Bloodstream Infection in an Immunocompromised Host After Unusual Presentation of Photo Recall Dermatitis.

BACKGROUND Photo recall dermatitis (PRD) is a phototoxic eruption that occurs in areas of previous ultraviolet-damaged skin following systemic administration of a drug, most commonly chemotherapeutic medications. Although generally considered a self-limited dermatologic phenomenon, its implications in immunocompromised patients have not been well characterized. Secondary infection complicating PRD has not been previously established in the literature. This case is unique because the patient had an unusual rash while neutropenic, then developed a bloodstream infection (BSI), with the skin as he

Benzalkonium Chloride in Eye Drops Causing Allergic Contact Dermatitis: A Case Series.

We present three patients who developed allergic contact dermatitis (ACD) from benzalkonium chloride (BAK) in ophthalmic drops. BAK is a preservative commonly found in ophthalmic formulations and there is a rising trend of ACD to this allergen. The irritant and sensitising properties of BAK on the ocular mucosa will be discussed. This paper will also explore BAK allergy from eye drops in patients with atopic dermatitis and those using dupilumab.

Saving the Tiniest Heinies From Diaper Dermatitis in High-Risk Neonates.

In 2023, 53% of 300 patients in our NICU developed diaper dermatitis during their hospital stay. Literature indicates that providing effective skin care to patients can help prevent diaper dermatitis. The purpose of this project was to answer the following clinical question: Does application of Cavilon No Sting Barrier with reapplication every 3 days to high-risk newborns, either experiencing neonatal withdrawal due to maternal drug use or who were under observation for suspected infections and were noted to have blanchable redness on their buttocks on admission, prevent diaper dermatitis? Thi

Post-Occlusion Application of Skin Protective Creams Does Not Affect Occlusion-Induced Barrier Changes: A Randomized Controlled Experimental Study.

BACKGROUND: Glove occlusion increases skin susceptibility to irritants ('occlusion effect'). Previous experimental data showed that applying protective creams (PCs) before glove occlusion does not prevent this effect and may even aggravate irritant responses. OBJECTIVES: To assess whether the application of PCs after glove occlusion modifies the occlusion effect. METHODS: In a randomized, single-blind (participants were blinded) study, 68 healthy volunteers were examined over 10 days. Two commercially available PCs (A, B) and one standardized model cream (C) were applied after occlusion for

Fragrance in Dermatology: Revisiting Sensitization and Clinical Use.

Fragrance is widely used in consumer products and holds significant cultural and personal value, yet in dermatology it is most often approached as a risk factor due to its role as a leading cause of allergic contact dermatitis (ACD) and potential irritant or sensory trigger in susceptible individuals. As a result, avoidance or minimization of fragranced products is frequently recommended, particularly for patients with eczema, sensitive skin, xerosis, chronic pruritus, or suspected fragrance allergy. However, this paradigm may oversimplify sensitization risk and does not fully reflect advances

Yellow Palm Discoloration After Phenol Exposure.

Phenol exposure typically causes irritant contact dermatitis with yellow-brown discoloration. A worker developed characteristic discoloration followed by rapidly evolving distant eruptions consistent with an id reaction after phenol exposure. Symptoms resolved with conservative treatment and elimination of exposure. This rare presentation highlights the diagnostic value of yellow-brown discoloration and the importance of prompt occupational history-taking.

Clinically overt aortic aneurysm in giant cell arteritis: a nationwide hospital-based study.

BACKGROUND: Epidermal Langerhans cells (LCs) play a critical role in the induction of contact hypersensitivity. The LCs leave the skin, move to the regional lymph nodes and present the allergens embedded in the HLA-DR molecule to naive T-lymphocytes. To allow LC emigration from the epidermis, E-cadherin must be downregulated. In this study, we have examined the early events that occur in the human epidermis after exposure to three strong contact sensitizers and two commonly used fragrances by examining alterations of E-cadherin and HLA-DR expression. METHODS: To determine whether E-cadherin a

Development of itch biosensors with engineering membrane receptors that are coupled to field-effect transistors.

Biosensors inspired by biological sensory systems are valuable tools for detecting physiological and environmental stimuli with high degrees of specificity and sensitivity. An itch irritant biosensor to detect environmental changes or pruritogenic substances in human blood or tissues highly associated with inflammation and prevalent conditions like atopic dermatitis (AD) has not been developed. To address this gap, we developed a novel bioelectronic sensor by integrating the human itch receptor Mas-related G-protein-coupled receptor X2 (MRGPRX2) with a graphene field-effect transistor (GFET).

Processionary Moth Exposure in Europe: Toxic-Irritant and Immunoglobulin E-Mediated Endotypes, Diagnosis and Management.

Processionary moths (Thaumetopoea spp.), particularly the oak processionary moth (Thaumetopoea processionea), are an increasingly relevant cause of allergic and toxic disease in Europe, occurring in the context of reported range expansion, changing environmental conditions and persistent contamination with airborne urticating hairs (setae). Clinical manifestations range from toxic-irritant dermatitis and ocular injury to respiratory symptoms, while a subset of exposed individuals develops genuine IgE-mediated disease, including rhinoconjunctivitis, asthma, and rare cases of anaphylaxis. Granular parakeratosis associated with the use of laundry sanitizers containing benzalkonium chloride: a case report.

BACKGROUND: Granular parakeratosis (GP) is a rare benign inflammatory dermatosis that preferentially involves the axillae and other intertriginous, friction-prone skin sites and typically presents with pruritic erythema and scaling. Cases associated with benzalkonium chloride (BAC) exposure have rarely been systematically summarized, and further clinical evidence is needed. Herein, we report a case of generalized GP potentially associated with long-term exposure to a BAC-containing laundry sanitizer, with the aim of expanding the clinical spectrum of cutaneous adverse reactions associated with

Vulvar Dermatitis: Diagnosis and Management.

Candidiasis, herpes simplex virus, bacterial vaginosis, irritant contact dermatitis, allergic contact dermatitis, psoriasis, and eczema/lichen simplex chronicus are often misdiagnosed, mistreated, and underrecognized causes of vulvar discomfort. Vulvar discomfort is frequently encountered in dermatology, gynecology, urology, and primary practice. A lack of ownership among specialties often leads to significant knowledge gaps among these providers and delays in care. Clinical symptoms of vulvar dermatitis can overlap, posing a diagnostic challenge. Herein, we provide a tool to guide the diagnos
