Clinical and Somatosensory Correlates of Persistent Zoster-Associated Pain at One Month: A Prospective Multicenter Cohort Study.

Background/Objectives: Persistent pain after acute herpes zoster (HZ) carries substantial functional burden, but early markers of pain persistence remain uncertain. In an exploratory analysis, we examined whether baseline clinical features and a simple affected-to-contralateral pressure-pain threshold (PPT) ratio were associated with persistent zoster-associated pain approximately one month after rash onset. Methods: In this prospective multicenter observational cohort, 117 adults with acute HZ (mean age 58.7 ± 14.7 years, 60.7% women, 18.8% with diabetes mellitus, 16.2% immunosuppressed, 58.1

Varicella zoster vasculopathy presenting as bilateral subdural haematoma in an infant: case report of a rare case and literature review.

BACKGROUND: Varicella-zoster virus (VZV) infection is typically benign in infants but may rarely cause neurological complications. The occurrence of chronic subdural haematoma following VZV infection in children has not previously been reported. CASE DESCRIPTION: We report the case of a 6-month-old male infant who developed chicken pox with fever, irritability, and generalized vesicular rash. His rash and fever settled over 2 weeks, but he had persistent irritability, decreased feeding, intermittent vomiting, regression of developmental milestones, and developed progressive weight loss (50th

Acute cerebellitis as a post-infectious complication of primary varicella in an immunocompetent young adult: A case report.

BACKGROUND: Acute cerebellitis is a well-documented neurological sequela of primary varicella-zoster virus (VZV) infection. Although frequently reported in children, this complication remains uncommon in adults and may present diagnostic difficulties, especially when neuroimaging studies are normal. CASE PRESENTATION: A previously healthy, immunocompetent 20-year-old man was admitted with a four-day history of progressive gait instability and imbalance. Nine days prior to admission, he had developed a generalized vesicular rash consistent with primary varicella, initially involving the trunk

Psoas-lumbar vertebral index and short-term pain relief after pulsed radiofrequency treatment of the dorsal root ganglion in postherpetic neuralgia in Korea: a retrospective pilot study.

PURPOSE: The psoas-lumbar vertebral index (PLVI), a marker of central sarcopenia measured using computed tomography (CT) or magnetic resonance imaging (MRI), has been linked to the development of postherpetic neuralgia (PHN). Whether PLVI predicts short-term pain relief after pulsed radiofrequency (pRF) treatment of the dorsal root ganglion (DRG) in established PHN remains unknown. This pilot study examined the association between PLVI and the reduction in numeric rating scale (NRS) pain scores 1 month after pRF. METHODS: Patients with PHN who underwent DRG pRF and had CT or MRI scans availab

Exploring spillovers of vaccines: A practical application of the Expanded Impact Inventory.

Including all relevant costs and effects in economic evaluations can be challenging, especially when spillovers in other individuals or sectors are likely. Previous literature has highlighted that the combination of relevant spillovers is evaluation and context specific. This emphasises the importance of identifying (the evidence on) relevant spillovers systematically. Spillovers are defined here as "all impacts from an intervention on all parties or entities other than the users of the intervention under evaluation". In this study, we evaluate the feasibility of applying the recently introduc

FDG Uptake Extending From the Dorsal Root Ganglion to the Sciatic Nerve After Herpes Zoster.

A woman in her 50s receiving paclitaxel, carboplatin, and pembrolizumab for recurrent cervical cancer developed herpes zoster and received antiviral therapy. 18F-FDG PET/CT performed 22 days after zoster onset, when the skin lesions had crusted, demonstrated continuous uptake extending from the left L5 dorsal root ganglion to the sciatic nerve. To our knowledge, this is the first reported case of FDG PET findings indicating ganglioneuritis extending from the dorsal root ganglion to a peripheral nerve after herpes zoster. Recognition of this pattern is important in patients receiving chemoimmun

Pruritus-dominant herpes zoster with concurrent contralateral upper limb pain in a patient with complete spinal cord injury: A case report.

CONTEXT: Sensory impairment after spinal cord injury (SCI) may complicate herpes zoster (HZ) recognition. FINDINGS: A 76-year-old man with complete T4* paraplegia developed pruritus and vesicles in the right T4 dermatome without eruption-site pain. The asterisk denotes adjustment for stroke-related left upper-limb weakness. Empiric intravenous acyclovir preceded dermatology assessment. Contralateral shoulder and hand pain developed the day after eruption and was absent on the final treatment day. CONCLUSION/CLINICAL RELEVANCE: Focal pruritus warrants skin examination. When HZ is strongly sus

Supervised Reduction Protocol for Management of Herpetic Stromal Keratitis.

PURPOSE: Herpetic stromal keratitis (SK) caused by herpes simplex virus (HSV) or varicella zoster virus (VZV) can cause sight-threatening corneal scarring. Given high recurrence rates of herpetic SK with steroid reduction and limited consensus on treatment duration, we examine outcomes of an approach we term the supervised reduction protocol (SRP). METHODS: Among 162 patients with ocular HSV or VZV evaluated between September 2016 and June 2025, 53 had SK managed with SRP. Under the SRP, patients underwent frequent slit-lamp evaluation allowing for early detection and management of stromal in

Gut microbial and metabolomic alterations associated with herpetic trigeminal neuralgia: an exploratory multi-omics study.

BACKGROUND: Herpetic trigeminal neuralgia (HTN) is a neuropathic pain disorder caused by varicella-zoster virus invasion of the trigeminal nerve, but its mechanisms remain poorly elucidated. Accumulating evidence implicates gut microbiota dysbiosis and metabolic perturbations in modulating chronic pain via the gut-brain axis, yet their roles in HTN have not been systematically explored. METHODS: Integrated multi-omics analysis was performed on 19 patients with HTN and 18 healthy controls. Fecal metagenomic sequencing characterized gut microbial composition and function. Untargeted metabolomic

Case Report: Fatal varicella-zoster virus meningoencephalitis with Froin-like cerebrospinal fluid and hydrocephalus following pembrolizumab-containing chemoimmunotherapy.

BACKGROUND: In patients receiving immune checkpoint inhibitors (ICIs), central nervous system (CNS) infections may mimic or coexist with neurological immune-related adverse events (irAEs). ICI-driven immune dysregulation has been proposed to amplify inflammation against latent pathogens ("immunotherapy infections due to dysregulated immunity", ITI-DI), but this remains a hypothesis-generating framework rather than a validated diagnosis. CASE PRESENTATION: A 54-year-old man with stage IIIC lung adenocarcinoma developed a herpes zoster eruption and lumbosacral pain 7 days after the first infusi

Altered Regional Spontaneous Brain Activity and Default Mode Network Functional Connectivity in Herpes Zoster-Related Pain Patients with High Anxiety.

Pain and psychological disorders are two common symptom clusters in patients with herpes zoster-related pain (HZ-RP). We utilized functional magnetic resonance imaging (fMRI) to investigate functional characteristics of the brain in HZ-RP patients with different levels of anxiety. Twenty-five HZ-RP patients with high anxiety (HZ-RP with HA) and 26 HZ-RP patients with low anxiety (HZ-RP with LA) were enrolled. Fractional amplitude of low-frequency fluctuations (fALFF), regional homogeneity (ReHo) and functional connectivity (FC) based on the brain regions with significant differences and defaul

Upadacitinib in Colonic Crohn's Disease: A Narrative Review of Mechanistic Rationale, Clinical Trial Evidence, and Real-World Outcomes.

Crohn's disease (CD) is a chronic inflammatory bowel disorder with colonic involvement in a substantial proportion of patients, often associated with increased symptom burden and therapeutic complexity. Upadacitinib, a selective Janus kinase 1 (JAK1) inhibitor, has emerged as an effective oral therapy for moderate-to-severe CD, including patients with colonic-predominant disease. This narrative review synthesizes evidence from phase 2 and 3 randomized controlled trials, post-hoc subgroup analyses, long-term extension studies, and real-world observational cohorts evaluating the efficacy and saf

Exploring the Pathogenesis and Treatment Strategies of Post-Herpetic Neuralgia Based on the "Gut-Immune-Neural Axis": A Narrative Review.

Postherpetic neuralgia (PHN) is a prevalent and refractory neuropathic pain condition that arises after herpes zoster infection, characterized by complex pathophysiological mechanisms and limited efficacy of conventional treatments. Recent advances in the study of the gut-brain axis have expanded to encompass pain modulation, leading to the integrated concept of the "gut-immune-neural axis". This narrative review examines the proposed mechanisms by which gut microbiota may contribute to the onset and progression of PHN by modulating immune homeostasis, neuroinflammation, and neural plasticity.
