PURPOSE: Blood blister-like aneurysms (BBLAs) are rare, fragile lesions associated with a high risk of early rebleeding, yet they are frequently occult on initial non-invasive imaging. Early identification is therefore critical. This study introduces a novel non-contrast CT (NCCT) marker - the blister sign -for early recognition of BBLAs in acute subarachnoid hemorrhage (SAH).
METHODS: We conducted a retrospective multicenter cohort study across five tertiary neurovascular centers, including consecutive patients presenting with non-traumatic SAH in 2023. The blister sign was defined on NCCT a

A 58-year-old man was diagnosed with acute myeloid leukemia with myelodysplasia-related changes. Induction therapy with CPX-351 achieved complete remission, and a second course of consolidation therapy was administered. Shortly after the second course, he developed aphthous ulcers on the tongue that progressively worsened. Although a tongue biopsy showed no specific findings, serum anti-bullous pemphigoid 180 antibodies were positive. Mucous membrane pemphigoid (MMP) was diagnosed, and corticosteroid therapy led to a rapid improvement. Given the temporal relationship between CPX-351 administra

Background: The recent cohort study by Nilsson et al. provides the first registry-based comparison of neurological and psychiatric morbidity between dermatitis herpetiformis (DH) and celiac disease (CD) without DH, reporting divergent risk profiles. Methods: A critical appraisal was conducted focusing on methodological design, particularly the reliance on ICD-coded administrative data and its implications for phenotypic ascertainment in neurological and neuropsychiatric outcomes. Results: The study reports a modest neurological burden in DH (HR 1.27, 95% CI 0.94-1.71) compared with significant

BACKGROUND: Pemphigus vulgaris (PV) is a CD4+ T-cell-dependent autoantibody-mediated blistering disease associated with human leucocyte antigen (HLA) class II molecules. IgG autoantibodies against the primary autoantigen desmoglein 3 (Dsg3), a desmosomal adhesion protein on epidermal keratinocytes, cause loss of epidermal cell adhesion.
OBJECTIVES: To assess the clinical applicability of an innovative nanoparticle platform for the induction of immune tolerance exploiting the natural tolerance potential of liver sinusoidal endothelial cells. An open-label first-in-human study was conducted wit

BACKGROUND: Immune checkpoint inhibitors (ICIs) can trigger immune-related adverse events (irAEs), among which Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is exceptionally rare but potentially fatal. Sintilimab, a PD-1 inhibitor increasingly used for advanced gastric cancer, has only sporadically been linked to SJS/TEN.
CASE PRESENTATION: We report a 60-year-old man with metastatic gastric adenocarcinoma who developed TEN 7 days after the first sintilimab infusion. He presented with rapidly progressive diffuse erythema, bullae, and epidermal detachment involving >90% of the

Background/Objectives: Non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed to help alleviate pain and treat inflammation, but they are also recognized as common causes of severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). Despite their clinical importance, pharmacogenetic markers to predict individual susceptibility to NSAID-induced SJS/TEN remain insufficiently defined. This study investigated associations between HLA class I and II alleles, CYP2C9 polymorphisms, and NSAID-induced SJS/TEN in a Thai popula

Toxic epidermal necrolysis (TEN) is defined histopathologically by full-thickness epidermal necrosis with sparse dermal inflammation. Because mucosal involvement is considered a hallmark feature, mucosal-sparing TEN is exceedingly rare and may mimic other subepidermal blistering disorders. We report an amoxicillin/clavulanate-induced TEN case presenting with tense annular vesiculobullous lesions with a "string-of-pearls" appearance clinically suggestive of linear IgA bullous dermatosis (LABD). However, histopathology showed widespread keratinocyte necrosis across all epidermal layers and minim

BACKGROUND AND PURPOSE: Bruton tyrosine kinase (Btk) is essential for B cell function. Its role in myeloid cells is less understood. Greater insights into Btk significance in myeloid cells are needed to evaluate its potential as a therapeutic target during the effector phase of antibody-induced autoimmune diseases, where inhibiting autoantibody production suppresses tissue inflammation only after a delay. Such a situation can be observed, for example, in acute flares of pemphigoid diseases, a group of autoimmune blistering skin diseases.
EXPERIMENTAL APPROACH: We examined the effect of neutro

Epidermolysis bullosa acquisita (EBA) is a rare autoimmune blistering disease caused by autoantibodies to type VII collagen. While dipeptidyl peptidase-4 (DPP-4) inhibitors are established triggers for bullous pemphigoid (BP), their association with EBA has not been reported. A 68-year-old Japanese man with type 2 diabetes and chronic renal dysfunction, treated with linagliptin, developed widespread tense blisters with mucosal involvement. Histopathology and immunological studies confirmed EBA. Despite corticosteroids, cyclosporine, pulse therapy, and intravenous immunoglobulin, the disease re

Anti-p200 pemphigoid is a rare subepidermal blistering dermatosis with variable clinical manifestations that can mimic other bullous skin diseases. We report the case of a 65-year-old patient presenting with extensive bullae on the palms and soles, as well as mucosal erosions. These findings were initially consistent with a diagnosis of erythema multiforme. Direct immunofluorescence (DIF) revealed linear IgA, IgG, and C3 antibody deposits along the dermoepidermal junction. While indirect immunofluorescence (IIF) on salt split skin demonstrated IgA and IgG antibodies binding on the dermal side

A male infant, born to parents in a non-consanguineous marriage presented with a 1-month history of itchy, fluid-filled lesions in a string-of-pearl appearance all over the body following administration of the first dose of pentavalent vaccination. The results of the immunological tests confirmed the diagnosis of bullous pemphigoid. He was treated with topical and oral corticosteroids along with oral dapsone, followed by good improvement. The child received subsequent doses of the vaccine without any further relapses.

BACKGROUND: Pharmaceutical residues, particularly antidiabetic agents such as sitagliptin, have become emerging environmental pollutants due to their incomplete removal during wastewater treatment and high persistence in aquatic systems. Increasing detection of sitagliptin in sewage and surface water raises concerns about its potential ecological and human health effects. Recent clinical observations suggest that DPP4 inhibitors may be linked to bullous pemphigoid (BP), an autoimmune blistering skin disease, but the underlying mechanisms remain unclear.
OBJECTIVES: This study aimed to elucida

BACKGROUND: Alleviating sleep disturbances is a key therapeutic goal in pruritic dermatoses. However, sleep disturbance remains under-investigated in patients with bullous pemphigoid (BP), despite contributing to exhaustion in older adults and potentially increasing disease morbidity.
OBJECTIVES: To estimate the prevalence and severity of insomnia in BP patients, and examine its correlation with disease activity, self-rated pruritus, and quality of life (QoL).
METHOD: A national, multicenter, controlled study in 61 BP patients and 61 age- and sex-matched controls. Insomnia severity was asses

IgA-mediated Anti-Laminin-γ1 (p200) pemphigoid is a rare subtype of subepidermal autoimmune blistering disease (AIBD) characterized by IgA autoantibodies targeting the laminin γ1 chain, a 200 kDa protein located at the dermal-epidermal junction. Patients typically exhibit skin-dominant blistering lesions, also mucosal involvement has been reported. We report a 73-year-old male patient diagnosed with IgA-mediated anti-Laminin-γ1 (p200) pemphigoid. He initially presented with prominent palmoplantar blisters erosions on the eyelids, lips and face, accompanied by mild pruritus. Skin biopsy demonst

Moxifloxacin, a fluoroquinolone antibiotic with immunomodulatory activity, has not been evaluated for effects on autoantibody-driven inflammation. Pemphigoid diseases are a group of autoimmune blistering skin diseases driven by pathogenic autoantibodies and granulocytes. With current treatments for pemphigoid diseases associated with substantial adverse effects, there is a high medical need for new treatment strategies. We investigated the effect of moxifloxacin on skin inflammation in the antibody transfer mouse model of bullous pemphigoid-like epidermolysis bullosa acquisita. Moxifloxacin at

INTRODUCTION: Autoimmune blistering diseases (AIBDs) in children and adolescents present a major challenge in diagnosis and management because of their rarity and the presence of other differential diagnoses with blistering. They are potentially serious, and timely diagnosis and optimal management can help prevent morbidity related to both the diseases and their treatments.
METHODS: The Autoimmune Blistering Diseases Task Force of the European Academy of Dermatology and Venereology (EADV) has initiated evidence-based, expert-informed clinical guidelines for the diagnosis, treatment and monito

CASE: A 72-year-old woman with no dermatologic history developed tense blisters, erosions, and eczematous lesions over the right knee after cemented bicompartmental total knee arthroplasty. Lesions progressed with oral mucosal involvement. Two months postoperatively, admission to dermatology department led to a diagnosis of bullous pemphigoid, confirmed by direct immunofluorescence of a skin biopsy, and indirect immunofluorescence with ELISA from blood. Treatment with oral methotrexate and topical clobetasol halted blistering and achieved resolution within 4 weeks.
CONCLUSION: This case highl
