Clinically Probable Guillain-Barré Syndrome and Herpesvirus-Associated Syndromes in Advanced Untreated HIV: A Retrospective Case Cluster from India.

BackgroundGuillain-Barré syndrome (GBS) can be difficult to recognise in advanced untreated HIV, particularly where cerebrospinal fluid analysis and electrophysiology are unavailable.MethodsWe retrospectively reviewed adults with advanced HIV who were not receiving antiretroviral therapy and presented with acute symmetrical weakness at an Indian HIV specialty centre during 2019-2020. Cases were classified as clinically probable GBS with Brighton Collaboration level 3 diagnostic certainty.ResultsEight cases were identified among 139 admissions (5.8%). Mean CD4 count 78 cells/mm3. All eight had

Bridging the intention‒behavior gap in herpes zoster vaccination: A nationwide survey in a self-payment setting.

Herpes zoster (HZ) causes substantial morbidity, yet global uptake remains suboptimal where the vaccine is excluded from universal coverage. Taiwan exemplifies such a self-payment setting, with the HZ vaccine self-financed and local government subsidies restricted to limited eligible groups. This study analyzed data from a nationwide, web-based cross-sectional survey of adults aged ≥18y fielded in 2025, examining factors associated with HZ vaccination behavior, intention, and willingness to pay (WTP). Of 1690 respondents, 281 (16.6%) were vaccinated; among 1409 unvaccinated, 1140 (80.9%) were

Seroprevalence of varicella antibodies in unvaccinated children with nephrotic syndrome in disease remission.

Background and objectives Varicella-zoster (VZV) virus infection can be severe in children with nephrotic syndrome. Lack of varicella vaccination through the national immunisation programme in India leaves many vulnerable to infection. We conducted this study to document the seroprevalence of Varicella antibodies in patients with nephrotic syndrome. Methods This cross-sectional study (Oct 2024-May 2025) was conducted at a tertiary-care teaching hospital in India, in children (1-18 y) with nephrotic syndrome (both steroid-sensitive and steroid-resistant) in disease remission. Serum VZV IgG anti

Varicella zoster virus meningitis with a dermatomal rash in immunocompetent hosts: a systematic review and novel case report.

Varicella zoster virus (VZV) reactivation with central nervous system (CNS) involvement is an uncommon but important cause of lymphocytic meningitis in both immunocompromised and immunocompetent individuals. While cranial presentations, especially V1 involvement, are well documented, spinal-dermatomal VZV meningitis is rarely reported. We conducted a systematic review of PubMed, Scopus, and Embase using PRISMA guidelines to identify all cases of laboratory-confirmed VZV meningitis or encephalitis with a dermatomal rash in immunocompetent adults affecting spinal levels C1 and below. Data extrac

An Active Cytomegalovirus Infection and Epstein-Barr Virus Reactivation Complicated by Cytomegalovirus Meningitis and Virus-Associated Hemophagocytic Lymphohistiocytosis during Methotrexate and Upadacitinib Therapy in a Patient with Rheumatoid Arthritis.

Methotrexate (MTX) and Janus kinase (JAK) inhibitors are effective treatments for rheumatoid arthritis (RA); however, they may increase the risk of opportunistic infections. We herein report a case of a patient with RA receiving MTX and upadacitinib who developed an active cytomegalovirus (CMV) infection and Epstein-Barr virus (EBV) reactivation, complicated by CMV meningitis and virus-associated hemophagocytic lymphohistiocytosis (HLH). This case highlights that clinicians should consider not only herpes zoster, but also infection or reactivation involving non-VZV herpesviruses, including CMV

Prospective controlled study of the recombinant zoster vaccine in autoimmune rheumatic disease: robust seroconversion despite intravenous cyclophosphamide.

BACKGROUND: Recombinant zoster vaccine (RZV) prevents shingles in autoimmune rheumatic diseases (ARD), but intravenous cyclophosphamide (IVCYC) impact on immunogenicity is unknown. Given cell depletion/cytotoxicity, IVCYC is expected to induce profound immunogenicity impairment, we therefore evaluated its impact. METHODS: This subanalysis of prospective, randomized, placebo-controlled trial included ARD patients receiving IVCYC (ARD-IVCYC) compared with patients without IVCYC (ARD-no IVCYC) and healthy-controls. RZV was administered in two doses. Humoral response (≥4-fold), geometric mean tit

Long-Term Safety and Efficacy of Upadacitinib in Patients with Psoriatic Arthritis: 5-Year Results from the Phase 3 SELECT-PsA 1 Study.

INTRODUCTION: In the phase 3 SELECT-PsA 1 study, treatment with upadacitinib (UPA), an oral Janus kinase (JAK) inhibitor, improved signs and symptoms of psoriatic arthritis (PsA) from weeks 12 through 152. We now report the long-term extension phase, evaluating safety and efficacy of upadacitinib versus adalimumab (ADA) up to week 260 (~ 5 years). METHODS: Data on treatment-emergent adverse events and all primary and secondary endpoints were collected through week 260 for patients who received a placebo or ≥ 1 dose of UPA at either 15 mg (UPA15) or 30 mg (UPA30) daily, or ADA 40 mg every othe

Climate change and vertically transmissible infections in maternal and neonatal populations: a systematic review.

BACKGROUND: Climate change influences infectious disease dynamics and disrupts maternal and neonatal health services, both of which are pathways through which the risk of vertically transmissible infections may increase. METHODS: We systematically reviewed evidence on the association between climate-related environmental exposures and infection-related outcomes relevant to vertical transmission in pregnant or intrapartum women, neonates, and infants. Screening, extraction, and risk-of-bias assessment were performed independently in duplicate. Owing to substantial heterogeneity, findings were

Cocirculation of Human Monkeypox Virus Clade 1b with Varicella Zoster Virus, Uganda.

In July 2024, we reported information on the initial human mpox cases in Uganda. Genomic characterization of clinically suspected cases (July-December 2024) revealed cocirculation of several monkeypox virus clade 1b clusters and varicella zoster virus. Our findings highlight the need to refine differential diagnoses by molecular techniques and strengthen surveillance.

The epidemiological impact and effectiveness of herpes zoster vaccination among older and pharmacologically immunocompromised adults: a population-based observational study.

BACKGROUND: Australia used the live attenuated herpes zoster vaccine (ZVL, Zostavax®) for 70-79 year-old adults in its National Immunisation Program from 2016. In November 2023 this was replaced with the recombinant adjuvanted herpes zoster vaccine (RZV, Shingrix) and eligibility expanded to include adults ≥65 years old and immunocompromised adults ≥18 years. We aimed to assess the vaccine program impact and RZV effectiveness in eligible adults. METHODS: Using linked national immunisation registry and zoster-specific antiviral prescription data as a proxy outcome, patterns of antiviral use fr

Painless Thoracic Herpes Zoster Eruption in an 88-Year-Old Woman With Advanced Chronic Kidney Disease: A Case Report.

Herpes zoster (HZ) typically presents with a unilateral vesicular eruption accompanied by neuropathic pain. Although pain is a characteristic feature, painless presentations may occur and can create a diagnostic challenge. We report the case of an 88-year-old woman with advanced chronic kidney disease (CKD) who developed a unilateral vesicular eruption involving the right thoracic dermatome without pain at any stage of the disease. She was not receiving immunosuppressive therapy and had no known diabetes mellitus, previous HZ, or neurological disorder. The diagnosis was made clinically based o

Varicella zoster virus reinfection in a patient with refractory nephrotic syndrome: a case report.

Permanent immunity to varicella zoster virus (VZV) develops following infection in most patients, with reinfections extremely rare. Here, we report a 6-year-old boy with refractory nephrotic syndrome who contracted VZV infection during B-cell depletion after rituximab treatment. Although developing febrile neutropenia, he was promptly treated with acyclovir, which was successful. The VZV immunoglobulin G antibody titer was weakly positive (3.3) by enzyme immunoassay performed 42 days after the initial diagnosis of VZV infection, confirming the presence of VZV, although antibody production was

Exploring the role of infectious pathogens in Alzheimer's disease neuroinflammation.

Alzheimer's disease (AD) is the most common neurodegenerative disorder in older adults, characterized by progressive cognitive decline that ultimately leads to severe dementia. The primary pathophysiological hallmarks of AD include the accumulation of amyloid-β plaques, neurofibrillary tangles composed of hyperphosphorylated tau protein, synaptic dysfunction, and pronounced neuroinflammation. Although the exact relationship between neuroinflammation and AD pathogenesis remains incompletely understood, neuroinflammation is consistently recognized as an early and sustained feature of the disease

Adult vaccination in people living with HIV attending a tertiary centre infectious diseases clinic.

BACKGROUND: People living with HIV (PLWH) are at increased risk of acquiring vaccine-preventable diseases and of developing severe illness because of their underlying immunodeficiency. Despite this risk, vaccination coverage remains low among these patients. International and local guidelines exist to address immunisation recommendations in PLWH, but evidence on adherence, knowledge, and attitudes toward vaccination in this population remain limited. OBJECTIVES: The aim of the study was to investigate vaccination uptake, knowledge, and acceptance among PLWH. METHOD: Interviews and retrospect

Global burden of varicella and herpes zoster in women of childbearing age: insights from the global burden of disease study 1990-2021.

INTRODUCTION: Although the epidemiological characteristics of varicella and herpes zoster have been systematically studied, there is currently a lack of research on the disease burden and changing trends in women of childbearing age (WCBA) (15-49 years) with varicella and herpes zoster. METHODS: Based on data from the Global Burden of Disease (GBD) study, we investigated the prevalent cases, incident cases, and disability-adjusted life years (DALYs), prevalence rates, incidence rates, and DALY rates of varicella and herpes zoster among women of childbearing age across 204 countries and territ

Occurrence of Sensorineural Hearing Loss in Viral Meningitis and Encephalitis.

Hearing loss in viral neuroinfections is poorly investigated. The present study aimed to clarify if hearing loss can develop in viral meningitis and encephalitis. An OAE (oto-acoustic emission) was measured in adult patients with viral meningitis and encephalitis on the day of admission and 30-60 days after discharge. Frequencies were categorized as Low (1, 1.5, 2 kHz), Mid (3, 4, 5 kHz), Mid-high (6, 7, 8 kHz), and High (9, 10 kHz). An OAE was measured in 41 patients with viral meningitis and 8 patients with encephalitis. Six patients with viral meningitis (15%) experienced a decrease of > 5
