STUDY OBJECTIVE: To evaluate the clinical efficacy of diagnostic tests used for persons with suspected allergic disease.
DESIGN: Information synthesis based on historical review of developments in the understanding of the pathophysiology of allergic diseases and on selected recent literature on efficacy of specific diagnostic tests.

Hydroxycitronellal, an important ingredient in fragrances, was studied for its sensitizing potential in human skin. Fifteen human maximization tests were conducted with hydroxycitronellal obtained from four different sources at induction concentrations from 5 to 12%. No reactions were induced at 5% in two separate panels while 10% sensitized 2/25 panelists in one test but none in a second. Induction at 12% produced sensitization in 8 of 11 tests. A case of allergic contact dermatitis to mustard (Brassica nigra) in a salad maker is reported. The sources of skin contact included a commercial salad cream, a vinegrette, and various members of the mustard family, Cruciferae. The Cruciferae family includes many salad vegetables and condiments which contain allergenic isothiocyanates. The extent of skin contact among salad makers and other food handlers may be significant, and sensitization to isothiocyanates from this large family of vegetables may be an important cause of food contact allergy in these individuals.

6 patients are described who developed contact dermatitis after cereal contact on atopic skin for periods of 2 to 20 years. 2 patients were wheat flour patch-test-positive. Foods were tested by the scratch-chamber and open application techniques in 80 hand dermatitis patients, 55 of whom were food handlers. Both immediate and delayed reactions were seen. Most immediate scratch-chamber test reactions were provoked by vegetables and spices in patients with birch pollen allergy, and most delayed reactions from spices in patients with allergy to balsam of Peru, and from onion and leek. The evaluation of allergic and irritant reactions was difficult. Positive open application tests were seen in about 75% of patients with immediate or delayed scratch-chamber reactions.

Of 557 patients with occupational dermatoses seen at the Middle Road Hospital, Singapore, 55.7% (310) were irritant contact dermatitis, 38.6% (215) were allergic contact dermatitis and 5.7% (32) were non contact dermatitis. Bindii dermatitis is a distinctive eruption occurring in some Australian children during spring and early summer and characterized by discrete erythematous papules with puncta, concentrated over the palms, soles, and knees. In three affected individuals (but not in thirteen controls), pricking the skin with bindii seed produced an erythematous, indurated papule that appeared within 48 hours of intradermal pricking and simulated the primary lesion. Pathogenesis may involve an allergic reaction with a retained depot of antigen rather than a nonspecific foreign body injury reaction.

9 common spices were tested epicutaneously in 338 dermatological patients, 118 of whom were allergic to balsam of Peru. Positive reactions to one or more spices were seen in 50 patients all of whom except 2 were allergic to balsam of Peru.
