Pigmented cosmetic dermatitis and coal tar dyes.

Patch tests and photopatch tests with various cosmetics, coal tar dyes, perfumes, lanolin preparations and parabens-mix were performed in 53 patients with pigmented cosmetic dermatitis similar in clinical appearance to Riehl's melanosis. The results showed that coal tar dyes, particularly brilliant lake red R, were closely related to the cause of the disease. In patch testing and photopatch testing with coal tar dyes, the incidence of positive responses to the brilliant lake red R was found to be significantly higher than that to the other coal tar colors (22/38 = 57.9%). The relevance of ultr

Immune responses to environmental antigens that act on the skin: the role of lymphokines in contact dermatitis.

Immune responses to enviornmental agents affecting the skin may take various clinical forms, among which contact dermatitis is the most prominent representative of delayed-type hypersensitivity. Whereas in industrialized countries a relatively restricted amount of chemical agents is responsible for the majority of contact dermatitis cases, other factors from the environment such as natural flora, seasonal or nutritional factors may also play a role. Like other immune responses, contact dermatitis is strongly influenced by genetic factors and the existence of immune response genes, in part link

Efficacy of bufexamac (NFN) cream in skin diseases. A double-blind multicentre trial.

The effect of a new non-steroid anti-inflammatory substance (bufexamac) in a special constituent was compared with that of 0.1% triamcinolone acetonide, 1% hydrocortisone cream, and placebo during a double-blind multicentre trial. The clinical effect of these four creams was studied in 193 patients receiving treatment for the following skin disorders: atopic dermatitis, allergic contact dermatitis, and non-allergic contact dermatitis, as well as dermatitis seborrheica. After 2 and 4 weeks' treatment, when 193 and 157 patients, respectively, were re-examined, the effect of triamcinolone acetoni

[Leg ulcer and allergic eczematous contact dermatitis incidence of contact allergies induced by topical therapy (author's transl)].

In the IInd Department of Dermatology, Vienna University, 200 patients (143 female, 57 male) suffering from leg ulcers due to chronic venous insufficiency underwent patch testing with routine allergens, vehicles and their ingredients, as well as topical antibiotics. 114 patients (57%) had contact allergy. Balsam of Peru heads the list of allergens with postive reactions in 22%, closely followed by oil/water emulsions such as Drointin (21%), Ultrasicc (20%) and Ung. emulsificans aqu. (15,5%). The causative agents in these preparations were emulsifiers on the one hand (Lanette O, E, N) and prese

Health hazards to timber and forestry workers from the Douglas fir tussock moth.

A heavy infestation of the tussock moth caterpillar (Orgyia pseudotsugata McDonnough) in forested areas of Oregon was associated with itching of the skin and eyes, nasal discharge, cough, and, at times, respiratory difficulty, Personal interviews and inspection of forty-one occupationally exposed persons were supplemented by a questionnaire administered to 428 individuals, composing three groups at various degrees of risk and a control group. There was a cause and effect relationship between the adverse symptoms and the exposure to tussock moth larvae. The specific etiologic agent was not iden

Temperature dependent primary irritant dermatitis from lemon perfume.

In a recent outbreak of hand eczema amongst cleaning personnel after the introduction of a new, lemon-scented detergent, it was noted that the patients complained of a burning, stinging sensation when their hands were submerged in hot detergent solutions. Since routine patch testing with the Standard Series and perfume components was of no help in pinpointing the responsible agent, testing with selected perfume components was repeated at higher temperatures. Identical tests were placed on both forearms for 20 min, one arm being exposed to 43 degrees C, the other to 23-25 degrees C. Little or n

The effect of cyclophosphamide and irradiation on cells which suppress contact sensitivity in the mouse.

Contact sensitivity was produced in mice by painting the skin with picryl chloride and was assessed by the increase in ear thickness following local challenge. Contact sensitivity was passively transferred by immune lymph node and spleen cells taken at 4 days. The mice were then challenged immediately and the reactions read at 24 and 48 hr. Immune lymph node and spleen cells taken at day 8 virtually fail to transfer. Experiment showed that they contain cells which suppress passive transfer. These are demonstrated by mixing approximately equal numbers of 4-day cells, which transfer contact sens

Allergy to ingredients of vehicles.

Common ingredients of vehicles such as perfumes, antibacterial agents, emulsifiers and other surface active agents, propylene glycol, lanolin and wool alcohols were tested in eczema patients over a three-year period. Perfume allergy was detected in 3.6% of the cases, sensitivity to thiomersal in 2%, to sorbic acid in 0.8%, to parabens in only 0.3%, and to wool alcohols in 1.2%. Reactions to emulsifiers were seen over 1% of those tested.

[New method of diagnosis of drug hypersensitivity of the delayed type--the scarificaiton-film test].

An original method for diagnosis of drug allergy, i.e. a scarification-film test (SFT) in 2 variants has been worked out for the first itme on the basis of examination of 241 persons and is recommended for use. 10 mm parallel scarifications with intervals of 3-4 mm between them were made on the epidermis of the antibrachial flexor and the following agents were applied to them: 1) a film-forming aerosol composition with medicines (tetracycline)--the 1st variant of SFT. (2) powder or solutions of medicines (antibiotics, sulfanylamides, vitamins, etc.) covered with a film-forming ocmposition with

Contact dermatitis from propolis.

Two patients with contact dermatitis due to the natural product propolis (bee glue) are reported. They presented perioral eczema and stomatitis which were recalcitrant until propolis was considered as the cause. Patch tests with propolis preparations were positive in both patients, and, furthermore, in the second patient the lesions relapsed after provocation tests. European standard patch test including balsam of Peru were negative. The complexity of propolis, its supposed anti-inflammatory effect due to flavonoids, and the sensitizing agents originating mainly from the poplar trees are discu

Allergic contact dermatitis and conjunctivitis to corticosteroids.

This is a report of two patients suspected of having allergic dermato-conjunctivitis due to corticosteroids. Both were tested with the constituents of the ointments they were using, that is, corticosteroids, antibacterial agents, preservatives, and other active ingredients of the ointment bases. Both patients showed positive allergic patch test reactions to the respective corticosteroid betamethasone valerate, and one to hydrocortisone, neomycin, and a rubber additive. The importance of routine patch testing in ophthalmic practice to detect sensitizers in cases of allergy is mentioned. Hydroco

The treatment of hypermelanosis with 4-isopropylcatechol.

Over the past 6 years sixty-eight patients have been treated with 1% or 3% 4-isopropylcatechol (4-IPC). Fifty-four had melasma, the others a variety of disorders of pigmentation. Two-thirds of the patients were significantly improved. Twenty patients had skin irritation due to 4-IPC and four developed an allergic contact dermatitis. One patient developed confetti-like areas of depigmentation in the 4-IPC-treated areas. Light and electron microscopic studies showed that in the 4-IPC-treated areas there was a loss and damage of the melanocytes, but the keratinocytes and Langerhans cells were una

Occupational dermatosis due to a soil fumigant.

D-D mixture is a preplant soil fumigant. It contains dichloropropanes and dichloropropenes, epichlorohydrin and is known as a primary irritant. Three cases of untoward reaction to D-D are reported. By patch testing, the existence of a contact allergic sensitivity to D-D could be proved in one patient. The other two cases reacted negatively; they were probably of a primary irritant origin. Patch tests with compounds related to D-D suggest that the cause of contact allergy must be sought in the propene(s) fraction of D-D. In future cases of untoward reactions to D-D, patch testing appears to be

Contact sensitivity to emulsifiers.

Common emulsifiers were tested in over 1,200 patients with eczema. Triethanolamine stearate tested at 5% in petrolatum caused irritant reactions in 9.5% of the patients. On the other hand, non-ionic emulsifying agents tested at 10-20% produced irritation in only a few cases. Allergic reactions were found in 2.1% of those tested. Lanette, sorbitan sesquioleate, the Spans, polyoxyethylene oxypropylene stearate, polyoxyethylene sorbitol lanolin derivative, and triethanolamine stearate each elicited allergic reactions in 0.3-0.7% of the cases. The Tweens caused an allergy in only two cases, but gl

Sensitizing and irritating properties of star anise oil.

Star anise oil in 2 and 1% concentrations produced active sensitization in 5% of test subjects, and positive patch tests in 36 and 34%, respectively, of consecutive patients with dermatitis. A comparison of the results of tests with balsams and other essential oils showed that star anise oil does not give cross-reactions and pseudo cross-sensitivity. Patients positive to this oil are frequently positive to anethole and to other constituents of this oil: alpha-pinene, limonene and safrole. In tests with star anise oil it is noteworthy that a 1% concentration is strongly irritant, and 0.5% revea

Treatment of cutaneous ulcers with benzoyl peroxide.

Benzoyl peroxide, a powerful organic oxidizing agent, was applied topically according to a carefully developed technique to cutaneous ulcers of different types. The healing time was shortened greatly by the rapid development of healthy granulation tissue and the quick ingrowth of epithelium. Exceptionally large pressure ulcers with deep cavities, undercut edges and sinus tracts were sucessfully treated, as were stasis ulcers of long duration resistant to all other therapy. There were only 13 treatment failures among the 133 cases. The slow, sustained release of oxygen by benzoyl peroxide was t

The anti-inflammatory activity of MS-1112, a new steroid compound, after topical application and systemical administration was examined by means of delayed-type hypersensitivity in mice and compared with other glucocorticoid analogues, betamethasone-17-valerate (Val), dexamethasone (Dexa) and betamethasone (Beta). In the case of pretreatment and therapeutic effect, topically applied MS-1112 was most active in inhibiting the delayed-type hypersensitivity induced by picryl chloride. The order of those inhibitory activities of MS-1112 and Val was ointment greater than cream greater than gel. In c

Early morphologic alterations in mouse skin after topical application of 3-methylcholanthrene and its metabolites.

The effects of topical administration of 3-methylcholanthrene (MCA) or its metabolites on BALB/cKi mice were reported on inflammatory skin reactions, the alterations in epidermal thickness, the number of nucleated cells, pyknotic nuclei and/or nuclear fragments, and mitotic figures in the interfollicular epidermis (IFE). In the two-stage carcinogenesis system, MCA, the powerful complete carcinogen, induced an ordered sequence of cell changes strikingly similar to those caused by tumor-promoting agents such as the phorbol esters. These changes were absent after application of the "K-region" oxi

Handwashing practices for the prevention of nosocomial infections.

Handwashing is generally considered the most important procedure in preventing nosocomial infections, because many types of these infections may be caused by organisms transmitted on the hands of personnel. Personnel should wash their hands before and after significant contact with any patient. The risk of personnel acquiring transient hand carriage of organisms is usually greatest after contact with excretions, secretions, or blood; patients at greatest risk are those undergoing surgery, those with catheters, and newborn infants. Although handwashing with an antiseptic agent between patient c
