[The significance of dust in the evaluation of the potential health risk in chemical industry plants].

According to statistics, occupational diseases (and respiratory disorders in particular) are rare. Occupational disease accounted for only 1.5% of all job-related disabilities registered with the Swiss Accident Insurance Institute (SUVA) in 1984. Skin disorders (37.1%) are the leading form of occupational disease, followed by disturbances of the locomotor apparatus (36%) arising from physical causes. Pneumoconiosis (quartz, asbestos) accounts for 2.8% of cases. Chronic disorders of the respiratory tract brought about by irritant or toxic gases are buried away under the heading of 'chronic pois

Occupational skin disease in Singapore: epidemiology & causative agents.

Of 557 patients with occupational dermatoses seen at the Middle Road Hospital, Singapore, 55.7% (310) were irritant contact dermatitis, 38.6% (215) were allergic contact dermatitis and 5.7% (32) were non contact dermatitis. More than half of the patients were below 30 years old and most were from the construction (29.5%), metal/engineering (20.6%), electrical/electronic (16.4%), transport (5.5%) and food catering (3.8%) industries. Common occupational irritants included coolants, oils, cement, solvents, detergents and soldering flux. Chromates in cement was the most common occupational allerge

The effect of capsaicin on some experimental inflammations in human skin.

Topical application of capsaicin is thought to deplete substance P from local sensory nerve terminals. In experiments on human skin inflammation was induced by injection of substance P (SP) or histamine intradermally, UV irradiation, non-immunologic contact urticaria, tuberculin reaction, contact allergens and benzalkonium chloride with or without capsaicin pretreatment. The flare response to SP and histamine was suppressed by capsaicin pretreatment whereas the wheal was enlarged. Interestingly, capsaicin pretreatment enhanced the responses to all other inflammatory agents.

Jellyfish envenomation syndromes.

Jellyfish venoms are complex mixtures of polypeptides and enzymes pathogenic to man by means of their toxic or antigenic properties. With newer technology, more therapeutic agents will become available to treat the various envenomation syndromes caused by these animals. It will therefore be necessary to make an accurate diagnosis of the type of reaction the patient experiences, as well as to identify the offending species. Fatal reactions can be caused by hypersensitivity or induced by various toxins on the heart, respiratory center, or kidney. Cutaneous eruptions may be local or generalized,

[Occupational allergy to antibiotics].

Of 57 nurses suffering from dermatitis and having occupational contact with antibiotics 37 per cent exhibited positive tests with these drugs. Analogous figures for pharmaceutical industry workers were 34 subjects and 41 per cent and for veterinary surgeons 31 subjects and 29 per cent. A case of an ophthalmist sensitive to dethreomycin under occupational conditions was also recorded. Furthermore, six nurses exhibited inhalant allergy to penicillin manifested clinically by an anaphylactic shock or urticaria or, in two cases, by contact urticaria: one also due to penicillin and the other due to

Pharmacological studies on nonimmunologic contact urticaria in guinea pigs.

In the present study we examined the effects of chlorpheniramine and ranitidine, indomethacin, BW755C (an inhibitor of cyclo-oxygenase and lipo-oxygenase enzymes of arachidonic acid metabolism), dexamethasone, and capsaicin on nonimmunologic contact urticaria (NICU) induced in the guinea pig ear by benzoic, acid cinnamic acid, cinnamic aldehyde, methyl nicotinate, diethyl fumarate, or dimethyl sulfoxide. The intensity of edema in the urticarial reaction was quantified by measuring the ear thickness. Antihistamines inhibited reactions to intradermal histamine but not to agents causing NICU. Ind

Site variation in anthralin inflammation on forearm skin.

The effect of application site on anthralin inflammation was measured at 10 clinically normal volar skin sites on each forearm of 31 subjects as the increase in skin thickness at 48 h using Harpenden calipers. Pre-treatment and increase in skin thickness were significantly related to application site. Pre-treatment thickness increased distally and laterally by an amount depending on sex, and to a lesser extent other factors, whilst the increase in skin thickness values was greater proximally, laterally and on the right arm but was not affected by age, sex or skin type. When increases in skin t

Purity of standardized textile dye allergens: a thin layer chromatography study.

Twelve standard textile dyes were analyzed using thin layer chromatography and studied comparatively with dyes supposed to be chemically similar, having the same Colour Index number or Registration Code. According to the chromatograms, Disperse Yellow 3, Disperse Orange 1, Disperse Orange 3 and Disperse Orange 13 seemed, at least in the solvent system used, to be pure, while all the other dyes, in the same eluent system, seemed impure. All the standard Textile Dye allergens were similar to the dyes used in the clothing industry. Their use in patch tests may prove useful, without overlooking th

[Dental amalgams and allergy].

After a short description of the fundamental basis of allergy, particularly in relation to contact allergy, the authors review the 41 published cases of allergy to dental amalgam consisting of 30 female and 11 male patients. 20 of these patients recovered on removal of the dental amalgam. The most frequent symptoms were of the remote cutaneous type (38/41 cases) while local symptoms, particularly gingivitis and stomatitis, occurred in 17 cases. These probably underestimate the true prevalence of the condition for several reported cases have not been published. The authors then go on to describ

An approach for selection of health care personnel handwashing agents.

Given the wide range of available health care personnel handwashing agents, selection of an appropriate product may be difficult. This decision may be made on the basis of user preference, cost, or other factors unrelated to product effectiveness. Four criteria--efficacy, safety, cost, and acceptability--are appropriate for systematic evaluation of handwashing products. These criteria are applied to para-chloro-meta-xylenol (PCMX), a compound used with increasing frequency in health care personnel handwashing agents. Published data regarding the biochemical properties, efficacy, and safety of

The sensitizing capacity of coumarins (I).

10 coumarins used in perfumery, cosmetics and ointments, have been investigated by 2 different methods to determine their sensitizing capacity. Only 1 of the compounds, 4-methyl-esculetin, which serves as a sunscreen agent, was found to be a weak sensitizer. All other coumarins were negative. It is assumed that this dihydroxycoumarin becomes a sensitizer when it is enzymically oxidized to the corresponding orthoquinone in the skin.

[Remarks and recommendations on the definition and classification of eczematous diseases].

Numerous nomenclatures and classifications of eczemas have puzzled dermatologists for a long time. This confusion is due to the great number of external as well as internal noxious factors and/or agents causing eczemas being themselves a distinct and histologically well defined reaction of the papillary-epidermal compartment. Current classifications do not adequately reflect the complex causative conditions involved in the etiopathogenesis of eczematous disorders. I propose, therefore, to classify eczematous disorders according to the following three principal categories: "mainly endogenous" (

The effect of some immunomodulating agents on the sensitization phase of experimental contact allergic reactions.

In an experimental model which allows comparison of the macroscopic changes with the differentiated dermal inflammatory cell infiltrate of the allergic contact reaction to oxazolone in guinea pigs, the effects of single doses of the cytostatic agents cyclophosphamide, methotrexate and azathioprine and the anti-lymphocyte agent cyclosporin A administered prior to sensitization were studied. All the agents tested increased the degree of erythema and edema of reactions compared to controls. The dermal inflammatory mononuclear cell response showed little significant increase and would not seem to

Modulation of the population density of identifiable epidermal Langerhans cells associated with enhancement or suppression of cutaneous immune reactivity.

The epidermis on the backs or ears of DBA/2 mice treated for 7 days with a 20% concentration of monobenzyl ether of hydroquinone (MBEH) had a significantly greater population density of ATPase- and Ia-positive cells compared with control mice treated with diluent. There was no decrease or increase in ATPase- or Ia-positive cells at sites distal from the treated tissue. This increase in population density of Langerhans cells was associated with a significant increase in functional afferent immune reactivity measured by allergic contact hypersensitivity. We also found evidence for enhanced effer

Management of acute childhood poisonings caused by selected insecticides and herbicides.

Most childhood exposures to insecticides and herbicides do not result in poisonings. Decontamination and observation are usually adequate treatments. The most frequent exposures involve carbamate and organophosphate insecticides. These compounds inhibit acetylcholinesterase, resulting in cholinergic signs that are reversible with atropine administration. Recent reports from poison control centers indicate that organophosphates have been associated with most of the serious childhood poisonings. Pralidoxime, a cholinesterase reactivator, must be administered along with atropine to patients with

Contact sensitivity to proflavine.

Proflavine lotion is a commonly used topical antiseptic in the tropics, but its sensitizing potential was never emphasized and many who developed allergic contact dermatitis were never aware of it. In a study of 45 patients, most presented with acute or subacute dermatitis which started on the arms and legs. Concomitant cutaneous sensitivity to other medicaments and lanolin occurred in 66% of the patients.

Contact dermatitis caused by airborne agents. A review and case reports.

A general review is given of airborne-induced contact dermatoses, particularly of the irritant and allergenic types. Because the reports in the literature often omit the term airborne, 12 volumes of Contact Dermatitis (January 1975-July 1985) were screened, and the cases cited were classified in function of the anamnesis, lesion locations, causative irritants and allergens, and other factors. The present article also discusses differential diagnoses, in particular with regard to contact dermatitis of the face, ears, and neck. Finally, seven case reports of occupational and nonoccupational cont

Medicament contact allergy.

During a two-year period contact allergy to topically applied medicaments in the urban population of Malmö was investigated prospectively by patch testing patients with suspected products and their ingredients as well as with standard allergens. Contact dermatitis to specified medicaments was diagnosed in 40 of 408 patients with a positive patch test, and to standard allergens of medicament nature in a further 123 patients. Medicament contact allergy therefore constituted 40% of all allergies observed. This is equivalent to an annual incidence of 43/100,000. Among responsible products the lead

Contact dermatitis in tie and dye industry workers.

A survey of the 'Tie and Dye' industry of Jodhpur City in India was made to investigate occupational dermatoses. 49 (16.6%) of 250 workers had incapacitating dermatitis. Skin lesions were seen mostly over the dorsa of the hands and fingers. 26 patients were patch tested with various dyes and chemicals; 14 were positive. Fast Red RC salt was the most potent sensitizer. Other dyes showing positive reactions were Orange GC salt, Bordeaux GP salt, Blue B salt, Red B base and naphthol.
