. However, their use is increasingly associated with immune-related adverse events, including immunobullous dermatoses. These disorders, such as bullous pemphigoid, pemphigus vulgaris, linear immunoglobulin A bullous dermatosis, mucous membrane pemphigoid, paraneoplastic pemphigus, dermatitis herpetiformis, and lichen planus pemphigoides present significant diagnostic and therapeutic challenges. This clinical review explores the pathogenesis, clinical presentation,

Clinical Indicators Suggesting Dapsone-Induced Methemoglobinemia in Dermatology Outpatients: A Pilot Retrospective Cohort Study.

Background Dapsone is used widely in dermatology as an anti-inflammatory agent for diseases including lichen planus, dermatitis herpetiformis, bullous pemphigoid, and sub-corneal pustular dermatosis (SCPD). Anemia and methemoglobinemia are known hematologic adverse effects of dapsone. These occur through distinct mechanisms and therefore develop independently of each other. Mild-to-moderate methemoglobinemia produces non-specific symptoms such as fatigue, exertional dyspnea, headache, and postural syncope that may affect daily functioning, impair treatment compliance, and lead to unnecessary e

Dermatitis Herpetiformis Revealing Acute Lymphoblastic Leukemia in a Child: A Case Report.

A two-year-and-nine-month-old girl was referred for widespread bullous and herpetiform skin lesions associated with limb edema and asthenia, evolving over approximately seven months. The initial clinical evaluation raised suspicion of bullous systemic lupus erythematosus based on cutaneous findings and abnormal blood tests, including proteinuria, hypoalbuminemia, and low complement C4. Skin histopathology showed dermal edema with a neutrophilic infiltrate, and direct immunofluorescence revealed granular IgA and C3 deposits along the dermoepidermal junction, findings diagnostic of dermatitis he

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Lactoferrin concentration in breast milk during the first postpartum year in Lima Norte, Peru.

Bile duct injuries (BDI) tend to be more complex in laparoscopic than in open cholecystectomy procedures, and frequently involve young adults with benign pathologies. The ultimate consequence may be a liver transplantation (LT), making this situation one of the most rare transplant indications. Fatal post-transplant outcome is extreme infrequently reported. Aim of this study is to report on our single-case experience and to review the literature concerning lethal outcome after LT for major BDI following cholecystectomy. A 36-year old obese caucasian woman underwent a laparoscopic cholecystecto

Inflammatory Myopathies and Autoimmune Gluten-related Disorders: A Scoping Review of Pathophysiological Interconnections and Hypothesis.

INTRODUCTION: Anecdotal reports describe patients with concurrent idiopathic inflammatory myopathy (IIM) and celiac disease (CeD) in whom the introduction of a gluten-free diet led to dramatic improvement of myositis. We first systematically reviewed all peer-reviewed publications on concomitant IIM and duodenal biopsy-verified CeD. The collected evidence was suggestive of associations between myositis disease activity and gluten exposure in some patients with IIM-CeD. METHODS: To investigate possible explanations for the observations, an exploratory review of basic pathophysiological relatio

Enzyme-activating B-cell receptors boost antigen presentation to pathogenic T cells in gluten-sensitive autoimmunity.

Autoantibodies against the enzyme transglutaminase 3 (TG3) are characteristic to the gluten-sensitive skin disorder dermatitis herpetiformis (DH), which is an extraintestinal manifestation of celiac disease. We here demonstrate that TG3-specific B cells can activate gluten-specific CD4+ T cells through B-cell receptor (BCR)-mediated internalization of TG3-gluten enzyme-substrate complexes. Stereotypic anti-TG3 antibodies using IGHV2-5/IGKV4-1 gene segments enhance the catalytic activity of TG3, and this effect translates into increased gluten presentation to T cells when such antibodies are ex

From Gut to Gray Matter: A Case Report of Posterior Reversible Encephalopathy Syndrome in a Pediatric Patient With Celiac Disease.

Posterior reversible encephalopathy syndrome (PRES) is a rare neurological condition characterized by vasogenic edema, primarily affecting the posterior cerebral hemispheres. Although typically reversible with prompt treatment, PRES can lead to severe complications if not recognized early. This report presents an unusual and fatal case of PRES in a pediatric patient with celiac disease, a combination that is rarely documented in medical literature. A 9-year-old child with a history of celiac disease and dermatitis herpetiformis presented to the Emergency Room with a two-month history of vomiti

Prurigo nodularis phenotype in the bullous dermatoses spectrum.

Prurigo nodularis (PN) of Hyde is a highly pruritic condition that results from the vicious cycle of itching and scratching. An increasing number of case studies report PN-mimicking lesions in patients with confirmed autoimmune bullous dermatoses (AIBDs). The PN lesions usually precede the blister formation and thus raise the question about the pathogenic relation between the two conditions and the possible role of PN as a trigger of AIBDs. We shed light on PN in the context of AIBDs, especially regarding the clinical characteristics and pathogenesis of the PN-like phenotype of bullous pemphig

Efficacy of probiotics in dermatitis herpetiformis management: an umbrella review.

BACKGROUND: The available evidence on probiotics in Dermatitis Herpetiformis (DH) remains severely limited. Given the shared pathophysiology of DH and Coeliac disease (CD), we aimed to provide the hypothesis to synthesize the narrative reviews carried out so far on the use of probiotics in the treatment of DH, its impact on gut microbiota dysbiosis, and the gut-skin axis. METHODS: Relevant narrative reviews were searched for in electronic databases such as PubMed, Scopus, Cochrane Library, Embase, and Google Scholar.
