[Caustic diaper dermatitis and encephalitis secondary to the application of talcum contaminated with hexachlorophene].

Five observations of infants with transcutaneous intoxication by 6,3 p. 100 hexachlorophene contaminated talcum powder are reported. The diaper dermatitis is particular because of its topography (red pants shape), of its sudden occurring, of its papyraceous aspect evoking caustic origin, and of its association with severe encephalopathy. The neurological signs start with epileptic fits leading rapidly to coma. Prognosis is serious leading either to death or to paraplegia. Enquiry on that epidemic shows that mortality raised up to 18 p. 100. The neurological signs with oedematous degeneration o

24 and 48 h allergen exposure in patch testing. Comparative study with 11 common contact allergens and NiCl2.

Patch test reactions to 11 common contact allergens were studied after 24 h and 48 h occlusion with Finn Chambers in 390 patients. Concordant allergic results were found in 96 cases (74%). In 22 patients (17%), the reaction was positive only after 48 h and in 11 cases (8.5%) only after 24 h exposure. Most of the discordant reactions were to nickel, cobalt, neomycin, formaldehyde and perfume mix. Irritant reactions were found in 55 cases, the majority occurring after 48 h occlusion. Nickel chloride tested in parallel with 48 h exposure lead to more positive allergic and toxic reactions than nic

[Recommendations for local therapy of common dermatoses in children].

Recommendations for topical dermatologic therapy of the most common skin diseases in infancy and childhood are given with tabular summaries. They are concerned with diaper dermatitis, seborrhoeic dermatitis, atopic dermatitis, scabies and pediculosis. Special attention is given to topical steroid therapy in seborrhoeic and atopic dermatitis and to the possible neurotoxic side-effects of Lindan, the most effective substance against scabies and pediculosis.

Corn starch, Candida albicans, and diaper rash.

The growth of Candida albicans experimentally inoculated onto the skin of human volunteers was not enhanced by the addition of either cornstarch powder or talcum powder. Sufficient nutrients for growth of C. albicans exist on human skin, provided sufficient moisture is present. Cornstarch and talc powders were shown to minimize frictional injury experimentally induced in human volunteers. These studies indicate that cornstarch and talc powders do not enhance the growth of yeasts on human skin and do provide protection against frictional injury.

Histological changes in rabbits after application of medicaments and cosmetic bases (III).

An experimental study is reported of the irritant potential of oil of purcellin , isopropyl palmitate, nonyl phenol, Modulan , Amerchol and Acetulan when massaged in fixed amounts into the rabbit's skin daily for 30 days. Skin biopsies provided data on the number of epithelial cell layers, cell counts of the superficial dermis, and the fibre and basement membrane changes. All the test compounds induced changes in the form of increased number of epithelial cell layers and of the papillary dermis cell count, more marked for Acetulan or Amerchol . The difficulties in the evaluation of the results

Contact allergy to oleamidopropyl dimethylamine.

Contact allergy to the cationic emulsifier oleamidopropyl dimethylamine was demonstrated in 3 patients. In every case the emulsifier was present in a particular brand of body lotion. Patch test concentrations of 0.1% and 0.5% in water are proposed; slightly higher concentrations may induce irritant responses. Although these are the first documented cases of contact allergy to oleamidopropyl dimethylamine, it is argued that hypersensitivity to this compound may not be rare.

Irritant contact dermatitis to glyceryl monothioglycolate.

Glyceryl monothioglycolate has recently been implicated as an allergen. A case of a hairdresser who used this material is presented. He appeared to have an irritant contact dermatitis on his fingertips. Patch testing the patient and extensive patch testing in a group of fifty-four volunteers showed a very high incidence of irritant positive patch tests. No instance of allergy could be demonstrated in the reading of the patch tests or in biopsies of several individuals. Differentiation between allergens and irritants is difficult at times, but is necessary to avoid mislabeling of chemicals.

Allergic contact dermatitis to 2-bromo-2-nitropropane-1,3-diol in a hydrophilic ointment.

Seven patients are described who developed acute allergic contact dermatitis after using Eucerin cream on previously dermatitic skin for periods of time varying from 5 weeks to 2 years. Eucerin was preserved with 2-bromo-2-nitropropane-1,3-diol (BNPD) in 1978 to assist in controlling a problem with Pseudomonas aeruginosa contamination. All of our patients were BNPD and Eucerin patch test-positive. None of them was allergic to formaldehyde or to any other preservative known to be a formaldehyde donor. This was in contrast to other BNPD and other formaldehyde-releaser--sensitive patients we saw

Allergic contact dermatitis in two machinists.

Two machinists were seen whose chronic hand dermatitis was explained by allergic sensitivity to a biocide, Tris Nitro (2-[hydroxymethyl]-2-nitro-1,3-propanediol), which was added to their metalworking fluids (cutting oils) to prevent rancidity. The cause of the men's difficulty was first suspected when they had positive patch test reactions to a related biocide, Bronopol (2-bromo-2-nitropropane-1,3-diol), which we added to our standard screening tray because of its presence in more than 500 cosmetic formulations. The detective skills of the patients uncovered the chemical, Tris Nitro, related

[Skin irritation caused by propylene glycols].

Propylene glycol is used in foods, medicine and cosmetics because it is a good solvent which has, simultaneously, moisture-regulating, antiseptic, and preservative effects. Propylene glycol may produce eczematous skin reactions of toxic and, more rarely, of allergic nature. Positive patch test reactions to propylene glycol are difficult to interpret. Allergic reactions may be confirmed by a clear clinical relevance, repeated local skin provocation (usage test), or oral provocation. In the Department of Dermatology, University of Oregon, 84 patients were patch tested with 100% propylene glycol.

Metabolic alkalosis secondary to baking soda treatment of a diaper rash.

A 4-month-old infant was seen with hypokalemic metabolic alkalosis that was associated with prior application of liberal amounts of sodium bicarbonate (baking soda) to a diaper rash. After exclusion of other etiologies of the infant's acid-base disturbance, a complete resolution occurred following discontinuation of the baking soda applications. This case report provides a reminder of the significant side effects that may result from the excessive use of a seemingly harmless household substance.

Contact allergy from propylene glycol.

Propylene glycol (PG) 20% in water was tested in a group of 400 subjects with eczematous contact dermatitis. In six inpatients (1.5%) reactions were recorded as considered positive according to the following criteria: 1. the relevant history of prolonged contact with PG-containing topical pharmaceutical and cosmetic preparations; 2. the reaction extended in time outside the area of application of the test substance, and persisted locally for several day; 3. a similar reaction was obtained on repetition of the test with a lower concentration of PG; 4. the prompt improvement in clinical manifest

Histological changes in rabbits after application of medicaments and cosmetic bases. II.

A further study of the topical effects of certain pharmaceutical and cosmetic bases on rabbit skin is reported. Cetyl alcohol, myristic acid, castor oil and sorbitol were applied in fixed doses daily for 30 days and their irritant activity was assessed. The macroscopic and microscopic changes were on the whole minimal. Castor oil alone produced some macroscopic alterations in the form of slight erythema and edema, and microscopic changes consisting of acanthosis, disorganization of the basal layer and slight infiltration of the dermis.

Benzoyl peroxide.

The mode of action of benzoyl peroxide in acne is three-fold, i.e. sebostatic, comedolytic and inhibitory to P. acnes in-vivo. Benzoyl peroxide is the topical treatment of choice in acne vulgaris. This agent is well tolerated by most patients. Primary irritant dermatitis can be avoided by less frequent application and the true incidence of contact sensitivity is low. The gel preparation has achieved a high degree of cosmetic acceptability. A synergistic effect with retinoic acid can be demonstrated. Tolerance to benzoyl peroxide develops in most subjects necessitating more vigorous therapy, us

Hand dermatitis in haridressers.

Most junior hairdressers, while shampooing, develop a dry irritant dermatitis over the metacarpophalangeal joints. It responds to simple treatment and disappears when the shampooing stage is passed. A few hairdressers develop eczema of the fingers: the response to treatment is less predictable and the prognosis is worse. In those who give up hairdressing the eczema generally improves or heals, in those who continue hairdressing the eczema is likely to persist, especially in atopics.

Temperature dependent primary irritant dermatitis from lemon perfume.

In a recent outbreak of hand eczema amongst cleaning personnel after the introduction of a new, lemon-scented detergent, it was noted that the patients complained of a burning, stinging sensation when their hands were submerged in hot detergent solutions. Since routine patch testing with the Standard Series and perfume components was of no help in pinpointing the responsible agent, testing with selected perfume components was repeated at higher temperatures. Identical tests were placed on both forearms for 20 min, one arm being exposed to 43 degrees C, the other to 23-25 degrees C. Little or n

[The irritant and allergenic action of two isocyanates: toluene diisocyanate (TDI) and diphenylmethane diisocyanate (MDI)].

This is a study on the irritant capacity of two isocyanates (T. D. I. Toluene diisocyanate and M. D. I. diphenyl methane diisocyanate). The experiment was carried out on the hide and ocular mucous membrane of albino rabbits, using Draize method as well as the method for appraising cosmetics. An histopathological examination of the evolution of the lesions was then achieved together with a study on the delayed allergenic capacity. This study was carried out on albino guinea-pig hides using Kligman and Magnusson technique. In these experimental conditions, the conclusions were the following:--on
