Managing concurrent nephrotic syndrome, spontaneous bacterial peritonitis, and herpes zoster in an immunocompromised child: Diagnostic difficulties and treatment outcomes.

Nephrotic syndrome (NS) is a disease characterized by heavy protein loss in the urine due to increased permeability of the glomerular membrane, leading to low levels of albumin in the blood. Several studies have shown that primary or secondary kidney disease, congenital infections, diabetes, lupus, cancer, or the use of certain medications can cause it. Therefore, this study aims to present a case of NS in a 9-year-old boy complicated by spontaneous bacterial peritonitis and herpes zoster. The patient presented to the emergency department with a 2-day history of worsening abdominal pain accomp

Glial Contributions to Neuropathic Pain: Central and Peripheral Strategies in Postherpetic Neuralgia Pathogenesis and Therapeutics.

Neuropathic pain (NP), particularly postherpetic neuralgia (PHN), involves intricate mechanisms that extend far beyond neuronal dysfunction. In recent years, glial cell-mediated neuro-immune-inflammatory circuits mediated by glial cells (including spinal microglia, astrocytes, and dorsal root ganglion satellite glia) have been regarded as one of the key pathological mechanisms for the occurrence and maintenance of NP. Glial cells exacerbate pain pathogenesis by driving neuroinflammation, amplifying nociceptive signaling, and modulating ion channels and pain-related signaling molecules. A syste

Comparison of ultrasound-guided transforaminal epidural injection versus fluoroscopy-guided injection for lumbosacral herpes zoster: a noninferior analysis.

BACKGROUND: To evaluate efficacy and safety of ultrasound (US)-guided lumbar transforaminal epidural injection (TFEI) for relieving zoster-associated pain (ZAP) in lumbosacral nerves. METHODS: A retrospective observational cohort study was performed with 1:1 propensity matching to control cohort. Patients undergoing TFEI were included as a US cohort using US guidance and control cohort under fluoroscopy (FL) guidance. Primary endpoint was visual analog scale (VAS) pain scores at 1 month after the procedure with a non-inferiority margin of 10 mm. Secondary outcomes included sensory blockade, p

Ultra-High-Voltage Pulsed Radiofrequency Combined With Escitalopram for Refractory Postherpetic Itch in the Trigeminal Nerve.

Postherpetic itch (PHI) is a common complication of herpes zoster, predominantly affecting the head and face, and significantly impacting patients' quality of life. Few studies have addressed the concerns of PHI, and its available treatments have shown limited efficacy. We report a case of PHI in the trigeminal nerve in which a novel combination of ultra-high-voltage pulsed radiofrequency and escitalopram achieved significant relief of itch. This combined approach may provide a safe and effective option for intractable PHI.

Wolf's isotopic response of eczema after herpes zoster infection: case report and literature review.

Wolf's post-herpetic isotopic response (PHIR) refers to the development of new cutaneous conditions, such as tumors, infections, or immune-mediated diseases, at the site of a previously healed herpetic eruption. The exact pathogenesis of Wolf's PHIR remains incompletely understood. Four hypotheses have been proposed regarding the mechanism of postherpetic inflammatory isomorphic reactions: viral, immunological, vascular, and neurological. In this case, we show that a patient with eczema developed it at the original site following the resolution of the herpes zoster infection. After 2 weeks of

Extraintestinal Cutaneous Manifestations in Inflammatory Bowel Disease Among Non-White Patients: A Retrospective Multicenter Study.

BACKGROUND: The presence of extraintestinal manifestations with cutaneous diseases in patients with inflammatory bowel disease (IBD) can pose significant complications. Though the prevalence of IBD has been increasing in racial and ethnic minority groups, most literature has characterized several cutaneous manifestations (CM) of IBD in patients of white skin, with a lack of studies describing these complications in patients with other skin tones. METHODS: Our study aimed to determine the rates of various CM of IBD in skin of color using a health care database to identify white and non-white p

Herpes zoster hospitalization burden in Spain during the initial period of recombinant zoster vaccine implementation.

Herpes zoster (HZ) is a vaccine-preventable disease with increasing incidence and hospitalization burden, particularly among older adults and immunocompromised individuals, who have an increased risk. In 2021, Spain introduced systematic vaccination with the recombinant zoster vaccine (RZV). We conducted a retrospective, descriptive study using hospital discharge data from the Spanish Minimum Basic DataSet (MBDS) for the years 2022-2023. Hospitalization rates (HR), mortality rates (MR), case fatality rates (CFR), length of stay, comorbidities, and costs were analyzed nationally and for the reg

Herpes zoster vaccination and incident dementia in Canada: an analysis of natural experiments.

BACKGROUND: Two natural experiment studies have found evidence that live attenuated herpes zoster vaccination prevents or delays dementia onset. We aimed to determine the effect of live attenuated herpes zoster vaccination on incident dementia diagnoses among people aged 70 years and older using a natural experiment in Ontario, Canada, and to triangulate these findings, using a second natural experiment in Ontario and a quasi-experimental approach that uses data from multiple Canadian provinces. METHODS: Our analysis of natural experiments included people born in Canada between Jan 1, 1930, a

Epidemiology and risk factors of postherpetic neuralgia worldwide: protocol for a systematic review and meta-analysis.

INTRODUCTION: Postherpetic neuralgia (PHN) is a debilitating complication of herpes zoster (HZ) that significantly impairs quality of life, disrupting sleep, daily activities and work capacity. Globally, PHN represents a major public health challenge, with marked heterogeneity in its epidemiological patterns across different regions and demographic groups. The escalating incidence of both HZ and PHN underscores the urgent need to elucidate modifiable and non-modifiable risk factors, which is critical for implementing targeted prevention strategies and optimising therapeutic interventions. Central effects of short-term spinal cord stimulation in postherpetic neuralgia: a longitudinal fMRI and DTI study.

OBJECTIVE: Postherpetic neuralgia (PHN), a refractory neuropathic pain following herpes zoster reactivation, lacks clear central mechanisms for emerging therapies like short-term spinal cord stimulation (stSCS). This longitudinal study used multimodal neuroimaging to examine the effects of 14-day stSCS on brain function and white matter microstructure in PHN patients, and to identify neural correlates of clinical improvements. METHODS: In this longitudinal, single-arm, pre-post study, 17 PHN patients received 14 days of continuous stSCS. Clinical outcomes including pain intensity (Numeric Rat

Neurotoxicity from acyclovir and valacyclovir in patients undergoing peritoneal dialysis: Still rare, but still present.

Acyclovir and valacyclovir have been widely used for the treatment of herpes virus infections, including herpes zoster, with generally favorable outcomes. However, in patients with chronic kidney disease, particularly those undergoing peritoneal dialysis or hemodialysis treatment, neurotoxicity can be a significant complication as renal excretion is the primary elimination pathway. Here, we report three cases of automated peritoneal dialysis patients treated for herpes zoster who developed neurotoxicity following acyclovir or valacyclovir administration. Symptoms appeared 2 to 4 days after tre

[Prevention of herpes zoster infection in patients with chronic diseases: a necessity due to its sociosanitary impact].

Herpes zoster (HZ) is a common disease with a considerable burden on health care systems. Its prevalence is higher in people ≥ 50 years old, and even higher in people with chronic diseases such as diabetes mellitus, COPD, asthma or cardiovascular disease. The pain associated with HZ can be disabling and persistent and, as with other complications, can negatively affect patients' quality of life. Vaccination is the only strategy capable of preventing the onset of HZ and the development of its complications. The adjuvanted recombinant zoster vaccine (RZV) has proved to be effective in several po
