Ventilator-associated pneumonia in adult ICU patients with epidermal necrolysis: A retrospective cohort study from the French National Reference Centre.

BACKGROUND: Epidermal necrolysis (EN) is a rare yet severe drug-induced disease that results in the detachment of the epidermis leading to infection and acute respiratory failure requiring mechanical ventilation. This study aims to describe the prevalence of ventilator-associated pneumonia (VAP) and its microbiological characteristics in patients with EN, as well as identifying the associated risk factors and outcomes. METHODS: This is an observational, retrospective, single-centre cohort study of adult patients with EN who were admitted to the intensive care unit (ICU) and required mechanica

Bullous pemphigoid outcome measurement: a scoping review of outcome heterogeneity and trends in clinical research of treatments.

BACKGROUND: Bullous pemphigoid (BP) is the most common autoimmune bullous disease. It is characterized by the development of tense blisters and pruritus. BP is a chronic disease that requires long-term treatment often associated with serious side effects. Developments in understanding the disease have led to novel treatment approaches evaluated in clinical trials. However, the pooling of trial data remains challenging due to inconsistent outcome reporting. OBJECTIVES: To provide an overview of BP outcome measurement over the last two decades by mapping and listing all previously reported trea

Observations of dystrophic epidermolysis bullosa patients with collagen VII NC2 truncation provide new insights into anchoring fibril assembly.

Type VII collagen (C7) assembles into anchoring fibrils (AF) which are basement membrane zone (BMZ) structures that reinforce dermal-epidermal cohesion. Mutations in COL7A1, which encodes C7, cause dystrophic epidermolysis bullosa (DEB), a blistering disorder characterized by skin fragility. C7 consists of a central collagenous domain flanked by non-collagenous NC1 and NC2 regions. During AF assembly, C-proteinase-mediated cleavage of the NC2 domain enables anti-parallel dimer formation, followed by C7 lateral association into mature, banded AFs that loop through the dermis to anchor the epide

Predictors of Survival and Length of Stay in Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis: A Single-Center Retrospective Study of 111 Patients.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) comprise a spectrum of severe mucocutaneous drug reactions associated with significant morbidity and mortality. Current prognostication of survival and length of stay is imprecise and unreliable across different populations. We assessed and quantified patient, disease, and management factors associated with survival and length of stay in an Australian cohort of adults with SJS/TEN. We reviewed 16 years of SJS and TEN cases at a large tertiary referral center in Australia. Collated variables were demographics, comorbidities, di

Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis: A Systematic Review of Cases Secondary to Topical Medications.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are typically triggered by systemic medications but can also result from topical agents. In this systematic review of SJS/TEN cases caused by topical agents, the drug culprits, disease characteristics, and outcomes were comparable to systemic cases. Our findings highlight that topical formulations, though generally considered safer, can induce SJS and TEN, underscoring the need for clinician awareness, early recognition and accurate reporting.

SCORTEN and Novel Prognostic Markers in Stevens-Johnson Syndrome/Toxic Epidermal Necrolysis: A Systematic Review and Meta-Analysis.

Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, severe skin reaction with high mortality, most often triggered by medications. Early identification of patients at risk is essential for guiding treatment. The SCORTEN score, comprising seven clinical and laboratory variables, is widely used to predict mortality but may have limitations across different populations. We conducted a systematic review and meta-analysis to assess SCORTEN's predictive accuracy and identify additional blood biomarkers associated with mortality in SJS/TEN. The primary aim was to evaluate the pro

Characterisation of the wound microbiome and antimicrobial resistance profiles in clinical isolates from epidermolysis bullosa patients.

INTRODUCTION: Epidermolysis bullosa (EB) encompasses a rare, inherited group of skin disorders characterised by skin fragility, leading to painful, chronic wounds. While recent international studies have reported reduced microbial diversity and altered bacterial composition in EB blistered skins, no microbiome or antimicrobial resistance data is available for Australian EB patients. The objectives of this study were to characterise the wound microbiome of Australian EB patients and identify bacterial pathogens and their associated antimicrobial resistance profiles. METHODS: A total of 15 chron

Neonatal KLHL24-Associated Epidermolysis Bullosa Simplex: Clinical Presentation and Genetic Confirmation of a Rare Skin Fragility Syndrome.

Epidermolysis bullosa simplex (EBS) with cardiomyopathy is a rare subtype caused by gain-of-function pathogenic variants in the KLHL24 gene, leading to both skin and cardiac involvement. We report a neonate with congenital erosions, scarring, and follicular atrophoderma, but minimal blistering, later confirmed to have a pathogenic KLHL24 variant. This presentation highlights atypical neonatal features that may mimic other conditions and emphasizes the importance of early genetic diagnosis. Given the risk of dilated cardiomyopathy, ongoing cardiac monitoring and family evaluation are essential.

Identification of Novel ATP2C1 Mutations in a Spanish Cohort of Patients With Hailey-Hailey Disease.

Hailey-Hailey disease (HHD) is a rare autosomal dominant genodermatosis characterized by blisters and erosions in skin folds, significantly impairing patients' quality of life. HHD is caused by mutations in the ATP2C1 gene, which encodes the calcium transport protein SPCA1. Approximately 290 unique mutations have been identified to date; however, data remain scarce regarding mutations affecting patients in certain areas of Europe. The aim of this study was to analyze the ATP2C1 gene in a cohort of Spanish patients with HHD and to explore a possible genotype-phenotype correlation. We detected 1

[Anti-p200 pemphigoid with figurate exanthem : Rare cutaneous pattern of a bullous autoimmune dermatosis with paraneoplastic association].

A patient presented with bullous dermatosis. Clinically, the skin lesions showed a gyrate pattern resembling erythema gyratum repens (EGR). We diagnosed anti-p200 pemphigoid, which, like EGR, is a possible paraneoplasia. A search for tumors confirmed the presence of bronchial carcinoma. Other case reports with combined occurrence also point to underlying malignancies. This case highlights the importance of differential diagnosis in cases of figured efflorescences and bullous dermatoses and the need to investigate paraneoplasia.

Health-related quality of life in adults with epidermolysis bullosa: a cross-sectional study in seven European countries using EQ-5D-5L.

BACKGROUND: Epidermolysis bullosa (EB) is a rare genetic disorder that causes extreme skin fragility, chronic pain, and functional impairment, with major psychosocial and economic consequences. Health-related quality of life (HRQoL) data is critical to capture the full burden of EB. Health utilities derived from preference-based generic instruments such as the EQ-5D-5L provide standardized health status utility values that enable cross-disease comparisons and provide input data for cost-utility analyses to inform resource allocation. There is a notable lack of multinational, up-to-date utility

Social participation of patients with epidermolysis bullosa: barriers and facilitators in education, employment, and economic life - a biographic interview study.

BACKGROUND: Epidermolysis Bullosa (EB) is a rare, inherited skin disorder characterized by skin fragility, leading to painful blisters and wounds. While protective measures are essential for managing EB, they can inadvertently restrict physical activity and social participation. Recognizing this dilemma, the current psychosocial care guidelines for EB emphasize the need to support social participation as a key priority. Accordingly, this study explores barriers and facilitators influencing participation in education, employment, and economic life, as conceptualized by the International Classif
