6 duplicate floor pens with 20-day-old commercial broiler chicks each, were fed practical-type broiler diet with maize-palm kernel meal, supplemented with graded levels of biotin so that the rations had 40, 80, 120, 160, 200, and 240 micrograms of the vitamin per kg of feed. More than half of nickel-allergic patients develop secondary eruptions, that is, dermatitis in areas of the skin that are not in obvious contact with nickel-plated items. Similar eruptions occur after placebo-controlled oral challenge with inorganic nickel salts. The possible relationship between ingestion of nickel in food and secondary eruptions in nickel-sensitive patients is discussed, making reference to the available literature. Recommendations are given for the management of patients suspected to have nickel dermatitis aggravated by nickel in food.

We have prospectively followed 57 children of atopic parents up to 5 years of age, documenting clinical atopic disease and allergen skin test reactions. The cumulative prevalences of the clinical features of atopic disease over the 5 years were: atopic dermatitis (58%), wheeze (49%), recurrent wheeze (33%), rhinitis (68%) and immediate food reactions (18%). Recent reports suggest the high frequency of nickel induced allergic contact dermatitis and the absence in some patients of a clinical improvement following topical avoidance of sensitising allergen. In this study we evaluated the role of food ingested nickel in the maintenance and exacerbations of the disease. We have selected 13 babies presenting with atopic dermatitis during exclusive breast feeding. After evaluation of reaginic reaction, we detected beta-lactoglobulin (BLG) in breast milk and babies sera before and after maternal elimination diet and after maternal challenge. beta-Lactoglobulin was present only in the sera of infants ingesting breast milk containing BLG. Urticaria can be caused by a multitude of factors, both internal (eg, ingested drugs or foods) and external (eg, skin contact, heat, cold). Skin testing, ice-cube testing, skin biopsy, and food-challenge testing are some of the methods that can help link cause and effect. Acute urticaria may require the administration of epinephrine and diphenhydramine or a lowering of the antigen dose in an allergy shot. Chronic urticaria is plainly visible in clinical presentation but is more elusive in diagnosis and treatment. It constantly challenges the physician's investigative acumen.

The case report is presented of a 34-year old man with the sudden appearance of multiple granulomas at the site of green tattoo markings which had been undertaken 8 years previously. Unusual was his case history of an anaphylactic reaction after the ingestion of green pistachio nuts.
