Sensitive skin is a complex problem with genetic, individual, environmental, occupational, and ethnic implications. Further work is needed to rank irritants and irritant reactions to classify sensitive skin on the basis of standard models. The development of topical products designed for sensitive and ethnic skin may be advisable to optimize drug activity with minimal side effects. A total of 8149 patients were patch tested with the preservative Bronopol in 7 European contact clinics. The majority of patients (6507) were investigated in London. Reactivity was low, with a total of 10 irritant (0.12%) and 38 allergic reactions (0.47%). In only 17 cases (0.21%) was the patch test reaction to Bronopol considered to be of current or past clinical relevance. Concomitant sensitization to formaldehyde was present in about 1/3 of patients. Combination therapy with 5% minoxidil plus 0.5% anthralin was used to treat 51 patients with severe treatment-resistant alopecia areata. History of a cosmetically inadequate response to one or both drugs used as a single agent was present in 50 of the 51 patients. Hexylene glycol (HG), which has been used for years in industrial chemicals and cosmetics, has recently been introduced in topical corticosteroids. We studied the irritant and sensitizing properties of HG in eczema patients. HG at 50% or 30% and propylene glycol (PG) at 30% in water were patch tested in 823 eczema patients subjected to routine patch testing. Synopsis Of 1620 patients routinely patch tested with Kathon CG 100 p.p.m. aqua for suspected allergic contact dermatitis, 81 (5.0%) had a positive reaction. Sixty-two reactions (3.8%) were relevant for the dermatitis for which the patient consulted the dermatologist. Most patients were females who presented with dermatitis of the hands and/or face. Forty-six per cent of the patients had become sensitized by using cosmetic products on healthy skin. The other 54% had pre-existing dermatitis, especially atopic dermatitis and irritant dermatitis. Most reactions were caused by moisturizing creams.

In a prospective study, we have examined the incidence of fragrance sensitivity in Nottinghamshire coal miners. Our results confirm previous reports of an increased incidence of such sensitivity in miners (45%) when compared with both male (20%) and female (13%) non-miners. This increased incidence is not related to an increased use of perfumed cosmetics, but may be related to the use of a highly perfumed body lotion in subjects who already have a high incidence of irritant hand eczema. There was no significant increase in the rate of positive reactions to other applied allergens.

Kathon CG (K-CG) containing as active ingredients 2-methyl-4-isothiazolin-3-one and its 5-chloro analogue, is a very effective and widely used preservative system for cosmetics and toiletries.
