Intolerance reactions of the skin in which a decision is made on allergic and toxic genesis have increased in significance in recent years. Whereas antibodies blocking bacterial or viral antigens are formed in the normergic immune reaction, nonblocking antibodies to animal, plant or chemical heterologous substances are formed in an allergy as a result of an misdirected immunological reaction. Sensitization is favored by a) large structure and protein affinity of the allergen, b) irritation of the affected part of the skin by fungi or chemicals and c) genetic predisposition. Depending on the cl

Thirty seven breast fed infants with eczema were studied to see whether changes in their mothers' diets affected their skin condition. Nineteen mothers and babies took part in a double blind crossover trial of exclusion of egg and cows' milk, and 18 took part in open exclusion of 11 foods followed by double blind challenge to those mothers whose infants seemed to respond. Babies were examined at the beginning and end of each dietary period, and the extent and severity of the rash were given a numerical score. The eczema improved in six infants when their mothers avoided egg and cows' milk and

In a study of the manifestations of cow milk allergy in 100 young children (mean age 16 months), 30 items of historical data and information relating to the effects of a standardized milk challenge were entered into a computer data base. Three clusters of patients were derived using a K-means algorithm. In group 1 were 27 patients with predominantly urticarial and angioedematous eruptions, which developed within 45 minutes of ingesting cow milk. They had positive skin test reactions to milk and elevated total and milk specific IgE serum antibody levels. In group 2, 53 patients had pallor, vomi

Forty nine eczematous infants who were still solely and exclusively breast fed and who had never received anything but breast milk were studied for evidence of sensitisation to foods. Thirty four similar infants without eczema formed a control group. The eczematous infants were divided into three groups according to clinical criteria: (1) definite atopic eczema; (2) possible atopic eczema; (3) atopic eczema unlikely. Twenty three infants showed cutaneous hypersensitivity to foods, usually egg and cows' milk. Seven of 14 infants in group 1 and nine of 20 in group 2 were sensitised compared with

Thirty-seven patients with moderate or severe persistent atopic dermatitis gave a history of skin reactions following the ingestion of certain foods. These reactions fell into three groups: immediate reactions (within one hour) which included angio-oedema, contact urticaria, generalised itching and erythema, or urticaria; late reactions, where patients experienced late urticaria, late angio-oedema, itching, or exacerbation of the dermatitis; dual reactions where immediate reactions were followed by exacerbation of the atopic dermatitis. The foods implicated were either potential allergens or r

Staphylococcus aureus produces many extracellular products often referred to as toxins, some with definite disease-causing potential. The enterotoxins A through E are common causes of acute food poisoning characterized by a short incubation period after ingestion of performed toxin followed by nausea, vomiting, abdominal pain, and diarrhea. The epidermolytic toxins (A, B) are absorbed from a local site of colonization or infection and affect the granular cell layer of skin to cause the painful erythroderma and desquamation of the scalded skin syndrome. Other unique S. aureus strains produce on

We examined the development of skin test reactivity and clinical allergen sensitivity in infancy. Seventy-eight infants of atopic parents were skin prick tested every 4 mo from 4 to 16 mo and an additional 57 of these infants were tested at 20 mo. Wheal diameters were recorded for histamine (1 mg/ml) and specific allergen reactions by use of cow's milk, egg albumen, wheat, and Dermatophagoides pteronyssinus. The histamine mean wheal diameter was significantly lower at 4 and 8 mo compared to the older infants. Infants at 20 mo also had significantly smaller wheals than adult controls. Histamine

Between April 4 and May 10, 1982, fifty-four individuals developed a poison ivy-like dermatitis 1 to 8 days after eating imported cashew nuts. The patients had a very pruritic, erythematous, maculopapular eruption that was accentuated in the flexural areas of the body. Three had blistering of the mouth and four had rectal itching. Nineteen volunteers (eleven ill and eight well) were patch-tested with 2.5 micrograms of poison ivy urushiol and an acetone extract of cashew nut shells. Patch testing did not predict illness since positive tests to both materials occurred in those who had been ill a

The individual daily intake of gluten was calculated in 45 patients with dermatitis herpetiformis (DH) on the basis of a depth interview about food habits. Gastric and small intestinal morphology and function were studied concurrently. Mean daily gluten intake was estimated to be 15 g, a figure which corresponds well to the average gluten intake in Sweden. There was a significant correlation between the degree of morphological mucosal changes of the small intestine and the quantity of gluten ingested. All patients with jejunal villous atrophy consumed more than 10 g gluten daily and all but on

The presence and levels of furocoumarins in several parts of Ficus carica including the milky sap, were investigated. The results show that psoralen and bergapten are the only significant photoactive compounds, and are present in appreciable quantities in the leaf and shoot sap but are not detected in the fruit or its sap. These compounds are more concentrated in the leaf sap compared to the shoot sap. The psoralen levels are several times higher than those of bergapten. Lower concentrations of both compounds are present in autumn compared to spring and summer. These findings suggest that the

The effects of ingestion of cow's milk in young children with cow's milk allergy (CMA) is reported, following a recently concluded study of 100 infants and young children who were challenged and observed in a hospital environment. The existence of various immediate manifestations of this disease, and those that develop over hours or days following milk exposure, are highlighted. Attempts to define different syndromes, with the aim of aiding the clinical recognition of CMA, are also reported.

A major aetiologic role for foods has been demonstrated in urticaria, atopic eczema and dermatitis herpetiformis. In some patients with urticaria, whealing occurs within minutes of the ingestion of a particular food. In most, but not necessarily all cases, this appears to be a consequence of IgE-mediated cutaneous mast cell degranulation, i.e. a classical type I hypersensitivity response. In other patients with recurrent urticaria, the whealing may be provoked by foods by a much slower and more insidious reaction. This type of reaction has been established in the case of several common food ad

Rhesus monkeys were fed a diet marginally deficient in zinc (4 ppm zinc) throughout pregnancy and were monitored for changes in hematological, biochemical, and immunological parameters. This dietary zinc level was chosen because it did not produce an overt deficiency syndrome when fed for 10 wk to nonpregnant monkeys. Deprived animals were compared to control groups fed a zinc replete (100 ppm) diet ad libitum or on a food restricted (pair fed) basis. Beginning in the 3rd trimester zinc-deprived monkeys exhibited characteristic signs of deficiency including dermatitis, anorexia, and low levels

Both upper and lower respiratory tracts can be affected by food allergy. Manifestations in either may be exclusively due to food allergy (common in infants) or may result from the combined effects of food allergy plus another defect such as gastroesophageal reflux, a congenital defect of the heart or tracheo-bronchial tree, an immunodeficiency syndrome such as isolated IgA or IgG4 deficiency, or a concomitant inhalant allergy. Chronic rhinitis is the most common respiratory tract manifestation of food allergy. When it occurs in conjunction with lung disease, it may be a helpful indicator of ac

One of the more frequent problems in medicine is the question of a possible association between ingestion of a particular food or drug, and symptoms experienced by a patient. Food allergens are mainly cow's milk, egg, fish, nuts and fruits. Ingested antigen/allergen may induce antibody production and thus sensitization. But the presence of specific antibodies or a positive skin test are not synonymous with allergic symptoms. The convincing diagnostic procedure in adverse reactions to foods or drugs is a controlled exposition. The thrombocytopenia test is a useful addition. Intestinal biopsy is

In order to understand why non-atopic people do not have adverse symptoms to food antigens which enter the circulation after eating, 8 non-atopic and 10 atopic eczema- and milk-allergic subjects were challenged with milk, and the types of circulating immune complexes formed were analysed. Although the amount of beta-lactoglobulin incorporated into complexes did not differ statistically between the groups, the type of immune complex did. Of the non-atopic individuals, 5 formed IgA and 2 IgG complexes. Of the milk-allergic group, all showed a rise in at least one type; 5 formed IgA, 7 IgG, 6 IgE

A woman who had ingested honeybee royal jelly as a nutrient, showed an exacerbation of dermatitis when it was applied to her feet. A topical fungicide also aggravated her skin lesions. Patch testing showed positive reactions to the royal jelly, pyrrolnitrin in the fungicide and urushiol. Positive reactions to the royal jelly were found in 2 out of 10 controls, 1 of whom was sensitive to propolis.

Comparative study of 13 patients (group A) with severe attacks of angio-oedema and urticaria after eating celery and 143 atopics with hypersensitivity to mugwort-pollen (group B). Due to the severity of the reactions (group A) no oral provocation tests were performed. Scratch chamber tests with raw and cooked celery (and related plants) confirmed the case histories. RAST answers to celery were compared for both groups. RAST inhibition indicated that celery and mugwort-pollen and also celery and birch-pollen allergens cross-react. Whether or not the reactions (in group A) are type I allergic or

The specificity of immunoglobulin E for different cereal grain proteins was investigated using sera from twenty paediatric patients with asthma and/or eczema. Close correlations were observed between radioallergosorbent test values for grain extracts of wheat, rye and barley, and, to a lesser extent, oats. Of the different wheat four fractions tested, the globulins and glutenins consistently bound higher levels of IgE than the gliadins and albumins. This is in contrast both with bakers' asthma (an allergy to inhaled flour where the albumins are important allergens) and with coeliac disease (in
