We report on 161 patients suffering from inflammatory dermatoses on hands, forearms, and lower legs who had been initially treated with 0.1% difluocortolone valerate. During the maintenance therapy carried out over a period of 3 to 4 weeks, we tested the efficacy of Kamillosan cream vs. 0.25% hydrocortisone, 0.75% fluocortin butyl ester, and 5% bufexamac in a bilateral comparative study. A mouse model for assessment of the anti-inflammatory effect of topical glucocorticoids is described. Mice, sensitized to picryl chloride, had both ears painted with the sensitizer followed 2 hours later by the application of the topical steroid to one ear and the corresponding vehicle to the other. The contact sensitivity reaction, measured by swelling of the ear, was recorded 24 hours later. Dilutions of the steroid formulations inhibited the ear swelling in a manner related to dose-response. One hundred consecutive patients suspected to have contact dermatitis due to topical medicaments were patch tested with 20% neomycin, framycetin, gentamicin as well as other commercially available antibacterial agents. Neomycin sensitivity was observed in 16.0% cases, while framycetin and gentamicin sensitivity was seen in 11.0% and 4.2% respectively. Fifty percent neomycin hypersensitive cases cross - reacted with framycetin and 22.00/o cross reactivity was observed with getamicin.

Of 739 dermatitis patients, 4.1% showed a positive patch test reaction to benzoyl peroxide 1% in petrolatum. Forty-one patients with leg ulcers (patch test: negative to benzoyl peroxide) were treated with a 20% benzoyl peroxide emulsion for 4 weeks. After this therapy, the sensitization rate was as high as that for the maximization test, namely 76%. In most patients, the post-therapeutic allergy to benzoyl peroxide was accompanied by contact allergies to other therapeutic agents applied. Over the years, changes have had to be made in the list of topical drugs most frequently responsible for allergic contact dermatitis. The 4 most common sensitizers in Italy in the past have been penicillin, sulfonamide, promethazine and neomycin. Now the list is headed by neomycin, benzocaine and ethylenediamine. Seventy-three patients suffering from stasis dermatitis with or without leg ulcers (group A) and 565 patients with other eczemas (group B) were patch tested with a series of 37 vehicle constituents and topical preparations. Chronic or relapsing irritant eczema of the hands was the main complaint in 34 veterinary surgeons. 9 had contact allergic eczema of occupational character. In 8 cases, the allergies were probably due to antibiotics contaminating the skin during the administration of treatment. A selected group of 142 hospital "wet" workers with hand eczema was studied. 91% were female. Atopy (58%), metal dermatitis (41%), and hand eczema prior to the current wet work (67%) were more common than in the total cohort to which these patients belonged. Patch and photopatch test studies in 23 patients with reactions to sunscreen preparations revealed 17 photopatch and 38 patch test-positive reactions. The offending allergens could be traced in 18 of the 23 patients. Allergy to PABA was demonstrated in 11 patients, 5 of whom had photocontact allergy. Picryl chloride applied to the ears of Swiss mice induced a clearcut primary irritation inflammation (maximal after 3 to 6 hr) and after contact sensitization performed 7 days before a delayed hypersensitivity reaction. Five cases of contact sensitivity to topical corticosteroids are described. Two of the five patients presented with chronic hand eczema that was nonresponsive to topical steroid therapy. The others had eruptions suggestive of contact dermatitis. One patient was allergic only to amcinonide , while the other reacted to multiple corticosteroids.
