A First-In-Human Randomized Controlled Phase 1 Study Assessing the Safety and Tolerability of Topical TCP-25 Gel in Epidermal Suction Blister Wounds.

Inflammation and infection remain unmet challenges in wounds of various etiologies, delaying healing, impacting quality of life, and increasing healthcare costs. The thrombin-derived C-terminal peptide TCP-25 has demonstrated dual anti-inflammatory and antibacterial activities in animal wound infection models, thereby promoting healing, highlighting its therapeutic potential as a wound treatment. This first-in-human, double-blind, randomized, within-person and placebo-controlled clinical trial (NCT05378997) evaluated the safety, tolerability, and pharmacokinetics of topical TCP-25 in healthy v

Plakoglobin phosphorylation at serine 665 is capable of stabilizing cadherin-mediated adhesion in keratinocytes.

In pemphigus, autoantibodies against the desmosomal cadherins desmoglein (DSG) DSG1 and DSG3 cause intraepidermal blistering. Recently, we found that increasing cAMP with the phosphodiesterase-4 inhibitor apremilast stabilizes keratinocyte cohesion in pemphigus. This effect is paralleled by phosphorylation of the desmosomal plaque protein plakoglobin (PG) at serine 665 (S665). Here, we investigated the relevance of PG phosphorylation at S665 for stabilization of keratinocyte cohesion and further characterized the underlying mechanisms. Ultrastructural analysis of a recently established PG-S665

Cutaneous autoimmune blistering disease during mirikizumab therapy in a patient with ulcerative colitis: a new potential trigger?

Autoimmune blistering diseases (AIBDs) may occur as adverse effects of various therapies, including biologics. Data on their association with IL-23 inhibitors in ulcerative colitis (UC) remain limited. We report the case of a 59-year-old man with UC who developed pruritic cutaneous bullae emerging within days following mirikizumab infusion. Histopathological examination and direct immunofluorescence demonstrated findings consistent with an AIBD. Following the withdrawal of mirikizumab, the residual cutaneous lesions progressively improved under corticosteroid therapy, suggesting a temporal ass

Diagnosis and Management of Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis in Pediatric Patients: A Systematic Review of Clinical Guidelines and Consensus Statements.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare, life-threatening mucocutaneous reactions. Pediatric mortality is lower than in adults, but children face higher risks of recurrence and long-term sequelae. Despite disease severity, pediatric-specific guidelines remain scarce and often extrapolated from adult populations. We systematically reviewed and appraised national and international clinical practice guidelines and consensus statements on pediatric SJS/TEN, focusing on supportive care, pharmacologic interventions, mucosal management, and long-term follow-up. A case of delayed-onset Stevens-Johnson syndrome following co-amoxiclav administration in a 98-year-old female.

BackgroundStevens-Johnson Syndrome is a type IV hypersensitivity mucocutaneous reaction characterised by a widespread desquamating rash, and is considered to be an emergency due to its potentially fatal complications. Drugs account for 50-80% of Stevens-Jonson Syndrome/Toxic Epidermal Necrolysis (SJS/TEN) cases, with anticonvulsants, antibiotics, analgesics and allopurinol being the most common causative agents. Co-amoxiclav contains amoxicillin (a penicillin) and clavulanic acid (a beta-lactamase inhibitor) and is widely used in healthcare settings for treatment of common infections due to it

Molecular Insights Into TNF-α and IFN-γ-Induced Epithelial Cell Death in Toxic Epidermal Necrolysis.

Rapid and extensive epithelial cell death in skin and mucous membranes is the hallmark of Toxic epidermal necrolysis (TEN). High levels of TNF-α and IFN-γ have been detected in the blister fluids of skin lesions in TEN patients. Available literature shows that TNF-α synergy with IFN-γ (TNF-α/IFN-γ) elicited cell death in diverse cells. Thus, the cytotoxic effect and underlying mechanism of TNF-α and IFN-γ on HaCaT cells were investigated. Confluent HaCaT cells were subjected to serial doses of either TNF-α alone, IFN-γ alone, or combination TNF-α with IFN-γ (TNF-α/IFN-γ) for indicated time int

Treatment Landscape for Epidermolysis Bullosa Simplex: A Review of Established and Emerging Therapies.

Epidermolysis bullosa (EB) is a group of rare genetic blistering skin diseases, with EB simplex (EBS) being the most common subtype, accounting for around 70% of cases. Although there is no cure for EB, recent advancements led to the historic FDA-approvals of two treatments for dystrophic and junctional EB in 2023 and a third for dystrophic EB in 2025. However, no approved treatments exist for EBS despite its relative prevalence. Here, we outline established and emerging therapies for EBS. Supportive EBS management focuses on five key areas: skin-directed wound care, sweating reduction, kerato

Acantholytic disorders: Update on pathophysiology, diagnosis, and management.

Acantholytic skin disorders without a primary immunological etiology are characterized by impaired intercellular adhesion between epidermal keratinocytes. Genetic acantholytic disorders include Darier disease and Hailey-Hailey disease; the pathogenesis of Grover disease is unknown. Since these conditions are not commonly seen in clinical practice, diagnoses may be missed and there are no standardized guidelines to direct treatment. Topical steroids and retinoids are typically utilized for mild disease, and oral retinoids, particularly acitretin, have often been used for more severe disease, bu

Developing CRISPR-Based Therapies for Epidermolysis Bullosa: A Comprehensive Review of Current Strategies.

Currently, there is no permanent treatment for the group of severe monogenic fragile skin conditions epidermolysis bullosa (EB). The recent US Food and Drug Administration (FDA)-approved in vivo gene replacement therapy beremagene geperpavec (Vyjuvek®) provides a promising solution, but it requires ongoing application and is not applicable to all forms of EB. Targeted gene editing approaches directly addressing pathogenic mutations hold great promise for the development of durable personalized therapies. Here, we comprehensively describe the clustered regularly interspaced short palindromic re

Stevens-Johnson syndrome/toxic epidermal necrolysis induced by sintilimab in a patient with advanced non-small cell lung cancer: A case report.

RATIONALE: Sintilimab, an immune checkpoint inhibitor, enhances T-cell responses, leading to robust antitumor activity, and is approved for the treatment of lung cancer. While immune checkpoint inhibitors offer substantial clinical benefits, they are often associated with immune-related adverse events, particularly cutaneous toxicities. These skin reactions typically manifest as mild maculopapular rashes; however, more severe manifestations, such as Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN), can occur. Both SJS and TEN are life-threatening conditions with high mortali

Extracorporeal Membrane Oxygenation Therapy in Toxic Epidermal Necrolysis-Related Respiratory Failure: A Case Report.

Toxic epidermal necrolysis (TEN) is a rare and life-threatening cutaneous adverse reaction characterized by > 30% total body surface area desquamation and mucosal involvement. Although ocular, oral, and urogenital mucosal involvements are well described, pulmonary mucosal injury is uncommon and associated with high mortality. We report a case of a 33-year-old woman with rapidly progressive TEN complicated by respiratory failure. Despite early multidisciplinary management including local wound care, immunomodulatory therapy, systemic corticosteroids, and lung-protective mechanical ventilation,

The effect of different therapies and complete blood cell count-derived inflammatory biomarkers on the prognosis of Stevens-Johnson syndrome and toxic epidermal necrolysis.

BACKGROUND: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe mucocutaneous adverse drug reactions still with controversy in treatment. Complete blood cell count-derived inflammatory biomarkers reflect systemic inflammation, and their value in SJS/TEN has not been fully studied. OBJECTIVES: To compare the therapeutic efficacy of corticosteroid combined with adalimumab (CA), corticosteroid combined with intravenous immunoglobulin (CI) and corticosteroid monotherapy (CM), and explore prognosticators on severity and prognosis of SJS/TEN. METHODS: All inpatients with

Intravenous immunoglobulin as a therapy for severe or steroid-recalcitrant erythema multiforme major: a case series.

Erythema multiforme (EM) is typically self-limiting. However, severe EM major may cause significant morbidity and lacks evidence-based therapeutic guidelines. Intravenous immunoglobulin (IVIg) has primarily been used in recurrent EM. This study evaluates IVIg use in severe first-episode or steroid-recalcitrant EM major. A retrospective analysis reviewed six patients with EM major treated with IVIg at a tertiary Australian hospital. IVIg was administered in eight instances. Disease triggers included Mycoplasma pneumoniae in three admissions, unknown aetiology in three admissions, rhinovirus/inf

Erosive pustular dermatosis of the lower legs (EPDL): A rarely diagnosed neutrophilic dermatosis of the elderly.

Erosive pustular dermatosis of the lower legs (EPDL) is a rarely diagnosed, chronic inflammatory skin disease that occurs predominantly in elderly people. Predisposing factors include skin atrophy, chronic venous insufficiency, and trauma. Although the pathogenesis of EPDL has not yet been conclusively clarified, there are ongoing discussions on its nature as a neutrophilic dermatosis in which exogenous triggers lead to immunological dysregulation with local skin damage. Clinically, EPDL manifests with superficial, sterile pustules from which sharply defined erosions develop. The predilection

An evaluation of prademagene zamikeracel for the treatment of recessive dystrophic epidermolysis bullosa.

INTRODUCTION: Prademagene zamikeracel is an autologous, genetically corrected epidermal graft therapy designed to address the underlying molecular defect in recessive dystrophic epidermolysis bullosa (RDEB). The product is manufactured from a patient's own keratinocytes, which are modified ex vivo to overexpress a normal copy of COL7A1, the gene coding for type VII collagen (C7), which is essential for anchoring fibril formation and stable dermal-epidermal cohesion. In individuals with biallelic COL7A1 variants, loss of C7 results in severe skin fragility, painful and widespread wounds, debili

A rare drug reaction: Toxic epidermal necrolysis following polymyxin B administration in a post-Bentall procedure.

Polymyxin B is a last-resort antibiotic used to treat multidrug-resistant (MDR) Gram-negative infections. While nephrotoxicity and neurotoxicity are well-documented, severe dermatologic adverse reactions such as toxic epidermal necrolysis (TEN) are exceedingly uncommon but clinically significant. A 54-year-old hypertensive male underwent an emergency Bentall procedure for DeBakey type I aortic dissection. The patient developed acute kidney injury requiring dialysis. The patient subsequently developed polymyxin B-sensitive Acinetobacter lwoffii septicemia, for which intravenous polymyxin B was
